Background: The long-term health-economic consequences of acute stroke are typically extrapolated from short-term outcomes observed in different studies, using models based on assumptions about longer-term morbidity and mortality. Inconsistency in these assumptions and the methods of extrapolation can create difficulties when comparing estimates of lifetime cost-effectiveness of stroke care interventions.
Aims: To develop a long-term model consisting of a set of equations to estimate the lifetime effects of stroke care interventions to promote consistency in extrapolation of short-term outcomes.
Objective: Hypertension is a recognized risk factor for the development of cognitive impairment and dementia in older adults. Aortic stiffness and altered haemodynamics could promote the transmission of detrimental high pressure pulsatility into the cerebral circulation, potentially damaging brain microvasculature and leading to cognitive impairment. We determined whether reservoir-excess pressure parameters were associated with cognitive function in people with hypertension (HT) and normotension (NT).

Yet, there are limited studies examining specific patterns of ACE co-occurrence considering their developmental timing.

Background: The 2015 American College of Medical Genetics/Association of Molecular Pathology (ACMG/AMP) variant classification framework specifies that case-control observations can be scored as 'strong' evidence (PS4) towards pathogenicity.
Methods: We developed the PS4-likelihood ratio calculator (PS4-LRCalc) for quantitative evidence assignment based on the observed variant frequencies in cases and controls. Binomial likelihoods are computed for two models, each defined by prespecified OR thresholds.

Montmorency cherry (MC) can improve endurance performance, but optimal pre-exercise timing of supplementation and influence of training status on efficacy are unknown. We investigated the effect of MC concentrate ingestion between 30- and 150-min pre-exercise in trained and recreational cyclists on 15-km time trial (TT) performance and exercise economy. Twenty participants (10 recreationally active, RA; 10 trained, T) completed 10 min of steady-state exercise (SSE) at 40%Δ (SSE) and a TT on four separate occasions following an unsupplemented (US), 30-, 90- or 150-min pre-exercise Montmorency cherry concentrate (MCC) supplementation conditions (MCC).

Introduction: We undertook phenotypic characterization of early-onset and late-onset type 2 diabetes (T2D) in adult black African and white European populations with recently diagnosed T2D to explore ethnic differences in the manifestation of early-onset T2D.
Research Design And Methods: Using the Uganda Diabetes Phenotype study cohort of 500 adult Ugandans and the UK StartRight study cohort of 714 white Europeans with recently diagnosed islet autoantibody-negative T2D, we compared the phenotypic characteristics of participants with early-onset T2D (diagnosed at <40 years) and late-onset T2D (diagnosed at ≥40 years).
Results: One hundred and thirty-four adult Ugandans and 113 white Europeans had early-onset T2D.

This article reports on a unique case of chronic ischaemia in the lower limb, presenting in a distinctive manner as a fungal toenail infection.

Background/aim: Genomic variants can predispose individuals to adverse drug effects (ADEs), implying the potential for personalised therapy based on genetic data to prevent them. However, existing pharmacogenomic databases lack a comprehensive list of such variants due to irregular updates and incomplete literature coverage. To facilitate the assessment of the feasibility of using pharmacogenetic testing on a larger scale and identify existing gaps in the literature, this study sought to compile a comprehensive list of genomic variants associated with ADEs, with a focus on serious ADEs.

Purpose: This study aimed to develop a new method that more closely represents the heavy to severe exercise domain boundary by evaluating the rates of blood lactate accumulation during the constant power output exercise bouts that are used in the assessment of the maximal lactate steady state (MLSS).
Methods: Eight well-trained male cyclists completed five exercise tests of up to 30 min for the determination of the traditional MLSS (MLSS TRAD ) and a further four maximal tests for the determination of critical power (CP). The rates of change of blood [lactate] between 10 min and the end of exercise in the MLSS tests were plotted against the corresponding power outputs, and a two-segment linear regression model was used to identify individualized break points in lactate accumulation versus power output (modified MLSS [MLSS MOD ]).

Design: Qualitative study using semistructured interviews.
Setting: Telephone and video call interviews and focus groups with professionals working in one of five local authority areas in England.
The search methods used in systematic reviews provide the foundation for establishing the body of literature from which conclusions are drawn and recommendations made. Searches should aim to be comprehensive and reporting of search methods should be transparent and reproducible. Campbell Collaboration systematic reviews strive to adhere to the best methodological guidance available for this type of searching.

Methods: Clinicians in the fields of respiratory medicine, palliative and primary care from five continents with expertise in respiratory medicine and palliative care rated 81 criteria over three Delphi rounds.
