Background: Hemophagocytic lymphohistiocytosis (HLH) is an immunologic syndrome characterized by excessive inflammation and tissue injury due to uncontrolled activation of the phagocytic system. The underlying mechanism is a lack of downregulation of activated macrophages and lymphocytes by natural killer and T cells. Unfortunately, the diagnosis is often delayed or missed due to the rarity of the disease, decreased awareness, and clinical picture variability.

Phantom Limb Pain Management.

J Hand Surg Am

October 2024

Division of Hand and Microsurgery, Department of Orthopaedic Surgery, Washington University School of Medicine, St. Louis, MO.

With an estimated 2 million major limb amputees and projections reaching 3.6 million by 2050, the increasing prevalence of limb loss in the United States underscores the importance of addressing complications associated with limb loss. Phantom limb pain (PLP) is a common and often chronic condition affecting 40% to 80% of amputees.

Cadaveric dissection has been a foundational principle of medical anatomic education for the last 400 years, yet medical anatomy course hours have been drastically reduced in the last decades. With this course reduction and the availability of other modalities for anatomic instruction, the physician may question the current role of cadavers in medical anatomy courses. This study aims to evaluate the role cadaveric dissection plays in anatomy programs in allopathic and osteopathic medical schools in Missouri and compare course hours to the national trend.

Article Synopsis
  • Peripheral nerve injuries pose significant challenges in hand and upper extremity trauma, with unsatisfactory repair outcomes despite advancements in microsurgery.
  • The regeneration of peripheral nerves is a complex process influenced by various factors, including neurotrophic factors that support cell growth and nerve recovery.
  • Innovative delivery methods, like gelatin-based conduits and hydrogels, alongside pharmacotherapies such as tacrolimus, show potential in enhancing nerve regeneration and improving clinical outcomes for patients.

Neuroendovascular rescue of patients with acute ischemic stroke caused by a large arterial occlusion has evolved throughout the first quarter of the present century, and continues to do so. Starting with the intra-arterial instillation of thrombolytic agents via microcatheters to dissolve occluding thromboembolic material, the current status is one that includes a variety of different techniques such as direct aspiration of thrombus, removal by stent retriever, adjuvant techniques such as balloon angioplasty, stenting, and tactical intra-arterial instillation of thrombolytic agents in smaller branches to treat no-reflow phenomenon. The results have been consistently shown to benefit these patients, irrespective of whether they had already received intravenous tissue-type plasminogen activator or not.

The effect of standardization of insertion and removal of percutaneous left ventricular assist device.

Cardiovasc Revasc Med

May 2024

University of Missouri - Columbia School of Medicine, Department of Medicine, Division of Cardiovascular Medicine, Columbia, MO, United States of America. Electronic address:

Article Synopsis
  • The study assessed the effects of standardizing the insertion and removal protocols for pVAD devices on clinical outcomes in patients.
  • There was a notable reduction in limb ischemia incidents after the protocol was implemented, dropping from 17.2% to 1.7%, with the change being statistically significant (p = 0.01).
  • Although the bleeding rates did not significantly change, adherence to the protocol improved in most areas, indicating overall better practice following standardization.

Adsorption-based extracorporeal therapies have been subject to technical developments and clinical application for close to five decades. More recently, new technological developments in membrane and sorbent manipulation have made it possible to deliver more biocompatible extracorporeal adsorption therapies to patients with a variety of conditions. There are several key rationales based on physicochemical principles and clinical considerations that justify the application and investigation of such therapies as evidenced by multiple ex vivo, experimental and clinical observations.

Introduction: Despite the rise of micro-invasive glaucoma surgery (MIGS), glaucoma drainage device implantation continues to be a mainstay among glaucoma surgical treatment options. Anterior chamber tube placement, while effective in reducing intraocular pressure (IOP), poses corneal endothelial risks. Ciliary sulcus tube placement shows promise in reduced corneal endothelial cell loss, but proper sulcus placement can be challenging.

Background: Strengthening health systems through planned safety and quality improvement initiatives is an imperative to achieve more equitable, resilient, and effective care. And yet, years of organizational behavior research demonstrate that change initiatives often fall short because managers fail to account for organizational readiness for change. This finding remains true especially among surgical safety and quality improvement initiatives in low-income countries and middle-income countries.
