Background: Hypertension management is a national priority. However, hypertension control rates are suboptimal and vary across clinics, even among those in the same health system and geographic region.

Objective: To identify organizational barriers and facilitators that impact hypertension management at the provider, clinic, and health system level.

Analysis of Orthopaedic Surgery Residency Match Results: 2016 to 2022.

: Objective data examining match trends and applicant characteristics are in high demand given the stakes involved with the orthopaedic surgery match. Our study sought to analyse the trends among matched orthopaedic surgery applicants to clarify how medical students can best prepare for and assess their chances of a successful orthopaedic residency match. : We used the publicly available National Residency Matching Program (NRMP) tool to gather characteristics for matched and unmatched orthopaedic surgery applicants from 2016 to 2022 and compare the probability of matching for all applicants versus each subgroup of reported applicant characteristics using risk difference (RD) and two-tailed p-values.

Geographic Variation in Inpatient Rehabilitation Outcomes After Traumatic Brain Injury.

Objective: To determine whether regional variations exist in functional outcomes of patients with traumatic brain injury (TBI) admitted to inpatient rehabilitation facilities (IRFs) across the United States, while controlling for demographic and clinical variables.

Multi-Institutional Study of Multimodal Analgesia Practice, Pain Trajectories, and Recovery Trends After Spine Fusion for Idiopathic Scoliosis.

Background: Posterior spinal fusion (PSF) surgery for correction of idiopathic scoliosis is associated with chronic postsurgical pain (CPSP). In this multicenter study, we describe perioperative multimodal analgesic (MMA) management and characterize postoperative pain, disability, and quality of life over 12 months after PSF in adolescents and young adults.

Methods: Subjects (8-25 years) undergoing PSF were recruited at 6 sites in the United States between 2016 and 2023.

Improving STEMI Management Internationally: Initial Report of the American College of Cardiology-Global Heart Attack Treatment Initiative.

Background: The burden of ST-segment elevation myocardial infarction (STEMI) worldwide is increasing as the leading cause of death; there are scant data on system-based initiatives and performance metrics relative to its management in low- and middle-income countries where resources are frequently limited.

Objectives: The Global Heart Attack Treatment Initiative (GHATI) ultimate aim is improving evidence-based STEMI care, adherence to guidelines, and tracking of clinical and institutional indicators in low- and middle-income countries. To achieve that goal, the process of care and outcomes of STEMI patients in those nations will be studied.

Variability in Notification of Positive Newborn Screening Results for Sickle Cell Trait Across the United States.

Universal in the United States (US) since 2006, newborn screening (NBS) programs for sickle cell disease (SCD) allow for early identification of the disease and, as an unintentional byproduct, identification of sickle cell trait (SCT). Unlike other carrier states, SCT is highly prevalent and is found in nearly 3 million Americans, which results in important reproductive implications. Currently, all NBS programs in the US are responsible for their own policies regarding SCT notification, and little is known about how SCT notification practices are performed and how these practices vary across NBS programs.

A Collaborative Online International Learning Experience for Doctoral Nursing Students and Faculty From Three Countries: Reshaping the Educational Landscape.

The COVID-19 pandemic presented opportunities for educational innovations and the development of intercultural learning experiences. A global health assignment guided by a collaborative online international learning pedagogy was assigned to doctoral nursing students from three different countries. Icebreaker activities, along with the Culturally You diagram, commenced the team-building process.

Generalized measures of population synchrony.

Synchronized behavior among individuals, broadly defined, is a ubiquitous feature of populations. Understanding mechanisms of (de)synchronization demands meaningful, interpretable, computable quantifications of synchrony, relevant to measurements that can be made of complex, dynamic populations. Despite the importance to analyzing and modeling populations, existing notions of synchrony often lack rigorous definitions, may be specialized to a particular experimental system and/or measurement, or may have undesirable properties that limit their utility.

BCMA-directed CAR T-cell Therapy in patients with multiple myeloma and CNS involvement.

We investigated BCMA-directed CART in patients with relapsed or refractory multiple myeloma (RRMM) and CNS involvement. Ten patients who received either ide-cel (n=6) or cilta-cel (n=4) were included in this analysis. Patients had brain/cranial nerve and/or spinal cord involvement/leptomeningeal disease evident on either MRI (100%) and/or CSF (40%).

Disparities in telehealth utilization between US rural and urban areas during the COVID-19 pandemic.

Purpose: During the COVID-19 pandemic, telehealth services were expanded across the United States to meet the increased demand and safety requirements of care. This observational study aims to understand rural-urban differences in telehealth utilization during the early part of the COVID-19 pandemic.

Methods: Individual-level data from the National Health Interview Survey 2020-2021 (age ≥18) were analyzed for this study.

Efficacy of an online self-management program for chronic burn pain: A randomized controlled trial of the Take Charge of Burn Pain program.

This randomized controlled trial investigated the effectiveness of an online self-management program, "Take Charge of Burn Pain (TCBP)," for 96 individuals living with chronic burn pain. Participants were randomly assigned to either the 7-week TCBP program integrating cognitive-behavioral therapy techniques, pain education, and self-management strategies or an attention control group focused on general burn recovery information. Assessments conducted at baseline, post-treatment, and 2- and 5-month follow-ups included measures of pain severity, pain interference, pain self-efficacy, posttraumatic stress disorder symptoms, and depression.

Impact of the CAPTURE COPD Screening Tool in US Primary Care: A Cluster Randomized Trial.

The impact of COPD screening on US primary care clinician behavior and patient outcomes is unclear. Assess the impact of receiving CAPTURE (COPD Assessment in Primary Care to Identify Undiagnosed Respiratory Disease and Exacerbation Risk) screening scores on clinical and patient outcomes. Cluster randomized trial included 49 usual care (COPD education only) and 51 intervention (COPD and CAPTURE education plus screening scores) primary care practices.

Investigating the impact of whole genome duplication on transposable element evolution in teleost fishes.

Transposable elements (TEs) can make up more than 50% of any given vertebrate's genome, with substantial variability in TE composition among lineages. TE variation is often linked to changes in gene regulation, genome size, and speciation. However, the role that genome duplication events have played in generating abrupt shifts in the composition of the mobilome over macroevolutionary timescales remains unclear.

Clinical Outcomes of Early Phenotype-Desirable Antimicrobial Therapy for Enterobacterales Bacteremia.

Importance: Initiating effective therapy early is associated with improved survival among patients hospitalized with gram-negative bloodstream infections; furthermore, providing early phenotype-desirable antimicrobial therapy (PDAT; defined as receipt of a β-lactam antibiotic with the narrowest spectrum of activity to effectively treat the pathogen's phenotype) is crucial for antimicrobial stewardship. However, the timing of targeted therapy among patients hospitalized with gram-negative bloodstream infections is not well understood.

Objective: To compare the clinical outcomes between patients who were hospitalized with Enterobacterales bloodstream infections receiving early vs delayed PDAT.

Community-Engaged Insights: Understanding the Housing Needs of Rural Caregivers During Hospital Treatment.

Purpose: This study explored the experiences of rural caregivers who require temporary housing during hospital treatment when healthcare is not available in their home communities. Understanding these experiences can identify challenges and inform solutions for improving rural access to healthcare.

Methods: We conducted a community-engaged qualitative study using semi-structured interviews with caregivers who stayed at a healthcare hospitality house in Charlotte, NC.

Risks of Psychedelics Necessitate More Safety Data Before Starting Trials in Adolescents.

We read with interest the Commentary by Jeffrey et al. encouraging clinical research using psychedelics for adolescents. Throughout their commentary, the authors operate under the general presupposition that psychedelics have demonstrated a favorable safety profile in adults, and that therefore teenagers approaching an adult level of neurodevelopment can likely be safely treated with appropriate safeguards.

Scoping review of drug dosing recommendations in sustained low-efficiency dialysis.

The objective of this scoping review was to answer the question, "What has been published describing drug dosing in sustained low-efficiency dialysis (SLED)?" PubMed, Embase, and Scopus were searched on November 18, 2022. Methodology followed the Arksey and O'Malley framework for scoping reviews and Preferred Reporting Items for Systematic Reviews and Meta-Analysis Extension for Scoping Reviews guidelines. Two investigators independently screened abstracts and full-texts of citations identified related to drug dosing and SLED.

Effectiveness of ceftazidime-avibactam versus ceftolozane-tazobactam for multidrug-resistant Pseudomonas aeruginosa infections in the USA (CACTUS): a multicentre, retrospective, observational study.

Background: Ceftolozane-tazobactam and ceftazidime-avibactam are preferred treatment options for multidrug-resistant Pseudomonas aeruginosa infections; however, real-world comparative effectiveness studies are scarce. Pharmacokinetic and pharmacodynamic differences between the agents might affect clinical response rates. We aimed to compare the effectiveness of ceftolozane-tazobactam and ceftazidime-avibactam for treatment of invasive multidrug-resistant P aeruginosa infections.

Pulsed Electromagnetic Fields for Cervical Spine Fusion in Patients with Risk Factors for Pseudarthrosis.

Background: Certain demographics and/or risk factors contribute to complications following cervical spinal surgery including pseudarthrosis, prolonged pain, and reduced quality of life (QoL). Pulsed electromagnetic field (PEMF) stimulation is a non-invasive therapy that may enhance fusion success in at-risk patients.

Objective: To evaluate the safety and efficacy of post-operative adjunctive PEMF therapy following cervical spinal surgery in subjects at risk for pseudarthrosis.
