Ann Emerg Med

August 2023

Department of Pediatrics, University of California Davis Health, University of California, Davis, School of Medicine, Sacramento, CA; Department of Emergency Medicine, University of California Davis Health, University of California, Davis, School of Medicine, Sacramento, CA.

Article Synopsis
  • The study aimed to assess the dehydration levels in children with diabetic ketoacidosis (DKA) and identify factors (clinical and biochemical) linked to its severity.
  • Analyzed data from 753 children showed that the majority experienced mild to moderate dehydration, with only a small percentage being severely dehydrated.
  • Results indicated that while biochemical factors were more related to dehydration severity, neither physical examination nor biochemical measures were strong enough to guide rehydration strategies effectively.

Article Synopsis
  • Diagnostic errors in pediatric emergency departments, rephrased as missed opportunities for improving diagnoses (MOIDs), are not well understood, prompting a survey of physicians to gather insights on their experiences and contributing factors.
  • Out of 1594 surveyed physicians, 412 responded, highlighting that common initial symptoms included abdominal pain, fever, and vomiting, often leading to misdiagnoses like gastroenteritis and viral syndrome.
  • Most MOIDs were detected on follow-up visits, with the most common misdiagnosis being appendicitis, and over half of the cases resulted in moderate to major harm to the patients, primarily due to issues in the patient-provider interactions.

While DNA and RNA helices often adopt the canonical B- or A-conformation, the fluid conformational landscape of nucleic acids allows for many higher energy states to be sampled. One such state is the Z-conformation of nucleic acids, which is unique in that it is left-handed and has a "zigzag" backbone. The Z-conformation is recognized and stabilized by Z-DNA/RNA binding domains called Zα domains.

Differential Structural Features of Two Mutant ADAR1p150 Zα Domains Associated with Aicardi-Goutières Syndrome.

J Mol Biol

April 2023

Department of Biochemistry and Molecular Genetics and RNA Bioscience Initiative, University of Colorado Denver School of Medicine, Aurora, CO 80045, USA; RNA Bioscience Initiative, University of Colorado Denver School of Medicine, Aurora, CO 80045, USA. Electronic address:

The Zα domain of ADARp150 is critical for proper Z-RNA substrate binding and is a key factor in the type-I interferon response pathway. Two point-mutations in this domain (N173S and P193A), which cause neurodegenerative disorders, are linked to decreased A-to-I editing in disease models. To understand this phenomenon at the molecular level, we biophysically and structurally characterized these two mutated domains, revealing that they bind Z-RNA with a decreased affinity.

Outcomes reported in randomized controlled trials for mixed and non-IgE-mediated food allergy: Systematic review.

Clin Exp Allergy

May 2023

COPSAC, Copenhagen Prospective Studies on Asthma in Childhood, Herlev and Gentofte Hospital, University of Copenhagen, Copenhagen, Denmark.

Background: Mixed and non-IgE-mediated food allergy is a subset of immune-mediated adverse food reactions that can impose a major burden on the quality of life of affected patients and their families. Clinical trials to study these diseases are reliant upon consistent and valid outcome measures that are relevant to both patients and clinicians, but the degree to which such stringent outcome reporting takes place is poorly studied.

Objective: As part of the Core Outcome Measures for Food Allergy (COMFA) project, we identified outcomes reported in randomized clinical trials (RCT) of treatments for mixed or non-IgE-mediated food allergy.

A recent randomized trial found that using a bougie did not increase the incidence of successful intubation on first attempt in critically ill adults. The average effect of treatment in a trial population, however, may differ from effects for individuals. We hypothesized that application of a machine learning model to data from a clinical trial could estimate the effect of treatment (bougie vs.

There is increased focus on the role of maternal interventions in the prevention of food allergy in infancy. There is no role for maternal dietary modifications during pregnancy or lactation, such as allergen avoidance, as a means of infant allergy prevention. Although exclusive breastfeeding is the recommended infant nutrition source globally, the effect of breastfeeding on infant allergy prevention remains unclear.

Cognitive function following diabetic ketoacidosis in young children with type 1 diabetes.

Endocrinol Diabetes Metab

May 2023

Department of Pediatrics, University of California Davis Health, University of California Davis, School of Medicine, Davis, California, USA.

Introduction: Young children with type 1 diabetes (T1D) may be at particularly high risk of cognitive decline following diabetic ketoacidosis (DKA). However, studies of cognitive functioning in T1D typically examine school-age children. The goal of this study was to examine whether a single experience of DKA is associated with lower cognitive functioning in young children.

Structure and IgE Cross-Reactivity among Cashew, Pistachio, Walnut, and Peanut Vicilin-Buried Peptides.

J Agric Food Chem

February 2023

Genome Integrity and Structural Biology Laboratory, National Institute of Environmental Health Sciences, 111 T.W. Alexander Drive, MD-MR01, Durham, North Carolina 27709, United States.

Peanut and tree-nut allergies are frequently comorbid for reasons not completely understood. Vicilin-buried peptides (VBPs) are an emerging family of food allergens whose conserved structural fold could mediate peanut/tree-nut co-allergy. Peptide microarrays were used to identify immunoglobulin E (IgE) epitopes from the N-terminus of the vicilin allergens Ara h 1, Ana o 1, Jug r 2, and Pis v 3 using serum from three patient diagnosis groups: monoallergic to either peanuts or cashew/pistachio, or dual allergic.

Introduction: Among critically ill patients undergoing orotracheal intubation in the emergency department (ED) or intensive care unit (ICU), failure to visualise the vocal cords and intubate the trachea on the first attempt is associated with an increased risk of complications. Two types of laryngoscopes are commonly available: direct laryngoscopes and video laryngoscopes. For critically ill adults undergoing emergency tracheal intubation, it remains uncertain whether the use of a video laryngoscope increases the incidence of successful intubation on the first attempt compared with the use of a direct laryngoscope.

The Preferred Reporting Items for Systematic reviews and Meta-Analyses (PRISMA) statement, published in 2009, was designed to help systematic reviewers transparently report why the review was done, what the authors did, and what they found. Over the past decade, advances in systematic review methodology and terminology have necessitated an update to the guideline. The PRISMA 2020 statement replaces the 2009 statement and includes new reporting guidance that reflects advances in methods to identify, select, appraise, and synthesise studies.

Background: A large body of research supports the deleterious effects of adverse childhood experiences (ACEs) on disease susceptibility and health for both the exposed individual and the next generation. It is likely that there is an intergenerational transmission of risk from mother to child; however, the mechanisms through which such risk is conferred remain unknown. The current study evaluated the association between maternal ACEs, neonatal brain development of the amygdala and hippocampus, and later infant negative emotionality at six months of age.

Article Synopsis
  • Current FDA-approved CAR T-cell therapies for B-cell cancers are effective but can lead to serious side effects due to their constant activity.
  • This study compared traditional CAR T-cells with a new type called switchable CAR T-cells (sCAR) that allow for regulated activity in a mouse model of B-cell lymphoma.
  • Results showed that while both CAR T-cell types were effective, sCAR T-cells had better tolerability, improving patient survival and reducing weight loss and related health issues.
