November 2024

Specialty Chair, VA San Diego Healthcare System, San Diego, California.

Article Synopsis
  • Stress fractures, which can be fatigue or insufficiency types, commonly cause pain in various patient groups like athletes and older individuals, and are best diagnosed using radiography as the first imaging method.
  • MRI improves the detection of these fractures that might not show up on X-rays, while other techniques like nuclear medicine scintigraphy and CT can also assist in diagnosis.
  • Prompt treatment is often needed to prevent incomplete fractures from worsening, especially for serious cases like femoral neck stress fractures, and accurate diagnosis is crucial to distinguish from other serious conditions, with guidelines from the American College of Radiology helping inform best practices.

Article Synopsis
  • Two non-fusion devices, the posterior dynamic distraction device (PDDD) and vertebral body tethering system (VBT), were approved for treating adolescent idiopathic scoliosis, with no prior comparative studies published.
  • A matched study involving 20 PDDD and 20 VBT patients highlighted that PDDD had significantly lower blood loss, shorter operative time, and shorter hospital stays compared to VBT, along with better curve correction results at multiple follow-up intervals.
  • Despite its advantages, PDDD showed a higher rate of revision surgeries compared to VBT patients after two years, indicating the need for continued monitoring in terms of long-term effectiveness.

August 2024

Department of Otolaryngology-Head and Neck Surgery, Cleveland Clinic Head and Neck Institute, Cleveland, OH, USA. Electronic address:

Article Synopsis
  • This study examines the link between cleft palate and cholesteatoma risk in patients with ectodermal dysplasia types (EDT-ACPs), which has not been extensively analyzed before.
  • Researchers conducted an online survey involving individuals with specific disorders related to ectodermal dysplasia, achieving a low response rate of 18% with 65 participants.
  • The findings reveal a significantly higher incidence of cholesteatoma (39%) in participants with a history of cleft palate compared to those without (13%), indicating a unique risk factor for those with EDT-ACPs that requires attention for early diagnosis and treatment to prevent serious complications like hearing loss.

ACR Appropriateness Criteria® Chronic Hand and Wrist Pain: 2023 Update.

J Am Coll Radiol

June 2024

Specialty Chair, VA San Diego Healthcare System, San Diego, California.

Chronic hand and wrist pain is a common presenting complaint. The intricate anatomy results in a variety of pain generators-multiple bones, articular cartilage, intrinsic ligaments, triangular fibrocartilage complex, joint capsules and synovium, tendons and tendon sheaths, muscles, and nerves-in a compact space. The need for imaging and the choice of the appropriate imaging modality are best determined by the patient's presentation, physical examination, and the clinician's working differential diagnosis.

Introduction: Cutaneous adverse reactions to epidermal growth factor receptor inhibitors (EGFRi) are some of the most common side effects that patients experience. However, cutaneous adverse reactions that cause dyspigmentation in patients have been rarely reported. Erythema dyschromicum perstans (EDP) is a rare pigmentary condition that causes ashy-grey hyperpigmented macules and patches, with a few cases reported from EGFRi in the literature.

Article Synopsis
  • Burn-related pain can significantly reduce quality of life and hinder recovery, emphasizing the need for careful assessment and management.
  • The assessment process should identify whether the pain is acute or chronic, and further classify it into nociceptive, neuropathic, or nociplastic types to inform treatment strategies.
  • A multimodal approach using combination therapies can enhance treatment effectiveness and minimize side effects, but if these are insufficient, surgical options should be explored, along with knowing when to refer patients to specialized pain management.

Euglycemic diabetic ketoacidosis (EDKA) is an uncommon subtype of diabetic ketoacidosis (DKA) which presents with similar laboratory findings to classic DKA with the exception of blood glucose levels being under 250 mg/dl. EDKA has several etiologies including pregnancy, starvation and the use of sodium-glucose cotransporter-2 inhibitors (SGLT-2). SGLT-2 inhibitors such as empagliflozin and dapagliflozin are increasing in popularity due to their positive benefits for patients with diabetes mellitus and cardiac disease.
