Growth in children with nephrotic syndrome: a post hoc analysis of the NEPTUNE study.

Pediatr Nephrol

September 2024

Division of Pediatric Nephrology, Northwell, Cohen Children's Medical Center, 2000 Marcus Ave, Suite 300, New Hyde Park, Northwell, NY, 11042-1069, USA.

Article Synopsis
  • Steroids, a common treatment for nephrotic syndrome in children, can negatively impact growth as shown in a study involving 318 children.
  • The study found that higher cumulative steroid exposure was linked to lower height z-scores, while new cases of nephrotic syndrome also contributed to decreased growth over time.
  • Interestingly, the use of rituximab was associated with improved height z-scores, suggesting it may have a beneficial effect on growth compared to steroids.

Despite clinical use of immunosuppressive agents, the immunopathogenesis of minimal change disease (MCD) and focal segmental glomerulosclerosis (FSGS) remains unclear. Src homology 3-binding protein 2 (SH3BP2), a scaffold protein, forms an immune signaling complex (signalosome) with 17 other proteins, including phospholipase Cγ2 (PLCγ2) and Rho-guanine nucleotide exchange factor VAV2 (VAV2). Bioinformatic analysis of human glomerular transcriptome (Nephrotic Syndrome Study Network cohort) revealed upregulated SH3BP2 in MCD and FSGS.

Background: Our previous study established a 2-dose regimen of high-dose trivalent influenza vaccine (HD-TIV) to be immunogenically superior compared to a 2-dose regimen of standard-dose quadrivalent influenza vaccine (SD-QIV) in pediatric allogeneic hematopoietic cell transplant (HCT) recipients. However, the durability of immunogenicity and the role of time post-HCT at immunization as an effect modifier are unknown.

Methods: This phase II, multi-center, double-blinded, randomized controlled trial compared HD-TIV to SD-QIV in children 3-17 years old who were 3-35 months post-allogeneic HCT, with each formulation administered twice, 28-42 days apart.

Improvement in management of pediatric renal disorders has led to patient survival rates of 85-90%, increasing the number of adolescent and young adult (AYA) patients with childhood onset chronic kidney disease (CKD) transitioning to adult care settings. Pediatric CKD patients differ from adults with CKD in view of early onset of disease (sometimes with fetal onset), different disease spectrum, the potential effect of CKD on neurodevelopment, and substantial involvement of parents in medical decision making. In addition to the usual challenges of emerging adulthood (graduation from school to work, independent living, peak in impulsivity and risk-taking behaviors), young adults with pediatric CKD need to learn to manage a serious medical condition independently.

Department of Pediatric Cardiology, Ward Family Heart Center, Children's Mercy Hospital, University of Missouri at Kansas City, Kansas City, Missouri. Electronic address:

Background: Infants with single-ventricle physiology have increased morbidity, mortality, and ventricular dysfunction in the pre-superior cavopulmonary anastomosis (pre-SCPA) period. Echocardiography-derived longitudinal strain (LS) is emerging as a reliable marker of single-ventricle performance. We aim to assess evolution of LS during the pre-SCPA period across univentricular morphologies and explore associations of LS with modifiable and nonmodifiable factors.

Transport Across Membranes: Techniques for Measuring Drug Import in Fungal Cells.

Methods Mol Biol

April 2023

Department of Cell and Molecular Biology and Biochemistry, School of Biological and Chemical Sciences, University of Missouri at Kansas City, Kansas City, MO, USA.

The ability of many antifungal molecules to traverse the fungal cell wall and accumulate within the cell is crucial to its ability to have the desired biological activity. Altered accumulation of the drug is an important mechanism of antifungal drug resistance. The best characterized mechanism for altered accumulation is through the action of the drug efflux pump which actively transports the drugs out of the membrane, although this is not the only mechanism for this phenomenon.

Transport across Membranes: Techniques for Measuring Efflux in Fungal Cells.

Methods Mol Biol

April 2023

Department of Cell and Molecular Biology and Biochemistry, School of Biological and Chemical Sciences, University of Missouri at Kansas City, Kansas City, MO, USA.

One of the most prevalent mechanisms of antifungal drug resistance is export of the molecule from the fungal cells through the action of putative efflux pumps or transporters. Drug efflux is a particularly common mechanism of resistance to azole antifungals, one of the most widely used classes of antifungal drugs. Here, we provide detailed protocols for two assays of small-molecule efflux activity: rhodamine 6G efflux and alanine-naphthylamide accumulation.

Imaging in hematopoietic stem cell transplantation patients is not targeted at evaluating the transplant per se. Rather, imaging is largely confined to evaluating peri-procedural and post-procedural complications. Alternatively, imaging may be performed to establish a baseline study for comparison should the patient develop certain post-procedural complications.
