Non-deletional alpha thalassaemia: a review.

Orphanet J Rare Dis

June 2020

Haematology Unit, Department of Pathology, Faculty of Medicine and Health Sciences, University Putra Malaysia (UPM), Serdang, Selangor, Malaysia.

Background: Defective synthesis of the α-globin chain due to mutations in the alpha-globin genes and/or its regulatory elements leads to alpha thalassaemia syndrome. Complete deletion of the 4 alpha-globin genes results in the most severe phenotype known as haemoglobin Bart's, which leads to intrauterine death. The presence of one functional alpha gene is associated with haemoglobin H disease, characterised by non-transfusion-dependent thalassaemia phenotype, while silent and carrier traits are mostly asymptomatic.

The cell membrane is a protective layer that strictly controls the passage of molecules restricting the delivery of biomolecules such as drugs, oligonucleotides, peptides, and siRNA into the cells. This shortcoming has been overcome by the discovery of Cell-Penetrating Peptides (CPPs) that has undergone 30 years of evolution. To date, CPPs are largely modified to improve its efficacy and to suit the different delivery applications.

Article Synopsis
  • Ewing sarcoma (ES) is a rare, aggressive bone tumor primarily affecting children and adolescents, with a unique case of extraosseous Ewing sarcoma (EES) reported in a 46-year-old man presenting nasal problems.
  • The patient's diagnostic process included nasal endoscopy and CT scans, revealing a mass in the nasal cavity, later confirmed as Ewing sarcoma through histological analysis.
  • Despite successful surgery and chemotherapy, follow-up scans indicated potential bone involvement, highlighting the rarity and complexity of diagnosing EES in head and neck regions, which demands careful attention and an early, multi-modal intervention for better outcomes.
