Indications for the addition of a patellofemoral joint arthroplasty following a previous unicondylar knee arthroplasty- a literature review and Delphi consensus.

Introduction: The aim of this study was to establish an international consensus statement on the indications for the addition of a patellofemoral joint arthroplasty (PFJA) in patients with a unicondylar knee arthroplasty (UKA) and symptomatic progression of patellofemoral compartment osteoarthritis.

Materials And Methods: A systematic review of the literature was conducted, and the results used to inform the development of a statement by an expert working group. This was then evaluated and modified, using a Delphi process, by members of the European Knee Society (EKS).

Glycated Haemoglobin (HbA) in Cardiac Surgery: A Narrative Review.

Perioperative dysglycaemia in cardiac surgery is associated with poor outcomes. Glycaemic variability rather than glucose levels is a predictor of the length of an ICU stay, a rise in creatinine and acute kidney injury after cardiac surgery. Glycated haemoglobin (HbA) values correspond closely to average blood glucose levels and cut-off values can be used to define a diabetic and pre-diabetic status.

A Prospective Observational Cohort Study Comparing High-Complexity Against Conventional Pelvic Exenteration Surgery.

Conventional pelvic exenteration (PE) comprises the removal of all or most central pelvic organs and is established in clinical practise. Previously, tumours involving bone or lateral sidewall structures were deemed inoperable due to associated morbidity, mortality, and poor oncological outcomes. Recently however high-complexity PE is increasingly described and is defined as encompassing conventional PE with the additional resection of bone or pelvic sidewall structures.

Current Status of Neoadjuvant Treatment Before Surgery in High-Risk Localized Prostate Cancer.

Localized high-risk (HR) prostate cancer (PCa) is a heterogeneous disease whose likelihood of a biochemical recurrence, metastatic progression and cancer-related mortality after initial treatment is higher when compared with patients with low (LR) or intermediate-risk (IR) disease. In the past, neoadjuvant therapy has shown an improvement in postoperative oncological variables but failed to demonstrate any survival advantages. With the promising results from novel treatments in metastatic and non-metastatic castration resistant PCa settings, new evidence has appeared in the literature in the neoadjuvant setting.

The Molecular Biology of Placental Transport of Calcium to the Human Foetus.

From fertilisation to delivery, calcium must be transported into and within the foetoplacental unit for intracellular signalling. This requires very rapid, precisely located Ca transfers. In addition, from around the eighth week of gestation, increasing amounts of calcium must be routed directly from maternal blood to the foetus for bone mineralisation through a flow-through system, which does not impact the intracellular Ca concentration.

Management of environmental impacts of fossil fuel use in refugee camps through transition to renewable energy infrastructure: Case studies in Uganda and Bangladesh.

Many refugee camps exist for decades but associated infrastructure needs are only planned for the very short term, including provision of power. This study advocates a shift in approach to sustainable electrification of essential services in refugee camps for lighting, refrigeration, health, water, education, alongside camp operations. Qualitative and quantitative surveys were conducted in refugee camps in Uganda and Bangladesh which assessed the electrical supply needs across such categories.

Women with persistent/recurrent vaginal discharge should be offered multiplex-7 PCR testing.

Background: Recurrent vaginal discharge is an important cause of repeated visits to the gynaecologists. Failure to correctly identify the causative organism with standard microscopy and culture techniques results in repeated unsuccessful treatment and the risk of developing antibiotic resistance. Multiplex PCR test is increasingly being used for investigating infections where multiple organisms may be involved.

A Comparative Assessment of Clinical Outcomes Using Cyanoacrylate Glue versus Subcuticular Sutures in Cosmetic Procedures at a Tertiary Care Center: Evaluation Based on the Southampton Scoring System.

Introduction Cosmetic surgery has advanced significantly, with wound closure techniques crucial for determining aesthetic and healing outcomes. Recently, cyanoacrylate glue and subcuticular sutures have gained attention for their unique benefits in cosmetic procedures. Cyanoacrylate glue, a non-invasive tissue adhesive, facilitates faster wound closure with minimal trauma, while subcuticular sutures offer durable, concealed closures, particularly suited for areas under mechanical stress.

Evaluating ADHD medication trial representativeness: a Swedish population-based study comparing hypothetically trial-eligible and trial-ineligible individuals.

Background: Randomised controlled trials (RCTs) evaluating ADHD medications often use strict eligibility criteria, potentially limiting generalisability to patients in real-world clinical settings. We aimed to identify the proportion of individuals with ADHD who would be ineligible for medication RCTs and evaluate differences in treatment patterns and clinical and functional outcomes between RCT-eligible and RCT-ineligible individuals.

Methods: We used multiple Swedish national registries to identify individuals with ADHD, aged at least 4 years at the age of diagnosis, initiating pharmacological treatment between Jan 1, 2007, and Dec 31, 2019, with follow-up up to Dec 31, 2020.

Improving Recall Accuracy in Sparse Associative Memories That Use Neurogenesis.

The creation of future low-power neuromorphic solutions requires specialist spiking neural network (SNN) algorithms that are optimized for neuromorphic settings. One such algorithmic challenge is the ability to recall learned patterns from their noisy variants. Solutions to this problem may be required to memorize vast numbers of patterns based on limited training data and subsequently recall the patterns in the presence of noise.

Quantitative imaging of loop extruders rebuilding interphase genome architecture after mitosis.

How cells establish the interphase genome organization after mitosis is incompletely understood. Using quantitative and super-resolution microscopy, we show that the transition from a Condensin to a Cohesin-based genome organization occurs dynamically over 2 h. While a significant fraction of Condensins remains chromatin-bound until early G1, Cohesin-STAG1 and its boundary factor CTCF are rapidly imported into daughter nuclei in telophase, immediately bind chromosomes as individual complexes, and are sufficient to build the first interphase TAD structures.

Association of Anaesthetists guidelines: the use of blood components and their alternatives 2024.

Background: The administration of blood components and their alternatives can be lifesaving. Anaemia, bleeding and transfusion are all associated with poor peri-operative outcomes. Considerable changes in the approaches to optimal use of blood components and their alternatives, driven by the findings of large randomised controlled trials and improved haemovigilance, have become apparent over the past decade.

Introduction: Glucagon-like peptide-1 receptor agonists, dual glucose-dependent insulinotropic peptide receptor agonists and sodium-glucose cotransporter-2 inhibitors are used increasingly in patients receiving peri-operative care. These drugs may be associated with risks of peri-operative pulmonary aspiration or euglycaemic ketoacidosis. We produced a consensus statement for the peri-operative management of adults taking these drugs.

Emergency and postoperative access to critical and enhanced care: a multicentre prospective observational study.

Introduction: The ability to admit patients to enhanced or critical care may be limited by bed availability. In a network with low provision of critical and enhanced care beds, we aimed to assess the proportion of patients having surgery with moderate (1%-< 5%) or high (≥ 5%) predicted risk of 30-day postoperative mortality and their postoperative care location. We also aimed to study referral and admission outcomes to critical care.

One-quarter of freshwater fauna threatened with extinction.

Freshwater ecosystems are highly biodiverse and important for livelihoods and economic development, but are under substantial stress. To date, comprehensive global assessments of extinction risk have not included any speciose groups primarily living in freshwaters. Consequently, data from predominantly terrestrial tetrapods are used to guide environmental policy and conservation prioritization, whereas recent proposals for target setting in freshwaters use abiotic factors.

An off-chip platform for on-demand, single-target encapsulation for ultrasensitive biomarker detection.

Closed-channel microfluidic systems offer versatile on-chip capabilities for bioanalysis but often face complex fabrication and operational challenges. In contrast, free-boundary off-chip microfluidic platforms are relatively simple to fabricate and operate but lack the ability to perform complex tasks such as on-demand single-target sorting and encapsulation. To address these challenges, we develop an off-chip platform powered by a fluorescent-activated mechanical droplet sorting and production (FAM-DSP) system.

Patients' Use of e-Consultations as an Alternative to Other General Practitioner Services: Cross-Sectional Survey Study.

Background: e-Consultations, defined as asynchronous text-based messaging, have transformed how patients interact with their general practitioner (GP). While e-consultations can improve patient access to GP care, concerns about increased workload for GPs are raised.

Objective: This study aimed to address three research questions: (1) For what purpose and with what expectations do patients initiate e-consultations? (2) If e-consultations had not been available, what alternative actions would the patient have taken? and (3) How are the alternative actions associated with patient and e-consultation characteristics?

Methods: A cross-sectional study was conducted through a web-based survey on Helsenorge.

Is there an association between birth characteristics and fractures in young adults? The HUNT Study, Norway.

Unlabelled: This population study investigated the association between birth characteristics and fracture risk in 11,099 young adults (aged 19-54 years). Our findings indicate that birth weight, gestational age, and birth weight for gestational age were not associated with fractures in the wrist, humerus, hip, and spine in this population.

Purpose: Skeletal development starts during fetal life, and it is estimated that most bone formation occurs in the 3rd trimester.
