In the field of cancer care, it is still difficult to say that advance care planning(ACP)has been fully integrated, and the perceptions of ACP vary depending on the individual's role. Since oncologists have a deeper understanding of the patient's condition and prognosis than the patients themselves, they are often the first to recognize the future need for ACP. It is important to gather and incorporate patients' preferences and relevant information regarding ACP early in the course of daily cancer treatment.

Characteristics of a revised quick sequential organ failure assessment score (RqSOFA) to predict in-hospital mortality of patients visiting the emergency department via ambulance: an observational cohort study.

Intern Emerg Med

December 2024

The Pediatric Research in Emergency Therapeutics (PRETx) Program, Division of Emergency Medicine, Department of Pediatrics, University of British Columbia, and BC Children's Hospital Research Institute, Vancouver, BC, Canada.

The National Early Warning Score (NEWS), Quick Sequential Organ Failure Assessment (qSOFA), and modified qSOFA (MqSOFA) are scoring systems that rely on vital signs. However, NEWS is time-consuming, qSOFA has low sensitivity, and MqSOFA includes a difficult calculation. To address these issues, we developed the Revised qSOFA score (RqSOFA) that consists of percutaneous oxygen saturation, oxygen usage, Simple Shock Index, and the parameters of qSOFA.

Article Synopsis
  • Antibiotic resistance globally causes increased health issues, costs, and mortality, highlighting the need for better antibiotic usage research, especially for treating acute cholangitis (AC), which often requires antimicrobial therapy.
  • The Tokyo Guidelines 2018 suggest 4-7 days of antibiotics after biliary drainage, but this recommendation lacks strong evidence and is mainly based on expert opinions.
  • Recent studies propose a shorter 1-3 day antibiotic treatment, prompting a reevaluation of treatment duration to improve effectiveness while minimizing resistance and side effects, taking into account local resistance patterns and patient-specific risks.

Allergen-specific antibodies (Abs), IgE, and IgG4 increase during the early phase of oral immunotherapy (OIT) of allergen food in patients; subsequently, IgE levels decrease and specific IgG4 levels increase after successful OIT treatment. The detailed profile of these Abs during OIT remains largely unclear. We developed a diagnostic tool to assess the OIT efficacy and extent of responsiveness based on a profiling method by identifying epitopes recognized by the Ab classes of IgE or IgG4.

  • Cisplatin causes damage to renal proximal tubular cells through mitochondrial impairment, leading to cell death and inflammation.
  • Dichloroacetate (DCA) protects against this damage by enhancing energy production and influencing specific apoptotic pathways, particularly by inhibiting JNK and caspase pathways, and increasing cFLIP levels.
  • In murine models, DCA reduced kidney injuries associated with cisplatin treatment, suggesting its potential therapeutic role against nephrotoxicity while promoting some ROS production and inflammation without full impairment.
