High NaCl diets often increase blood pressure and thereby accelerate lesions in arterial walls. Could high NaCl diets increase arterial lesions without raising blood pressure? To test this, 100 uninephrectomized Dahl salt-resistant (DR) rats (highly resistant to NaCl hypertension) were administered deoxycorticosterone acetate (DOCA) (250 mg/kg) in silicone implants and drinking water containing 1% NaCl for 6 weeks. Then the DOCA and saline were removed, and the rats were allowed to recover for 4 weeks.

The authors present the radiographic features of a previously incompletely delineated bone dysplasia, which they call spondylometaphyseal dysplasia, corner fracture type. Analysis of the genomic termini of Epstein-Barr virus can provide valuable insight into the cofactor role of EBV in the development of B cell lymphomas and lymphoproliferative disease. We report EBV genomic findings in pathologic specimens from 10 patients who developed lymphoma or lymphoproliferative disease after renal or bone marrow transplantation. Endonuclease restriction patterns of EBV genomic termini are highly variable in size in both the episomal and linear configuration.

Mapping the cardiogenic impedance signal on the thoracic surface.

The cardiogenic impedance signal from band electrodes placed in the traditional position around the neck and lower thorax was studied by mapping the location of the signal on the sternum using 10 cm strip electrodes in eight male subjects. The band current electrodes on the neck and waist were replaced with 10 cm strip electrodes on the forehead and 10 cm below the xiphisternal joint, respectively, with only small changes in the dZ/dt peak amplitude and Z0. Similarly, using a strip voltage pickup electrode at the level of the xiphisternal joint resulted in very small changes in the waveform.

This report is a 20-year follow-up of 14 patients treated with external beam craniospinal irradiation and intrathecal gold (10-45 mCi) for medulloblastoma. Six of the patients died within 2 years of treatment from persistent disease. No patients are alive without complications.

We implanted the artificial urinary sphincter in 15 incontinent patients for whom multiple urethral and bladder neck operations, including sphincter placement, had been unsuccessful. The 5 male and 10 female patients ranged from 3 to 26 years old (mean age 11 years). The etiology of incontinence was neurogenic bladder in 10 patients, epispadias in 2, exstrophy in 1, ectopic ureters in 1 and traumatic urethral disruption in 1.

We retrospectively reviewed the outcome of 37 prostate cancer patients with ureteral obstruction treated by percutaneous nephrostomy. The over-all survival was 57% at 1 year and 29% at 2 years (median survival time 21 months). The 1 and 2-year survival rates of 15 patients with no prior hormonal therapy were 73 and 47%, respectively, while those of patients who had previously received hormonal therapy were 48 and 19%, respectively.

Psychological and physical reactions to whirlpool baths.

Physical and psychological reactions to whirlpool baths were studied in a group of 40 healthy adult volunteers. Responses were measured after 10-min immersions in whirlpools under on and off conditions. Whirlpools and warm baths both induce increases in pulse and finger temperature, although a greater increase in finger temperature is associated with whirlpools.

The debate on the nosologic position of hemangiopericytomas of the meninges has been based mainly on light microscopic and ultrastructural considerations; recent immunohistochemical studies have yielded controversial results. We have used a panel of antibodies to vimentin, desmin, actin, S100 protein, epithelial membrane antigen, cytokeratins, Leu-7, factor VIII-related antigen and type IV collagen to compare the immunophenotype of 10 soft-tissue and seven meningeal hemangiopericytomas. The immunophenotypic profile of these tumors is identical, and differs from that of meningiomas in that epithelial membrane antigen and cytokeratin are not present.

Eighty patients with type I diabetes and end stage renal disease were prospectively evaluated for coronary artery disease with dipyridamole-thallium-201 scintigraphy and quantitative coronary angiography. Forty patients received dipyridamole orally, and 40 received it intravenously. The prevalence of coronary artery disease was 53%.

We present a modified technique for feminization genitoplasty, which combines the main features of the Hendren and Crawford pull-through operation and the Kogan reduction clitoroplasty. The unique features of the operation are the use of a flap of preputial skin to construct the vestibule and the anterior vaginal wall, and the universal use of the urogenital sinus to form the distal urethra, which avoids creation of female hypospadias. The new introitus is ample and, because there is no cut-back of the urogenital sinus, we avoid accidental incision of the urethral sphincter and the potential for urinary incontinence.

Smoking produces some potentially reversible effects that may be relevant to perioperative morbidity. Although the reversal of many effects requires long-term (weeks to months) smoking cessation, a short abstinence period (12 to 72 hours) is sufficient to normalize several important cardiovascular parameters. The physiology of carbon monoxide elimination is the basis for recommending a specific smoke-free interval in the immediate preoperative period.

In situ hybridization (ISH) and immunohistochemistry (IHC) were compared for detection of cytomegalovirus (CMV) and herpes simplex virus (HSV) in routinely processed tissue. Fifty-four formalin-fixed paraffin-embedded tissue samples infected with CMV (36 tissues) or HSV (18 tissues) from 30 autopsies were studied. All tissues had either positive viral cultures (38 of 54) or characteristic viral inclusions on hematoxylin and eosin examination (39 of 54).
