The role of modulators in cystic fibrosis related diabetes.

J Clin Transl Endocrinol

March 2022

Division of Endocrinology, Department of Pediatrics, University of Iowa Stead Family Children's Hospital, Iowa City, IA, USA.

The development and introduction of modulator therapies have completely shifted the paradigm for the treatment of cystic fibrosis (CF). Highly effective modulator therapies have driven marked improvements in lung function, exacerbation rate, weight and quality of life in CF patients. However, their effect on CF related diabetes (CFRD) is not well delineated.

Paraneoplastic syndromes are rare conditions associated with characteristic autoantibodies produced by malignancy, although similar autoantibodies and clinical presentations may occur in the absence of any neoplasm. Testing for paraneoplastic syndromes often involves panels of autoantibody assays. While autoantibody testing may reveal or confirm actionable clinical diagnoses, inappropriate utilization of testing may be low yield and further lead to false positives that may confuse the clinical picture.

Flow Monitoring of ECMO Circuit for Detecting Oxygenator Obstructions.

Ann Biomed Eng

December 2021

Department of Biomedical Engineering, University of Iowa, Iowa City, IA, 52242, USA.

Oxygenator thrombosis during extracorporeal membrane oxygenation (ECMO), is a complication that necessitates component replacement. ECMO centers monitor clot burden by intermittent measurement of pressure drop across the oxygenator. An increase in pressure drop at a preset flow rate suggests an increase in resistance/clot formation within the oxygenator.

Background: Chiari Malformation Type I (CM-I) is defined as cerebellar tonsil displacement more than 5 mm below the foramen magnum. This displacement can alter cerebrospinal fluid flow at the cervicomedullary junction resulting in Valsalva-induced headaches and syringomyelia and compress the brainstem resulting in bulbar symptoms. However, little is known about cognitive and psychological changes in CM-I.

Article Synopsis
  • Acute kidney injury (AKI) and fluid overload (FO) are significant issues in children undergoing extracorporeal membrane oxygenation (ECMO), particularly in cases of extracorporeal cardiopulmonary resuscitation (eCPR).
  • A study involving 131 patients found that those with fluid overload at ECMO discontinuation had a higher risk of mortality and a longer hospital stay regardless of AKI status.
  • The findings highlight the need for future research to focus on effective fluid management strategies in children receiving eCPR.

Advances in Neonatal Acute Kidney Injury.

Pediatrics

November 2021

Division of Nephrology, Department of Pediatrics, Medical University of South Carolina, Charleston, South Carolina.

In this state-of-the-art review, we highlight the major advances over the last 5 years in neonatal acute kidney injury (AKI). Large multicenter studies reveal that neonatal AKI is common and independently associated with increased morbidity and mortality. The natural course of neonatal AKI, along with the risk factors, mitigation strategies, and the role of AKI on short- and long-term outcomes, is becoming clearer.

Onco-nephrology is a recent and evolving medical subspecialty devoted to the care of patients with kidney disease and unique kidney-related complications in the context of cancer and its treatments, recognizing that management of kidney disease as well as the cancer itself will improve survival and quality of life. While this area has received much attention in the adult medicine sphere, similar emphasis in the pediatric realm has not yet been realized. As in adults, kidney involvement in children with cancer extends beyond the time of initial diagnosis and treatment.
