The present review intends to discuss the controversies and strengths in clinically node-positive patients with axillary nodal status ypN i+ / mi after neoadjuvant chemotherapy. Over the past 20 years, a de-escalation approach toward axillary surgery has been observed in patients with breast cancer. The worldwide use of sentinel node biopsy in the upfront setting and after primary systemic therapy substantially reduced surgical complications or late sequelae and eventually improving quality of life of patients.

Article Synopsis
  • Aneurysmal subarachnoid hemorrhage (aSAH)
  • leads to serious complications and high rates of disability and death, with delayed cerebral ischemia (DCI) often occurring without visible vasospasm.
  • A study involving 133 aSAH patients
  • found that higher levels of lactate and glucose in cerebrospinal fluid (CSF) measured shortly after the hemorrhage were strong indicators of neurological decline and poor recovery after 3 months.
  • Monitoring CSF lactate and glucose
  • could help predict worsening neurological conditions after aSAH, highlighting the need for future research with larger patient groups to confirm these findings and improve treatment strategies.

Pathologic evaluation of early breast cancer after neoadjuvant therapy is essential to provide prognostic information based on tumor response to treatment (pathologic complete response [pCR] or non-pCR) and to inform therapy decisions after surgery. To harmonize the pathologist's handling of surgical specimens after neoadjuvant therapy, a panel of experts in breast cancer convened to developed a consensus on six main topics: (1) definition of pCR, (2) required clinical information, (3) gross examination and sampling, (4) microscopic examination, (5) evaluation of lymph node status, and (6) staging of residual breast tumor. The resulting consensus statements reported in this document highlight the role of an accurate evaluation of tumor response and define the minimum requirements to standardize the assessment of breast cancer specimens after neoadjuvant therapy.

Surveillance for hepatocellular carcinoma at the community level: Easier said than done.

World J Gastroenterol

October 2021

Department of Medicine, Policlinico San Marco Zingonia, GSD University and Research Hospital, Osio Sotto 24040, Bergamo, Italy.

Surveillance for hepatocellular carcinoma (HCC) in high-risk patients with semiannual ultrasound examinations is advocated by all international guidelines. However, as long as the identification of the population to be screened and the surveillance programs are not well implemented, the real-life impact of HCC surveillance in reducing mortality for HCC cannot be known. We propose a new approach that promotes the identification of cirrhotic patients by primary care physicians (PCPs) and referral of patients to the hepatologist for surveillance.
