Music therapy can be a form of preventive or early intervention. It strengthens and utilizes women's own resources to build resilience, aids relaxation, reduces symptoms of anxiety and depression, promotes parent-infant attachment, and adapts to physical and psychological challenges during the perinatal period. The inclusion of music therapy in Irish maternity services has the potential to improve the quality of healthcare delivery provided to parents and infants.

Neonatal bacteraemia in Ireland: A ten-year single-institution retrospective review.

PLoS One

August 2024

School of Medicine and Centre for Interventions in Infection, Inflammation and Immunity (4i), University of Limerick, Limerick, Ireland.

Neonatal sepsis is a catastrophic condition of global concern, with reported mortality rates exceeding 10%. Bloodstream infections are an important cause of sepsis, and epidemiological studies of these infections are crucial for predicting the most common aetiological agents and antimicrobial susceptibility patterns and for developing antimicrobial guidelines. For the ten-year study period from July 2013 to June 2023, all neonatal bacteraemia cases were reviewed prospectively using an enhanced surveillance protocol.

Background: Respiratory syncytial virus (RSV) is the world's leading cause of viral acute lower respiratory infections (ALRI) in infants. WHO has identified maternal RSV vaccination a priority and candidate vaccines are in development; however, vaccine hesitancy remains an impediment to successful implementation of maternal immunization. This study, the largest antenatal survey conducted to-date, aimed to examine maternal RSV awareness, likely acceptance of RSV vaccination in pregnancy, and attitudes to maternal vaccination.

Human milk is the preferred source of enteral nutrition for very low birthweight (VLBW) infants, and it possibly decreases dependence on parenteral nutrition (PN) and reduces incidence of late-onset sepsis (LOS). No systematic review to date has specifically addressed the value of early versus late introduction of human milk diet (HMD) on duration of PN and incidence of LOS among VLBW infants. To review the evidence for an early versus late introduction of HMD on duration of PN and incidence of LOS in VLBW infants.

Healthcare practitioners' experiences and perspectives of music in perinatal care in Ireland: An exploratory survey.

Midwifery

May 2024

Specialist Perinatal Mental Health Team, University Maternity Hospital Limerick, Limerick V94C566, Ireland; School of Medicine, University of Limerick, V94DK18, Ireland.

Introduction: Evidence shows that music can promote the wellbeing of women and infants in the perinatal period. Ireland's National Maternity Strategy (2016-2026) suggests a holistic approach to woman's healthcare needs and music interventions are ideally placed as a non-pharmacological and cost-effective intervention to improve the quality of care offered to women and infants. This cross-sectional survey aimed to explore the healthcare practitioners' personal and professional experiences of using music therapeutically and its impact and barriers in practice.

The microbial pathology of maternal perinatal sepsis: A single-institution retrospective five-year review.

PLoS One

December 2023

School of Medicine and Centre for Interventions in Infection, Inflammation, and Immunity (4i), University of Limerick, Limerick, Ireland.

Introduction: Greater than half of in-hospital maternal deaths are caused by sepsis, a condition that occurs when infection exceeds local tissue containment and results in organ dysfunction. Determining the source of infection can be challenging. Microbiological cultures of the uterine cavity are often difficult to obtain, so antimicrobial susceptibility results may not be available to guide treatment.

Article Synopsis
  • Some doctors disagree on when to give platelet transfusions to premature babies with very low blood platelets, which could affect their health.
  • Studies suggest that giving too many platelets might increase the chances of a serious lung problem called bronchopulmonary dysplasia (BPD) in these babies.
  • Researchers want to gather information from different studies to see if giving platelets is connected to BPD or death in preterm infants, helping doctors make better decisions.

Background: Challenges in recruiting appropriately trained obstetricians and gynaecologists have been identified across the world. Given well documented staff shortages within obstetrics and gynaecology in Ireland, it is increasingly important to understand the factors which influence medical students to choose or reject a career in the speciality. The aim of this study was to ascertain the perceptions of final year graduate entry medical students of obstetrics and gynaecology, including the factors which may influence a student's decision to pursue in a career in the speciality.

Objectives: Perinatal substance abuse (PSA) is associated with increased risk of prematurity, low birth weight, neonatal abstinence syndrome, behavioral issues and learning difficulties. It is imperative that robust care pathways are in place for these high-risk pregnancies and that staff and patient education are optimized. The present study explores the knowledge and attitudes of healthcare professionals toward PSA to identify knowledge gaps to enhance care and reduce stigma.

  • The study examined 3 patients with AAC resulting from COVID-19 and analyzed 23 relevant studies, finding a mean patient age of 64.7 years, with common symptoms like fever and abdominal pain, and many patients had underlying conditions such as hypertension and diabetes.* -
  • Results indicated that AAC can occur before, after, or concurrently with COVID-19 pneumonia; the majority of cases were severe (grade II), and treatments included surgery for over half of the patients

Human milk (HM) fortification has been recommended for the nutritional optimization of very low-birthweight infants. This study analyzed the bioactive components of HM and evaluated fortification choices that could accentuate or attenuate the concentration of such components, with special reference to human milk-derived fortifier (HMDF) offered to extremely premature infants as an exclusive human milk diet. An observational feasibility study analyzed the biochemical and immunochemical characteristics of mothers' own milk (MOM), both fresh and frozen, and pasteurized banked donor human milk (DHM), each supplemented with either HMDF or cow's milk-derived fortifier (CMDF).

Changes in preterm birth and stillbirth during COVID-19 lockdowns in 26 countries.

Nat Hum Behav

April 2023

Children's Hospital Research Institute of Manitoba, Department of Pediatrics and Child Health, University of Manitoba, Winnipeg, Manitoba, Canada.

Article Synopsis
  • Preterm birth (PTB) is a major cause of infant mortality globally, and studies show mixed results regarding its rates during COVID-19 lockdowns, with changes ranging from -90% to +30%.
  • Analysis of data from 52 million births in 26 countries indicates modest reductions in PTB rates during the first three months of lockdown, but no significant changes in the fourth month.
  • High-income countries showed an increase in stillbirth risk during the first month of lockdown, with Brazil experiencing increased stillbirth rates throughout the entire lockdown period, highlighting the need for further investigation into these trends.

Background: Respiratory syncytial virus (RSV) is the principal cause of acute lower respiratory infections (ALRI) among infants worldwide, and an important cause of morbidity, hospitalisation and mortality. While infants are universally exposed to RSV, most mortality occurs among normal term infants from low-income and middle-income countries. Breastfeeding has been suggested to have a protective effect against RSV infection.

Despite a number of public health and policy-based initiatives, Ireland's national breastfeeding rates are among the lowest globally. Regionally, the Mid-West of Ireland has historically had low breastfeeding initiation rates, and parts of its major urban area such as Limerick City suffer the highest levels of economic deprivation in the country. In that context, this repeated cross-sectional study analysed breastfeeding initiation trends in the Mid-West of Ireland for two decades, from 2001 to 2020 inclusively.
