Efficacy and safety of oral NSAIDs and analgesics in the management of osteoarthritis: Evidence from real-life setting trials and surveys.

Semin Arthritis Rheum

February 2016

MRC Lifecourse Epidemiology Unit, University of Southampton, Southampton, UK; NIHR Musculoskeletal Biomedical Research Unit, University of Oxford, Oxford, UK.

Non-steroidal anti-inflammatory drugs (NSAIDs) are at the cornerstone of treatment for osteoarthritis (OA). In recent years, the widespread use of oral NSAIDs has been called into question due to the appearance of significant upper gastrointestinal (GI) complications and cardiovascular (CV) adverse events (AEs). However, NSAIDs are non-homogeneous, and there are noticeable differences between them in AE risk for GI and CV events.

January 2016

Musculoskeletal Unit, Menzies Institute for Medical Research, University of Tasmania, Medical Science 1 Building, Private Bag 23, 17-Liverpool Street, Hobart, TAS, 7000, Australia.

Article Synopsis
  • - The study examined the long-term effects of meniscal tears on knee health in middle-aged adults over 8 years, finding that 22% had meniscal tears at the beginning and 16% experienced increased severity over time.
  • - Participants with worsening meniscal tears reported greater knee pain, with the offspring of patients who had knee replacements experiencing more pain compared to controls.
  • - The findings suggest that changes in meniscal tears are linked to risk factors of knee osteoarthritis (OA) and are related to worsening pain and structural damage, indicating meniscal tears could be a factor in the development of knee OA.

Unlabelled: Knowledge translation (KT) involves implementation of evidence-based strategies and guidelines into practice to improve the process of care and health outcomes for patients. Findings from pragmatic trials may be used in KT to provide patients, healthcare providers and policymakers with information to optimize healthcare decisions based on how a given strategy or intervention performs under the real world conditions. However, pragmatic trials have been criticized for having the following problems: i) high rates of loss to follow-up; ii) nonadherence to study intervention; iii) unblinded treatment and patient self-assessment, which can potentially create bias; iv) being less perfect experiments than efficacy trials; v) sacrificing internal validity to achieve generalizability; and vi) often requiring large sample sizes to detect small treatment effects in heterogeneous populations.

Electronic address:

Introduction: The angiotensin II type 1 receptor (AT1R) is responsible for the main effects of the renin-angiotensin system (RAS), and its expression pattern is altered in several diseases. The [(11)C]methylated derivatives of the clinically used AT1R blocker (ARB) losartan and its active metabolite EXP3174, that binds with higher affinity to AT1R, were evaluated as potential PET imaging tracers in rat kidneys.

Methods: [(11)C]Methyl-losartan and [(11)C]methyl-EXP3174 were synthesized by [(11)C]methylation of the tetrazole-protected analogs using [11C]methyl iodide.

Objective: In the perspective of personalized management of osteoarthritis (OA), a clinically relevant concern is the impact of meniscal extrusion (Ext) on response to treatment. This study aimed at determining the effects of conventional OA pharmacological treatments and those of the combination of glucosamine and chondroitin sulfate (Glu/CS) on knee structural changes in the presence or absence of Ext, using data from the progression cohort of the Osteoarthritis Initiative.

Methods: In this longitudinal study, knee OA participants were stratified based on whether (+) or not (-) they received analgesics/NSAIDs (+ and -analgesics/NSAIDs) and/or Glu/CS (+ and -Glu/CS) for 24 consecutive months and on the presence (Ext+) or absence (Ext-) of medial meniscal extrusion at baseline.
