In order to study the usefulness of upper mid-arm circumference (MAC) and mid-arm circumference:head circumference ratio (MAC:HC) measurements in assessing longitudinal growth in hospitalized preterm infants, we prospectively measured weights, lengths, occipitofrontal head circumferences (OFC), MACs, MAC:HCs, weight/length for age, nutritional intakes, and serum transthyretin and albumin levels in 50 preterm, low-birth-weight, appropriate for gestational age newborn infants during their first 4 postnatal weeks and at hospital discharge. At some time during hospitalization, weight measurements were abnormal (greater than or equal to 2SD from the gestational age mean) in 48% of the infants as compared with 24% with abnormal MAC measurements (p = 0.002).

Biodistribution and radioimmunoscintigraphy studies of renal cell carcinoma using tumor-preferential monoclonal antibodies and F(ab')2 fragments.

The in vivo localization of renal cell carcinoma-preferential monoclonal antibodies A6H, D5D, and C5H was evaluated and the biodistribution of F(ab')2 antibody fragments of A6H and the intact Mab were compared in over 100 nude mice. A6H localized well to most renal cell carcinoma xenografts studied; the median tumor to blood ratios ranged from 6.4 to 11.

The outcome of renal transplantation in children with the prune belly syndrome.

Children with the prune belly syndrome are at high risk for renal failure. The effect of the prune belly syndrome on the outcome of renal transplantation was evaluated in a retrospective study in which 8 transplant recipients with this syndrome were randomly matched with 13 control, nondiabetic transplant patients. The parameters evaluated were patient and graft survival, renal function and interval until transplantation.

The role of ilioinguinal lymphadenectomy and significance of histological differentiation in treatment of carcinoma of the penis.

We reviewed retrospectively the medical records of 58 patients treated for squamous cell carcinoma of the penis who were followed for more than 3 years or until they died. Tissue sections from all patients were reviewed. Of 15 patients with stage I disease 11 underwent partial penectomy, and 4 underwent partial penectomy and immediate ilioinguinal lymphadenectomy; none died of cancer.

The importance of separation of prostatic lobes in relief of prostatic obstruction by balloon catheter urethroplasty: studies in dogs and humans.

We performed balloon urethroplasty in six older normal dogs and 10 patients with benign prostatic hypertrophy to establish the mechanism by which this technique results in relief of bladder outlet obstruction. Thirty-millimeter balloon catheters were used in both the animal and human studies. Autopsy studies showed disruption of the anterior prostatic commissure in four of the six dogs, disruption of both anterior and posterior commissures in one dog, and no disruption in the remaining dog.

Splenic abscess as a complication of percutaneous nephrostomy.

We report a case of splenic abscess as a complication of percutaneous nephrostomy tube placement. The patient was a sixty-three-year-old man suffering from a recurrence of transitional cell carcinoma of the bladder after cystectomy and neobladder substitution. Computed tomography of the abdomen identified the abscess, which was drained percutaneously.

The effect of prolonged intrauterine hyperinsulinemia on iron utilization in fetal sheep.

Newborn infants of poorly controlled insulin-dependent diabetic mothers demonstrate a redistribution of iron from serum and tissue stores into red blood cells. These changes may be due to increases in iron utilization during augmented Hb synthesis, which compensates for chronic intrauterine hypoxemia induced by prolonged fetal hyperinsulinemia. We tested this hypothesis by measuring plasma iron, total iron-binding capacity, percent iron-binding capacity saturation (total iron-binding capacity saturation), Hb concentration, total red cell Hb, and total red cell iron in the arterial blood of 11 chronically instrumented fetal sheep after 7-12 d of infusion with 15 U/day of insulin (n = 5) or placebo (n = 6).

Urethral false passage as a complication of intermittent catheterization.

Intermittent catheterization is used commonly to treat bladder dysfunction. We treated 10 patients who were experiencing difficulty with intermittent catheterization, 9 of whom had a false urethral passage. Of these patients 6 had previously undergone a bladder neck or urethral operation.

Technetium-99m sulfur colloid spleen imaging following partial pancreatectomy and splenic artery and vein resection.

We retrospectively studied the records and [99mTc]sulfur colloid (TSC) spleen studies of 38 patients who underwent distal pancreatectomy with splenic artery and vein resection for donation to HLA-compatible relatives. The spleens in immediate postoperative TSC studies were normal in 11% of cases, showed no uptake in 16%, showed diffusely decreased uptake in 50%, and showed focal defects in 26%. Twenty of the patients, all with abnormal initial TSC studies, had repeat studies 2 wk to 3 yr later; 15% showed no change, 35% showed some improvement, and 45% became normal.

Retrograde left atrial catheterization.

Catheterization of the left atrium can provide valuable information about both congenital and acquired heart disease. The traditional approach to examination of the left atrium has been the transseptal route, which has occasionally been associated with serious complications. A method of retrograde left atrial catheterization has been developed that is performed with a modified standard catheter.

Correlation of Fourier spectral shift-determined hepatic acoustic attenuation coefficients with liver biopsy findings.

We calculated hepatic acoustic attenuation coefficients (ACs) in 13 patients who had liver biopsies and in 3 normal subjects by means of Fourier spectral analysis of reflected ultrasound signals from increasing depths in the liver. Rheumatic fever and rheumatic heart disease are considered to result from abnormal immune responses after Group A streptococcal pharyngitis. Production of interleukin 1 (IL-1), tumor necrosis factor-alpha (TNF), interleukin 2 (IL-2) and immunoglobulin (Ig) by blood and tonsillar mononuclear cells from rheumatic or healthy children was measured after stimulation in vitro by pokeweed mitogen (PWM) or the streptococcal extracellular product, blastogen A (BLA). Tonsillar cells from patients with rheumatic heart disease produced significantly less IL-1, TNF, IL-2, and Ig than control tonsillar cells.

Improved survival and leukocyte reconstitution without detrimental effects on engraftment in murine recipients of human recombinant granulocyte colony-stimulating factor after transplantation of T-cell-depleted histoincompatible bone marrow.

In vivo administration of human recombinant granulocyte colony-stimulating factor (rG-CSF) was evaluated for effects on survival, hematologic recovery, and engraftment in an allogeneic murine bone marrow transplantation (BMT) model involving T-cell depletion. Post-BMT recipients of continuous subcutaneous infusions of rG-CSF (n = 62) for 14 days had a significant survival advantage compared with post-BMT controls (n = 60) that received phosphate-buffered saline (PBS) infusions. Moreover, recipients of rG-CSF had significantly increased numbers of circulating leukocytes on days 7 and 14 post-BMT.

High magnesium diets increase blood pressure and enhance stroke mortality in hypertensive SHRsp rats.

Accelerated idioventricular rhythm complicating atrioventricular junction ablation for automatic atrial tachycardia.

A 13-year-old male with histologic evidence of cardiomyopathy, drug-refractory primary atrial tachycardias, and deteriorating left ventricular function underwent transcatheter His bundle ablation to control ventricular rate. Following an initial successful ablation at the level of the atrioventricular node, the patient exhibited an accelerated escape rhythm of apparent junctional origin (ventricular cycle length = 470 msec, HV = 100 msec) with complete heart block. A second ablation procedure was undertaken, following which an accelerated idioventricular rhythm (cycle length = 500 msec) became apparent and has persisted (follow-up 15 months).

Aid for safer sclerotherapy of the internal spermatic vein.

A device was developed for use during sclerotherapy to treat spermatic varicoceles. It is used to provide external compression of the internal spermatic vein, to prevent distal flow of the sclerosing agent into the scrotal venous network. The device has been successfully used in 27 patients (43 spermatic veins).

True histiocytic lymphoma: histopathologic, immunophenotypic and genotypic analysis.

Five cases of true histiocytic lymphoma (THL) were analysed by immunophenotyping and immunoglobulin/T-cell receptor genotyping. These cases showed striking morphologic diversity but a strong degree of immunophenotypic homogeneity. The malignant cells reacted with multiple histiocytic markers including CD11c (Ki-M1, LeuM5), CD14, CD68 (Ki-M6) and Ki-M8; anti-HLA-DR and non-specific esterase staining was also found in all cases.

Urinary tract disorders in patients with chronic granulomatous disease.

Twelve of the 25 patients with chronic granulomatous disease treated at our institution between 1957 and 1987 were found to have urinary tract disorders. All 12 patients were male and 22 years of age or younger when chronic granulomatous disease was diagnosed. Six patients had hydroureteronephrosis in association with recurrent episodes of pyelonephritis, retroperitoneal lymphadenitis, and granuloma formation.

Management of cystine nephrolithiasis by endourologic methods and shock-wave lithotripsy.

Cystinuria is a rare cause of renal calculi, accounting for about 1 percent of cases. Herein we describe our experience with a series of 10 patients with cystinuria and highlight the particular problems of the use of extracorporeal shock-wave lithotripsy (ESWL) to treat these types of stones and our resultant emphasis on percutaneous techniques.

A 10-year experience with 290 pancreas transplants at a single institution.

Since our report at the 1984 American Surgical Association meeting of 100 pancreas transplants from 1966 through 1983, another 190 have been performed. The current series, begun in 1978, now numbers 276 cases, and includes 133 nonuremic recipients of pancreas transplants alone (PTA), 46 simultaneous pancreas/kidney transplants (SPK), and 97 pancreas tranplants after a kidney transplant (PAK). Duct management techniques used were free intraperitoneal drainage in 44 cases, duct occlusion in 44, enteric drainage in 89, and bladder drainage in 128.

Transplant renal artery stenosis: evaluation with duplex sonography.

Determination of the cause of hypertension in renal transplant recipients has required invasive studies such as angiography. Consequently there has been recent interest in noninvasive evaluation. Over a 19-month period, duplex sonography was performed on the renal transplant vasculature in 31 allograft recipients who also underwent correlative arteriography.

Nonintubated Anderson-Hynes repair of ureteropelvic junction obstruction in 60 patients.

We reviewed our experience with 68 consecutive Anderson-Hynes ureteropyeloplasties. The 64 infants, children and young adults ranged from 2 days to 28 years old (median age 2 years), and 28 were less than 1 year old. Intubation was used in only 4 patients: 2 who also underwent ureteral reimplantation for vesicoureteral reflux, 1 with stones in the renal pelvis and 1 with pyonephrosis.
