Background And Objective: To determine whether there is disproportionate reporting of hepatobiliary disorders in the United States (US) FDA Adverse Event Reporting System (FAERS) for individuals prescribed ketamine or esketamine.

Design: We identified Medical Dictionary for Regulatory Activities (MedDRA) terms in the FAERS related to hepatobiliary disorders.

Main Measures: Formulations of ketamine and esketamine were evaluated for the proportionality of reporting for each hepatobiliary disorder parameter using the reporting odds ratio (ROR).

Evidence suggests that bats are important hosts of filoviruses, yet the specific species involved remain largely unidentified. Niemann-Pick C1 (NPC1) is an essential entry receptor, with amino acid variations influencing viral susceptibility and species-specific tropism. Herein, we conducted combinatorial binding studies with seven filovirus glycoproteins (GPs) and NPC1 orthologs from 81 bat species.

Environmental contaminants assessment for frequently harvested migratory waterfowl in the Northeast Atlantic flyway.

Waterfowl serve as indicators of ecosystem health and represent a pathway of contaminant exposure for hunters who consume them. In the northeast Atlantic Flyway, data on baseline contaminant loads in waterfowl are lacking. We assessed five species of commonly harvested (and consumed) waterfowl for mercury, polychlorinated dibenzo-p-dioxins and polychlorinated dibenzofurans, polychlorinated biphenyls (PCBs), organochlorine pesticides (OCPs), and per- and polyfluoroalkyl substances (PFAS).

This proposed scientific statement is focused on providing new insights regarding challenges and opportunities for cardiovascular health (CVH) promotion in Africa. The statement includes an overview of the current state of CVH in Africa, with a particular interest in the cardiometabolic risk factors and their evaluation through metrics. The statement also explains the main principles of primordial prevention, its relevance in reducing noncommunicable disease and the different strategies that have been effective worldwide.

Implementation of a Virtual Cardiology Curriculum to Address the Deficit of Cardiovascular Education in Haiti.

Background: Cardiovascular disease (CVD) is the leading cause of death in low- and middle-income countries such as Haiti. Our team has demonstrated in a pilot study the implementation of a virtual cardiology curriculum to address the deficit of cardiology education in Haiti among medicine residents.

Objectives: The objective of this study was to determine if cardiology education can be delivered nationwide in Haiti via a virtual platform with quantifiable improvement.

Pancreatic tuberculosis (TB) is an uncommon extrapulmonary presentation of TB. Identification of coinfection with HIV may unmask not only disseminated TB but also immune reconstitution inflammatory syndrome (IRIS). We present the case of a 70-year-old Indian woman newly diagnosed with AIDS and pancreatic tuberculosis with miliary disseminated disease.

Background: Recent data showed an association between malnutrition and increased all-cause mortality and thromboembolic risk in patients with atrial fibrillation (AF). However, the impact of malnutrition on the clinical outcomes for patients undergoing catheter ablation for AF is still debated. Our study aimed to examine this relationship using all existing available data.

Application of the DRASTIC Model to Assess the Vulnerability of Groundwater Contamination Near Zaporizhzhia Nuclear Power Plant, Ukraine.

Russia's invasion of Ukraine continues to have a devastating effect on the well-being of Ukrainians and their environment. We evaluated a major environmental hazard caused by the war: the potential for groundwater contamination in proximity to the Zaporizhzhia Nuclear Power Plant (NPP). We quantified groundwater vulnerability with the DRASTIC index, which was originally developed by the United States Environmental Protection Agency and has been used at various locations worldwide to assess relative pollution potential.

Background And Objective: As the global population ages, degenerative spinal disorders are on the rise, leading to an increased focus on optimizing spinal fusion therapies. Despite the high success rate of iliac crest bone autografts, their usage is hampered by donor site morbidity and limited supply. The objective of this review is to assess the viability of ceramic-based synthetic materials as alternatives in spinal fusion surgeries.

Warming-induced changes in seasonal priority effects drive shifts in community composition.

Shifting community assembly dynamics are an underappreciated mechanism by which warming will alter plant community composition. Germination timing (which can determine the order in which seedlings emerge within a community) will likely shift unevenly across species in response to warming. In seasonal environments where communities reassemble at the beginning of each growing season, changes in germination timing could lead to changes in seasonal priority effects, and ultimately community composition.

Practices and perspectives of genetic counselors about high-risk pancreatic cancer screening: A cross-sectional survey study.

Surveillance of individuals at high-risk of pancreatic cancer using CAPS criteria and other expert consensus guidelines may result in earlier pancreatic cancer detection in some cases; therefore, clinicians are responsible for appropriately identifying and referring these individuals to appropriate high-risk pancreas cancer screening programs. This study aimed at assessing the perspective, knowledge, and clinical practices of cancer genetic counselors surveyed nationwide towards identification of individuals at high-risk of pancreatic cancer and utilization of high-risk pancreatic cancer screening programs. One hundred and eighty-nine genetic counselors who listed "Cancer" as their specialty on the NSGC website responded to the survey, which consisted of multiple practice-based, knowledge-based, and clinical vignette-based questions.

Seeking Care for Obsessive-Compulsive Symptoms Among African Americans: Findings From the National Survey of American Life.

Although obsessive-compulsive disorder (OCD) is associated with clinically significant distress, many OCD patients do not seek treatment. Studies show that Black Americans with OCD are even less likely to obtain treatment due to differences in access. This study explored demographic and symptom outcomes associated with mental health service use for obsessions and compulsions among a nationally representative sample of African American adults (n = 3,570).

Decoding Extracellular Protein Glycosylation in Human Health and Disease.

Protein glycosylation, the covalent attachment of carbohydrate, or glycan, structures onto the protein backbone, is one of the most complex and heterogeneous post-translational modifications (PTMs). Extracellular protein glycosylation, in particular N- and mucin-type O-glycosylation, plays pivotal roles in a number of biophysical and biochemical processes, such as protein folding and stability, cell adhesion, signaling, and protection. As such, aberrant glycosylation is implicated in a variety of diseases, including cancer.

Investigating Environmental Determinants of Hookworm Transmission using GPS Tracking and Metagenomics Technologies.

To identify potential sources of hookworm infections in a Ghanaian community of endemicity that could be targeted to interrupt transmission, we tracked the movements of infected and noninfected persons to their most frequented locations. Fifty-nine participants (29 hookworm positives and 30 negatives) wore GPS trackers for 10 consecutive days. Their movement data were captured in real time and overlaid on a community grid map.

Monocyte dysfunction contributes to the inflammatory pathogenesis of cystic fibrosis (CF) caused by CF transmembrane conductance regulator (CFTR) mutations, making CF a clinically relevant disease model for studying the contribution of monocytes to inflammation. Although CF monocytes exhibited adhesion defects, the precise mechanism is unclear.

The effect of conflict on damage to medical facilities in Mariupol, Ukraine: A quasi-experimental study.

Medical facilities are civilian objects specially protected by international humanitarian law. Despite the need for systematic documentation of the effects of war on medical facilities for judiciary accountability, current methods for surveilling damage to protected civilian objects during ongoing armed conflict are insufficient. Satellite imagery damage assessment confers significant possibilities for investigating patterns of war.

Artificial Intelligence-Guided Lung Ultrasound by Nonexperts.

Importance: Lung ultrasound (LUS) aids in the diagnosis of patients with dyspnea, including those with cardiogenic pulmonary edema, but requires technical proficiency for image acquisition. Previous research has demonstrated the effectiveness of artificial intelligence (AI) in guiding novice users to acquire high-quality cardiac ultrasound images, suggesting its potential for broader use in LUS.

Objective: To evaluate the ability of AI to guide acquisition of diagnostic-quality LUS images by trained health care professionals (THCPs).

Multicomponent Deimplementation Strategy to Reduce Low-Value Preoperative Testing.

Importance: Routine preoperative blood tests and electrocardiograms before low-risk surgery do not prevent adverse events or change management but waste resources and can cause patient harm. Given this, multispecialty organizations recommend against routine testing before low-risk surgery.

Objective: To determine whether a multicomponent deimplementation strategy (the intervention) would reduce low-value preoperative testing before low-risk general surgery operations.

Palliative Care Initiated in the Emergency Department: A Cluster Randomized Clinical Trial.

Importance: The emergency department (ED) offers an opportunity to initiate palliative care for older adults with serious, life-limiting illness.

Objective: To assess the effect of a multicomponent intervention to initiate palliative care in the ED on hospital admission, subsequent health care use, and survival in older adults with serious, life-limiting illness.

Design, Setting, And Participants: Cluster randomized, stepped-wedge, clinical trial including patients aged 66 years or older who visited 1 of 29 EDs across the US between May 1, 2018, and December 31, 2022, had 12 months of prior Medicare enrollment, and a Gagne comorbidity score greater than 6, representing a risk of short-term mortality greater than 30%.

Self-affirmation intervention for patients newly diagnosed with advanced cancer: a preliminary efficacy trial.
