Background This project investigates a music intervention to deprescribe antipsychotics in rural Veterans with dementia. Methods The Veterans Health Administration Home-Based Primary Care Program is care provided in the home by an interdisciplinary team with the goals of decreasing hospitalizations and falls, providing education to patients and caregivers, and improving quality of life. Eighteen Home Based Primary Care Veterans with dementia and active antipsychotic prescriptions were identified with the goal to deprescribe antipsychotics in 50% of them using a music intervention.

Drug reaction with eosinophilia and systemic symptoms (DRESS) is a rare but potentially life-threatening adverse drug reaction characterized by extensive skin rash in association with hematological abnormalities, including eosinophilia and atypical lymphocytosis, lymphadenopathy, fever, and extensive visceral organ involvement. Here, we presented a rare case of vancomycin-induced DRESS syndrome in a male who was treated with IV vancomycin for a brain abscess.

This case report describes a 10-year-old spayed female domestic medium-hair cat with chronic upper respiratory signs related to a remnant of the left globe in the orbit after incomplete enucleation, causing a draining tract. A CT scan, complete blood (cell) count, biochemical analysis, rhinoscopy, biopsy, culture, histopathologic analysis, and surgical revision of enucleation were done. Complete blood (cell) count was unremarkable.

Objective: To compare cardiovascular outcomes in dogs undergoing adrenalectomy for pheochromocytoma with and without phenoxybenzamine pretreatment.
Animals: A total of 65 medical records from dogs at 5 veterinary hospitals.
Procedure: Records from January 2004 to December 2021 were evaluated for systolic, mean, and diastolic arterial pressures; numbers of hypertensive and hypotensive episodes; highest and lowest systolic arterial pressure differences; dose and duration of phenoxybenzamine; and presence of intraoperative arrhythmias.

Purpose: This study aimed to assess the impact of systemic medications, including GLP-1 receptor agonists (GLP-1 RAs), fenofibrates, thiazolidinediones (TZDs), and calcium channel blockers (CCBs), on the risk of developing diabetic macular edema (DME) in patients with type 2 diabetes mellitus (T2DM).
Design: A retrospective cohort study was conducted using electronic medical records (EMR) data from the TriNetX health research network, covering a period from October 2004-2024.
Participants: The study population comprised patients diagnosed with T2DM who were newly initiated on GLP-1 RAs, fenofibrates, TZDs, or CCBs.

Background: Previous studies suggest that early developmental delay is a common feature of Juvenile-Onset Huntington's disease (JOHD), with highest incidence in those with very high CAG repeats (> 80). However, all reports of developmental delay in JOHD are exclusively based on retrospective review of medical charts. Comprehensive assessment of birth history metrics may provide better insight into the question of early life development in JOHD.

Heavy menstrual bleeding (HMB) is a common symptom in adolescence, often leading to significant disruptions in daily life, such as school absences, shame caused by the stigma surrounding menstruation, and symptoms from iron deficiency. Further, HMB may be the first and/or only sign of an underlying bleeding disorder. Navigating the symptoms, effects, and treatments of HMB during adolescence requires a collaborative approach between the patient, caregivers, and healthcare providers.

These visual pathways can negatively impact visual perception and decision-making. The Vestibulo-Ocular Reflex (VOR) is a compensatory mechanism that stabilizes gaze on a target during head movement, such as running or turning.

Background: Spetzler-Martin (SM) Grade III brain arteriovenous malformations (BAVMs) represent a transitional risk zone between low- and high-grade BAVMs, characterized by diverse angioarchitecture. The primary treatment options are endovascular embolization, microsurgical resection (MS), and stereotactic radiosurgery (SRS). This study compares the efficacy and outcomes of MS and SRS.

» Suture button fixation has emerged as an effective surgical construct in arthroscopic and open Latarjet and anterior glenoid reconstruction with free autograft bone with high rates of bone block healing.» Biomechanical data suggest that screw and suture button mechanical fixation constructs provide similar load to failure and stability for the Latarjet procedure.» Preliminary bone healing models have identified that flexible fixation may exhibit a higher degree of bone callus maturation, whereas rigid fixation results in excessive callus hyperplasia» Mechanical tensioner use provides consistent tensioning of suture button constructs and improves bone-to-bone healing rates when used for anterior glenoid reconstruction surgery.

Background: Cardiac amyloidosis (CA) is an underdiagnosed disease, often presenting with electrical conduction abnormalities or ventricular arrhythmias, prompting implantation of cardiac implantable electrical devices (CIEDs).
Objectives: This study sought to evaluate the performance of right ventricular sensing (RVSens) during implantation of CIEDs in patients presenting with increased left ventricular (LV) wall thicknesses (ILVWT) and preserved LV function to predict the diagnosis of CA.
Methods: All consecutive patients with ILVWT (maximal LV wall thickness ≥12 mm), preserved LV function and no significant aortic stenosis who underwent implantation of CIEDs at 2 French referral centers for rare and inheritable cardiac diseases were retrospectively evaluated.

Esophageal cancer, particularly squamous cell carcinoma, poses significant diagnostic challenges due to its aggressive nature and similarity to metaplastic tissue. Accurate diagnosis often requires multiple biopsies and vigilant surveillance, especially in high-risk individuals with conditions such as gastroesophageal reflux disease (GERD) and a history of smoking. We present a 66-year-old female patient with a history of severe GERD and smoking, who underwent routine endoscopy revealing a gastric cardia nodule.

Background: The identification of a heterozygous exonic GGC repeat expansion in ZFHX3 underlying spinocerebellar ataxia type 4 (SCA4) has solved a 25-year diagnostic conundrum. We used adaptive long-read sequencing to decipher the pathogenic expansion in the index Utah family and an unrelated family from Iowa of Swedish ancestry. Contemporaneous to our discovery, other groups identified the same repeat expansion in affected individuals from Utah, Sweden, and Germany, highlighting the current pivotal time for detection of novel repeat expansion disorders.

Rationale And Objective: Acute kidney injury (AKI) is a common complication among hospitalized adults, but AKI prediction and prevention among adults has proved challenging. We used machine learning to update the nephrotoxic injury negated by just-in time action (NINJA), a pediatric program that predicts nephrotoxic AKI, to improve accuracy among adults.
Study Design: A retrospective cohort study.
