We report here on a patient with concomitant indolent lymphoma who showed a rapid progressive deterioration of his general condition and emerging neurological symptoms. The combination of severe B symptoms with hypermetabolic involvement of the adrenal glands and multiple central nervous system (CNS) lesions initially suggested a malignant disease. However, when the patient presented to us with biopsy results from one of the CNS lesions, the biopsy revealed granulomatous inflammation but no evidence of malignancy.

Background: Although some evidence suggests that alcohol, substance use, and mental health issues diminish adherence to HIV Pre-Exposure Prophylaxis (PrEP) among gay, bisexual, and other men-who-have-sex-with-men (gbMSM), findings are somewhat inconsistent and have primarily derived from studies involving non-random samples. Medical chart extraction can provide unique insight by in part surmounting sampling-related limitations, as data for entire PrEP clinic populations can be examined. We investigate the influence of the network topology on the asymptotic dynamical patterns, attractors, in a general model of excitable dynamics on signed directed graphs. In this framework, network topology manifests itself as an interplay of positive and negative feedback loops. A small change in a feedback loop, by addition or removal of edges in the graph, can drastically change the dynamical patterns in the network, characterized by the appearance and disappearance of attractors from the attractor space of the network.
