In Chile the reimbursement/patient/year for chronic peritoneal dialysis (PD) is US$14,654 and for chronic hemodialysis (HD) US$10,909. However, no study comparing global (direct plus indirect) costs has been performed in our country. Our research objective was to compare global costs and quality of life between the two therapies.

Enhanced pro-inflammatory cytokine production is considered a pathogenic factor in non-alcoholic fatty liver disease (NAFLD). Peripheral blood production of interleukin-1alpha (IL-1alpha) and tumor necrosis factor-alpha (TNF-alpha) was studied in relation to the severity of histological changes of the liver in obese NAFLD patients. Basal levels in serum and production of IL-1alpha and TNF-alpha in peripheral blood cell cultures after stimulation with lipopolysaccharide (enzyme-linked immunoabsorbent assays) were measured in 11 patients with steatosis and 15 with steatohepatitis, who underwent gastrectomy with a gastro-jejunal anastomosis in roux and Y, and in 9 controls who underwent anti-reflux surgery.

Background: Lymph node status is the most important prognostic indicator in breast cancer in recently diagnosed primary lesion. As a part of an interregional protocol using scintimammography with Tc99m compounds, the value of planar Tc99m sestamibi scanning for axillary lymph node evaluation is presented. Since there is a wide range of reported values, a standardized protocol of planar imaging was performed.

The aim of this study was to compare the extent and severity of wall motion abnormalities, perfusion and glucose metabolism, in recent myocardial infarction in patients with and without revascularization. Forty-nine patients were studied (82% men; mean age 58 years) by using echocardiography, 201Tl single photon emission computed tomography (SPECT) rest and redistribution, and 2-[18F]fluoro-2-deoxy-D-glucose (18F-FDG) SPECT at a mean of 9.2 days (range, 1-24 days) after myocardial infarction.

Dipeptidyl peptidase IV (DPP IV) (CD26) plays a critical role in the modulation and expression of autoimmune and inflammatory diseases. We recently reported that sera from patients with rheumatoid arthritis and systemic lupus erythematosus contained low levels of DPP IV and high titers of anti-DPP IV autoantibodies of the immunoglobulin A (IgA) and IgG classes and found a correlation between the low circulating levels of DPP IV and the high titers of anti-DPP IV autoantibodies of the IgA class. Since streptokinase (SK) is a potent immunogen and binds to DPP IV, we speculated that patients with autoimmune diseases showed higher DPP IV autoantibody levels than healthy controls as a consequence of an abnormal immune stimulation triggered by SK released during streptococcal infections.

Methods: Serum DPP IV was purified by ion-exchange and affinity chromatography techniques and its specific activity and sera levels were determined by an enzyme-linked assay (ELISA).

The main goal of this work was to know the value of ventricular function in addition to perfusion Tc-99m sestamibi images in the assessment of coronary artery disease (CAD) when using dipyridamole (DIP) associated to isometric exercise. We analyzed 52 patients with suspected CAD; 40 of them had coronary lesions > or = 50% and 12 patients without CAD, conforming study and control groups, respectively. Twenty-eight patients had prior myocardial infarction.

The indications for routine intraoperative cholangiography remain controversial. We present here our recent results concerning the frequency of unknown retained common bile duct stones in 253 consecutive patients who underwent laparoscopic cholecystectomy without intraoperative cholangiography in whom the presence of preoperative choledocholithiasis had been excluded by clinical, biochemical, and ultrasonographic evaluation. These patients were followed up for at least 4 years after surgery with evaluations similar to those made preoperatively.

As the open approach rhinoplasty has gained popularity, newer techniques for aggressive reshaping of the alar cartilage have become available. The most effective way to increase alar dome definition is the dome plication technique described by Daniel, which places a horizontal mattress suture, applied with incremental tension, between the medial and lateral crura. The disadvantage of this technique is that it can produce either overtightening or distortion of the cartilage, with excessive pinching of the nasal tip.

A prospective randomized clinical trial was performed in order to evaluate the results of three surgical techniques for the treatment of gastric outlet obstruction secondary to duodenal ulcer. A new systematization for onlay tip grafting is described, using a minigraft constructed from either irradiated, glutaraldehyde-processed bovine cartilage or soft-grade silicone blocks. The graft design consists of two pyramidal volumes amputated in the apex and carved with mild concave surfaces. The lateral profile of the graft is asymmetrical with an acute inclination in the front and a slight inclination in the back.
