Alpha-blockers after shock wave lithotripsy for renal or ureteral stones in adults.

Cochrane Database Syst Rev

November 2020

Urology Section, Minneapolis VA Health Care System, Minneapolis, Minnesota, USA.

Background: Shock wave lithotripsy (SWL) is a widely used method to treat renal and ureteral stone. It fragments stones into smaller pieces that are then able to pass spontaneously down the ureter and into the bladder. Alpha-blockers may assist in promoting the passage of stone fragments, but their effectiveness remains uncertain.

Objective: Studies suggest female physicians experience higher rates of infertility than the general population. The overall objective of this study was to determine the rate of impaired fecundity in a sample of female emergency physicians and compare it to the Centers for Disease Control and Prevention (CDC) National Survey of Family Growth (NSFG) data. Impaired fecundity is defined as physical difficulty in getting pregnant or carrying a pregnancy to live birth.

Validated image ordering guidelines for odontogenic infections.

Int J Oral Maxillofac Surg

May 2021

Department of Oral and Maxillofacial Surgery, University of Florida Jacksonville, Jacksonville, FL, USA. Electronic address:

Overuse of computed tomography (CT) is a prevalent problem across multiple disciplines in healthcare and is common in the workup of odontogenic infections. To address this problem, an imaging pathway was created through collaboration of the oral maxillofacial surgery and emergency medicine departments to reduce unnecessary CT orders. A prospective study was implemented to assess the success of the imaging pathway to guide in the selection of the most appropriate radiological imaging modality when managing an odontogenic infection.

Article Synopsis
  • Cryptogenic sensory polyneuropathy (CSPN) is a prevalent neurological condition that causes significant pain, with limited effective treatment options currently available.
  • Researchers conducted a clinical trial comparing four medications (pregabalin, duloxetine, nortriptyline, and mexiletine) to determine which was most effective for pain reduction in CSPN.
  • The trial involved 402 participants over about three years, using a randomized approach to evaluate the efficacy and tolerability of each medication.

Background: Although risk factors for atrial fibrillation (AF) and atrial flutter (AFL) are known, identifying patients who will develop AF/AFL within the near future remains challenging. We sought to evaluate if the CHADS-VASc risk score (CVRS) can identify hospital readmissions with AF, AFL, or acute cerebrovascular accident (CVA) among hospitalized patients without prior history of AF/AFL.

Methods: Using the Nationwide Readmission Database, a study cohort included patients without prior AF/AFL or new diagnosis of AF/AFL at the index hospitalization from 2012 to 2014.

Background: The American College of Surgeons (ACS) National Surgical Quality Improvement Program (NSQIP) calculator is a useful tool used by physicians to better inform patients on the surgical risk of postoperative complications. It makes use of the NSQIP database to derive the chance for several adverse outcomes to occur postoperatively given certain patient's factors. The aim of this study was to assess its applicability in a series of patients undergoing an Ivor Lewis esophagectomy.

Infections Associated with Temporary External Fixation.

J Surg Orthop Adv

November 2020

Department of Orthopaedic Surgery, University of Florida-Jacksonville, Jacksonville, Florida.

Infections associated with external fixation are thought to be related to the fixator construct, the length of time a fixator is on the limb and the injury severity score (ISS). Patients who had temporary external fixation applied for lower extremity fractures were included. Charts were reviewed for prevalence of infection, loss of reduction or other external fixator modification.

Head and neck surgery is a risk factor for atrial fibrillation: incidence and outcomes.

Int J Oral Maxillofac Surg

December 2020

Department of Oral and Maxillofacial Surgery, University of Florida Jacksonville, Jacksonville, FL, USA. Electronic address:

Atrial fibrillation (AF) is the most common postoperative arrhythmia and can cause increased length of stay, costs, morbidity, and mortality. Little information exists about postoperative AF after major head and neck surgery, but it is thought to occur more frequently than after surgery at other extra-thoracic sites. A retrospective cohort study was implemented, including patients who had undergone major head and neck surgery and who had follow-up records covering a minimum of 60 days postoperative.

An analysis of unplanned readmissions after head and neck microvascular reconstructive surgery.

Int J Oral Maxillofac Surg

December 2020

Department of Oral and Maxillofacial Surgery, University of Florida Jacksonville, Jacksonville, FL, USA. Electronic address:

The 30-day readmission rate is a highly scrutinized metric of quality surgical care, because readmission is costly and perceived to be avoidable with planning and patient education. Head and neck surgery patients generally have multiple risk factors for readmission, as readmitted patients are generally older, with more co-morbidities, lower socio-economic status, and a history of multiple emergency department visits and readmissions. A retrospective cohort study was implemented to determine the incidence and etiology of 30-day readmission after microvascular head and neck reconstructive surgery, focusing on social risk factors.

Germ cell tumors in the mediastinum are rare and often occur in young patients but may occur in older patients. Seminoma, embryonal carcinoma, yolk sac tumor, choriocarcinoma and teratoma have distinct morphologic features with high grade nuclei. They are the primary diagnostic consideration in young males but may be lower on the list in older patients, where they may be misdiagnosed as carcinomas.

Decreasing Admissions to the NICU: An Official Transition Bed for Neonates.

Adv Neonatal Care

February 2021

Department of Pediatrics, Division of Neonatology, University of Florida Jacksonville, Jacksonville, Florida (Drs Baker and Alissa); and College of Nursing, University of Florida, Gainesville, Florida (Dr Parker).

Background: Evidence supports the need to decrease healthcare costs. One approach may be minimizing use of low-value care by reducing the number of unnecessary neonatal intensive care unit (NICU) admissions through the use of official neonatal transition beds.

Purpose: To evaluate whether transition beds decrease unnecessary NICU admissions and estimate the cost savings of this practice change.

Endoplasmic reticulum (ER) stress as well as oxidative stress have been shown to play important roles in metabolic and cardiovascular disease, and drugs that counteract the effects of ER and oxidative stresses may be clinically useful. To identify novel compounds that ameliorate ER and oxidative stresses, we screened two drug libraries purchased from Evotec, San Francisco, CA; the NIH clinical collection 1 (446 compounds) and the NIH clinical collection 2 (281 compounds). Human coronary artery endothelial cells (HCAEC) were tested for ER and oxidative stress.

Article Synopsis
  • - The study aimed to explore how the timing and volume of crystalloid fluids administered prior to blood transfusion affects mortality rates in pediatric trauma patients, proposing that earlier blood transfusions with less crystalloid would lead to better outcomes.
  • - Conducted across 24 trauma centers, the study involved 712 children under 18 years with a focus on prehospital and emergency department resuscitation methods, finding that 43.7% received crystalloid boluses and 20.9% received blood products, including significant instances of massive transfusion.
  • - Results showed no significant impact on mortality related to transfusion timing or volume; however, receiving multiple crystalloid boluses was linked to longer recovery times in terms of ventilator support, intensive
