Failure to rescue (FTR) is a standardized patient safety indicator (PSI-04) developed by the Agency for Healthcare Research and Quality (AHRQ) to assess the ability of a healthcare team to prevent mortality following a major complication. However, FTR rates vary and are impacted by non-modifiable individual patient characteristics such as baseline frailty. This raises concerns regarding the validity of FTR as an objective quality metric, as not all patients have the same baseline frailty level, or physiological reserve, to recover from major complications.

Frailty in aneurysmal subarachnoid hemorrhage: the risk analysis index.

J Neurol

October 2023

Department of Neurosurgery, University of New Mexico Health Sciences Center, 1 University New Mexico, MSC10 5615, Albuquerque, NM, 81731, USA.

Background: Few studies have evaluated frailty in the setting of aneurysmal subarachnoid hemorrhage (aSAH) using large-scale data. The risk analysis index (RAI) may be implemented at the bedside or assessed retrospectively, differentiating it from other indices used in administrative registry-based research.

Methods: Adult aSAH hospitalizations were identified in the National Inpatient Sample (NIS) from 2015 to 2019.

High-value care for senior medical students.

Clin Teach

December 2020

Department of Internal Medicine, University New Mexico School of Medicine, Albuquerque, New Mexico, USA.

Background: Physicians have an opportunity to address rising health care costs by optimising resource use. Recent research suggests that regional spending patterns influence student engagement with high- and low-value behaviours, revealing that these behaviours may be adopted early in training. The major curriculum for the internal medicine clinical experience does not address high-value care (HVC).

Who's that SMARTgirl? Reaching Cambodian Female Entertainment and Sex Workers with HIV Prevention Services.

AIDS Behav

March 2020

Division of Prevention Science and Community Health, Department of Public Health Sciences, University of Miami, 1120 NW 14th St., Office 1005, Miami, FL, 33136, USA.

Engagement in prevention services is crucial to reducing HIV risk among female entertainment and sex workers (FESW), and SMARTgirl is the national social marketing HIV prevention program for Cambodian women engaged in sex and entertainment work. Informed by the Behavioral Model of Vulnerable Populations, three multivariate logistic regression analyses examined correlates of three indices of engagement along the SMARTgirl HIV prevention continuum: (1) receipt of outreach services (past 3 months); (2) being registered as a SMARTgirl member; and (3) SMARTgirl club attendance (past year). Among the 1077 FESW enrolled in nine Cambodian provinces, women working in a brothel or freelance (adjusted odds ratio [aOR] 2.

Objectives: At our institution, learner communication skills during an objective structured clinical examination (OSCE) are measured by standardized patients (SPs) using the New Mexico Clinical Communication Scale (NM-CCS). Faculty physicians also conduct a direct observation of a clinical encounter (DOCE) to observe students' interactions with real hospitalized patients. The objective of this study was to determine whether students have similar communication skills scores with real patients as compared with SPs.

Background: HIV prevention for female entertainment and sex workers (FESW) may be optimized by addressing individual and structural risks. We examined the impact of a sequentially delivered intervention to decrease sexual risk, amphetamine-type stimulant (ATS) use, and improve economic well-being in Cambodian FESW.

Methods: A cluster randomized stepped-wedge trial was conducted in 10 Cambodian provinces to test multi-level intervention in high risk FESW.

Objective: To evaluate the patterns of (18)F-FDG uptake at the surgical margin after hepatectomy to identify features that may differentiate benign and malignant uptake.

Methods: Patients who had undergone a PET/CT after hepatectomy were identified. Delay between resection and PET/CT, presence of uptake at the surgical margin, pattern of uptake, and maximal standardized value were recorded.

Physiologic intracranial calcification with hyperintensity on MR imaging: case report and experimental model.

AJNR Am J Neuroradiol

January 1989

Center for Non-Invasive Diagnosis, University New Mexico, School of Medicine, Albuquerque 87131.

CT and MR imaging showed basal ganglia calcification that appeared as high signal intensity on T1-weighted images of a patient with pseudohypoparathyroidism. MR imaging of an experimental model of calcium phosphate suspensions showed a signal similar to that seen in the patient. Additionally, T1 and T2 relaxation times of the solutions were measured and showed decreases in both parameters with increasing calcium phosphate concentrations.
