Article Synopsis
  • Coronary chronic total occlusion (CTO) increases the risk of ventricular arrhythmias (VAs), which can lead to serious heart issues like ventricular tachycardia and sudden cardiac death.
  • A meta-analysis reviewing nine studies with over 3000 participants found that patients with CTOs have a 2.25 times higher risk of VAs compared to those with coronary artery disease (CAD) without CTOs.
  • The analysis also suggested that percutaneous coronary intervention (PCI) for CTOs significantly lowers the risk of VAs compared to patients managed with optimal medication alone, indicating the potential benefits of revascularization.

Article Synopsis
  • Early reoperation rates after sleeve gastrectomy (SG) and Roux-en-Y gastric bypass (RYGB) are low, but approximately 32% of reoperations after SG and 24% after RYGB happen within 24 hours due to complications like hemorrhage.
  • Factors such as older age, hypertension, and liver disease increase the risk of early reoperation, particularly in SG patients, and longer surgery times are a common factor for both procedures.
  • The findings suggest that patients with significant comorbidities should be carefully evaluated as candidates for same-day surgery due to the heightened risks associated with potential early complications.

Cannabis has been used for recreation and medical purposes for more than a millennium across the world; however, its use's consequences remain poorly understood. Although a growing number of surgical patients are regular cannabis consumers, little is known regarding the pharmacological interactions between cannabis and general anesthetics; consequently, there is not a solid consensus among anesthesiologists on the perioperative management of these patients. The existing evidence about the molecular mechanisms underlying pharmacological interactions between cannabinoids and anesthetic agents, both in animal models and in humans, shows divergent results.
