Highly selective inhibitors of cyclin-dependent kinase 4 and 6 (CDK4/6is) have emerged as a standart of care for first- and second-line therapies in combination with endocrine therapy (ET) for HR+/HER2- metastatic breast cancer (MBC) patients. It has been reported that combination therapy is more effective than ET alone and is safe in elderly patients as well as young patients. Nevertheless, elderly and very old patients with HR+/HER2-MBC treated with CDK4/6 inhibitor (CDK4/6i) combinations are relatively underrepresented in randomized controlled trials.

Background: The SIPPET randomized clinical trial showed that in previously untreated patients (PUPs) with severe hemophilia A, treatment with plasma-derived factor (F)VIII (pdFVIII) within the first 50 exposure days (EDs) was associated with a lower cumulative incidence of inhibitors than with recombinant FVIII (rFVIII). Switching to rFVIII beyond 50 EDs with pdFVIII is a treatment often implemented by many centers. The question is whether or not this switch may induce a risk of inhibitor development.

This study aims to improve understanding of the anatomy of the deep brain nuclei relevant to deep brain stimulation as well as stereotactic lesioning procedures, including radio frequency, high-focused ultrasound, and radiosurgery. We created interactive, three-dimensional virtual models from cadaveric dissections and radiological segmentation. We used five brain specimens (ten hemispheres) obtained from routine autopsies, prepared according to Klingler's method.

  • Spinal muscular atrophy (SMA) is a genetic disorder leading to muscle weakness and atrophy, with high rates of morbidity and mortality despite recent treatments.
  • * The study reviewed 16 SMA type 1 patients, noting complications like gastrointestinal issues, respiratory distress, and infections, with a 50% mortality rate during the follow-up period.
  • * Findings suggest higher survival rates are linked to more nusinersen treatments and better overall health scores, indicating a need for a multidisciplinary approach to patient management.

Purpose: To assess the impact of the The Controlling Nutritional Status (CONUT) score, an indicator of nutritional status, on the survival and prognosis after radical cystectomy.

Materials And Methods: The medical records of patients who underwent consecutive radical cystectomy operations with the diagnosis of muscle-invasive bladder cancer at our clinic were retrospectively examined. The patients were separated into two groups based on the cut-off CONUT score which was derived using the receiver operating characteristic (ROC) curve.

  • Macrosomia, or excessive fetal growth, is commonly seen in infants of mothers with diabetes, but it can also occur in non-diabetic women, leading to various complications.
  • A study analyzed 8,956 non-diabetic pregnant women to explore the link between second-trimester lipid profiles and macrosomia, excluding those with pre-existing conditions that could affect lipid levels.
  • Results showed that higher triglycerides, total cholesterol, and LDL cholesterol were associated with macrosomia, while low HDL cholesterol was linked negatively; only triglycerides remained significantly associated after further analysis.

Background And Objectives: Bipolar electrocautery systems in neurosurgical procedures may induce thermal damage to adjacent tissues, especially neural tissues. Therefore, it is crucial to control thermal spread from the tips of bipolar forceps into adjacent tissues. The goal of this study was to compare the thermal damage induced in unintended adjacent tissues during coagulation with 6 different bipolar forceps.

November 2024

Izmir Biomedicine and Genome Center, Balcova, 35330, Izmir, Turkey; İzmir International Biomedicine and Genome Institute, Dokuz Eylül University, Izmir, Turkey.

Oxygen support plays a critical role in the management of preterm infants in neonatal intensive care units. On the other hand, the possible effects of oxygen supplementation on cellular functions, specifically glucose metabolism, have been less understood. PURPOSE: of the study is to investigate whether supplemental oxygen alters glucose metabolism and pentose phosphate pathway (PPP) activity in the brain tissue and its relevance with silent information regulator proteins (SIRT) pathway.

  • The study investigates the prognostic factors for C3 glomerulopathy (C3G), focusing on the total chronicity score (TCS) derived from a new histological index (C3G-HI).
  • 74 patients from 20 medical centers were analyzed to determine factors affecting kidney health outcomes, with a particular emphasis on hemoglobin levels and their correlation with patient survival and kidney function.
  • Results indicated that low hemoglobin levels were linked to worse outcomes in C3G patients, and a TCS of 4 or higher correlated with significantly lower kidney survival rates over three years.

  • The study evaluated five different scoring systems (FGSI, UFGSI, ACCI, qSOFA, LRINEC) to predict outcomes in patients with Fournier gangrene, analyzing data from 311 patients between 2010 and 2024.
  • Univariate and multivariate analyses showed that UFGSI and FGSI were the most effective scoring systems in predicting mortality, with higher values seen in non-survivor patients.
  • ROC analysis determined optimal cut-off values for each scoring system, with UFGSI proving to be the most accurate for predicting mortality, achieving 84% sensitivity and 97% specificity at a score ≥12.

Background: Although pulse oximetry technology, which is considered the standard of care to ensure optimum oxygenation, is indispensable in clinical practice, especially in the detection of hypoxemia, it has some limitations in the detection of hyperoxemia. Oxygen Reserve Index can provide clinicians with a crucial pathway in detecting and preventing hyperoxia, noninvasively. Our aim in this study is to determine the hyperoxia detection ability of ORi and to investigate the effectiveness of ORi and SpO-guided FiO titration in preventing hyperoxia.
