In response to the evolving needs of knowledge synthesis, this manuscript introduces the concept of narrative reanalysis, a method that refines data from initial reviews, such as systematic and reviews, to focus on specific sub-phenomena. Unlike traditional narrative reviews, which lack the methodological rigor of systematic reviews and are broader in scope, our methodological framework for narrative reanalysis applies a structured, systematic framework to the interpretation of existing data. This approach enables a focused investigation of nuanced topics within a broader dataset, enhancing understanding and generating new insights.

Clerkship directors must balance the mental wellbeing of their medical students with the demanding schedule that rotations in procedural specialties such as surgery and obstetrics and gynecology require. In this paper, the Undergraduate Medical Education Committee of the Association of Professors of Obstetrics and Gynecology argues the importance of maintaining adequate clinical exposure for learners. Involving students in overnight call provides additional clinical involvement, improved relationships with the clinical team, and a better perspective on specialist lifestyle.

  • Granuloma annulare is a harmless inflammatory skin condition with unknown causes, mainly showing up as red ring-like patches on limbs.
  • A 59-year-old woman with severe, hard-to-treat generalized granuloma annulare responded well to adalimumab, requiring ongoing treatment for 2 years.
  • The case supports adalimumab's role as a long-term treatment option for stubborn cases of generalized granuloma annulare, showing significant lesion improvement within 2 months.

Autoimmune encephalitis is increasingly recognized as a neurologic cause of acute mental status changes with similar prevalence to infectious encephalitis. Despite rising awareness, approaches to diagnosis remain inconsistent and evidence for optimal treatment is limited. The following Canadian guidelines represent a consensus and evidence (where available) based approach to both the diagnosis and treatment of adult patients with autoimmune encephalitis.

Article Synopsis
  • Clerkship grades in obstetrics and gynecology are critical for residency applications, and this study explored grading practices among clerkship directors after two decades of lack of analysis.
  • A survey of 174 clerkship directors revealed diverse grading systems, with a majority using 4-tiered systems, and significant findings about student evaluations linked to unconscious bias and representation.
  • The study concludes that there's a need for more consistent grading practices and suggests measures like removing exam score thresholds and enhancing faculty training to address inequities in evaluations.

Background: Emerging data suggest that direct oral anticoagulants may be a suitable choice for anticoagulation for cerebral venous thrombosis (CVT). However, conducting high-quality trials in CVT is challenging as it is a rare disease with low rates of adverse outcomes such as major bleeding and functional dependence. To facilitate the design of future CVT trials, SECRET (Study of Rivaroxaban for Cerebral Venous Thrombosis) assessed (1) the feasibility of recruitment, (2) the safety of rivaroxaban compared with standard-of-care anticoagulation, and (3) patient-centered functional outcomes.

First Nation people residing in rural and remote communities have limited primary healthcare access, which often affects chronic disease management leading to poor health outcomes. Individuals with lived experiences of chronic disease and substance use, along with health directors, advocated for improved services. Subsequently, an urban healthcare team in partnership with four First Nation communities developed an Outreach clinic to address healthcare access barriers.

Background: Incomplete reperfusion (IR) after mechanical thrombectomy (MT) can be a consequence of residual occlusion, no-reflow phenomenon, or collateral counterpressure. Data on the impact of these phenomena on clinical outcome are limited.

Methods: Patients from the ESCAPE-NA1 trial with IR (expanded Thrombolysis In Cerebral Infarction (eTICI) 2b) were compared with those with complete or near-complete reperfusion (eTICI 2c-3) on the final angiography run.

Respiratory System Dynamics.

Semin Respir Crit Care Med

October 2023

Division of Respirology, Critical Care and Sleep Medicine, University of Saskatchewan College of Medicine, Saskatoon Saskatchewan, Canada.

While static mechanical forces govern resting lung volumes, dynamic forces determine tidal breathing, airflow, and changes in airflow and lung volume during normal and abnormal breathing. This section will examine the mechanisms, measurement methodology, and interpretation of the dynamic changes in airflow and lung volume that occur in health and disease. We will first examine how the total work of breathing can be described by the parameters of the equation of motion, which determine the pressure required to move air into and out of the lung.

Article Synopsis
  • - The Early Warning System (EWS) aims to detect early signs of clinical deterioration in hospitalized patients by using vital signs data to improve communication between healthcare teams and initiate timely interventions.
  • - A quality improvement project was conducted in a tertiary hospital, analyzing data pre- and post-EWS implementation to understand its effectiveness and address challenges faced during its rollout.
  • - The results showed a slight decrease in the total number of adverse events per week post-EWS implementation, suggesting that the standardised protocol was effective in recognizing patient deterioration.

Growth patterns and associated endocrine profiles were compared between dominant anovulatory (ADF) and ovulatory follicles (OvF) developing from different waves within and between menstrual cycles in women. Follicular mapping profiles of 49 healthy women of reproductive age and blood samples were obtained every 1-3 days. Sixty-three dominant follicles were classified into wave 1 (W1ADF; n = 8) and wave 2 (W2ADF; n = 6) anovulatory follicles and wave 2 (W2OvF; n = 33) and wave 3 (W3OvF; n = 16) ovulatory follicles.
