Background: Intraoperative hypotension is very common during surgery and is linked to major organ dysfunction and mortality. Current perioperative blood pressure management is largely based on universal blood pressure thresholds ranging from a mean arterial pressure of 60-70 mmHg. However, the effectiveness of this conventional management remains unproven in prospective randomized trials.

Many countries have published clinical practice guidelines for appropriate clinical decisions, optimal treatment, and improved clinical outcomes in patients with acute coronary syndrome. Developing guidelines that are specifically tailored to the Korean environment is crucial, considering the treatment system, available medications and medical devices, racial differences, and level of language communication. In 2017, the Korean Society of Myocardial Infarction established a guideline development committee.

Predictors for Vulnerable Plaque in Functionally Significant Lesions.

JACC Cardiovasc Imaging

September 2024

Department of Internal Medicine and Cardiovascular Center, Seoul National University Hospital, Seoul National University of College of Medicine, Seoul, South Korea. Electronic address:

Article Synopsis
  • The study investigates vulnerable plaque in significant coronary lesions, associating it with prognostic importance beyond just physical function.
  • Conducted across 5 countries, it involved 95 patients with specific pressure-related measurements, identifying features that predict vulnerable plaques.
  • Results showed a significant link between vulnerable plaque presence and pressure gradient (PPG) and fractional flow reserve (FFR), indicating these measures may help predict plaque vulnerability in low-FFR lesions.

Comparison between head rotation and standard techniques for i-gel™ insertion: a randomized controlled trial.

BMC Anesthesiol

July 2024

Department of Anesthesiology and Pain Medicine, Seoul National University Hospital, Seoul National University College of Medicine, 101 Daehak-ro, Jongno-gu, Seoul, 03080, Republic of Korea.

Background: This study evaluated the effect of head rotation on the first-attempt success rate of i-gel insertion, aiming to alleviate the effect of gravity on the tongue and reduce resistance between the device and the tongue.

Methods: Adult surgical patients were randomized to standard and head rotation technique groups. In the head rotation technique group, patients' heads were maximally rotated to the left before i-gel insertion.

Background: The renin-angiotensin-aldosterone system (RAAS) and adiposity measures are independently associated with the development of diabetes in African American adults. However, studies have not examined the combined interaction between RAAS and adiposity measures in relation to diabetes risk in African American adults.

Objective: We examined the longitudinal association of combined RAAS and adiposity measures with incident diabetes among African American adults in the Jackson Heart Study.

Article Synopsis
  • A study was conducted to improve the prediction of which coronary artery lesions could lead to acute coronary syndrome (ACS) by integrating artificial intelligence (AI) with traditional methods.
  • The research focused on patients who had undergone coronary CT angiography (CTA) before experiencing an ACS event, analyzing both culprit (problematic) and nonculprit lesions.
  • The new model incorporating AI features showed significantly better predictability for identifying high-risk lesions compared to standard methods, suggesting that AI can enhance cardiac risk assessment.

Orbital floor fractures are commonly encountered, but the dislocation of the eyeball into the maxillary sinus is relatively rare. When it does occur, globe dislocation can have serious consequences, including vision loss, enucleation, and orbito-ocular deformity. Immediate surgical intervention is typically attempted when possible.

Porocarcinoma in a palm reconstructed with a full thickness skin graft: A case report.

World J Clin Cases

January 2024

Department of Plastic and Reconstructive Surgery, Konyang University Medical Center, Konyang University of College of Medicine, Daejeon 35365, South Korea.

Background: Porocarcinoma is a rare type of skin cancer that originates from sweat gland tumors. It is an aggressive malignant skin cancer that is difficult to diagnose clinically owing to its rarity and similarity to squamous cell carcinoma (SCC).

Case Summary: This case involved a 92-year-old woman, a farmer by profession, presented with an exophytic and verrucous mass on her left palm that had formed 2 years prior and caused chronic pain and frequent bleeding.

  • Chromosomal microarray (CMA) testing helps doctors look for genetic problems when there are issues with a fetus or during pregnancy losses.
  • In a study with 842 cases from 2011 to 2020, researchers found that CMA showed significant findings in about 7.8% of prenatal tests and 16.3% of tests from miscarriages.
  • The most common problem detected was trisomy 21, which means having an extra copy of chromosome 21, affecting 7 samples in the study.

Background: There remains a limited comprehensive understanding of how dyslipidemia and chronic inflammation collectively contribute to the development of chronic kidney disease (CKD).

Objective: We aimed to identify clusters of individuals with five variables, including lipid profiles and C-reactive protein (CRP) levels, and to assess whether the clusters were associated with incident CKD risk.

Methods: We used the Korean Genome and Epidemiology Study-Ansan and Ansung data.

Comparison of METS-IR and HOMA-IR for predicting new-onset CKD in middle-aged and older adults.

Diabetol Metab Syndr

November 2023

Division of Nephrology, Department of Internal Medicine, Yongin Severance Hospital, Yonsei University College of Medicine, Gyeonggi, Republic of Korea.

Background: Chronic kidney disease (CKD) has emerged as a mounting public health issue worldwide; therefore, prompt identification and prevention are imperative in mitigating CKD-associated complications and mortality rate. We aimed to compare the predictive powers of the homeostatic model assessment for insulin resistance (HOMA-IR) and the metabolic score for insulin resistance (METS-IR) for CKD incidence in middle-aged and older adults.

Methods: This study used longitudinal prospective cohort data from the Korean Genome and Epidemiology Study.
