The human epidermal growth factor receptor 2 (HER2) gene is a critical biomarker for determining amplification status and targeting clinical therapies in breast cancer treatment. This study introduces a computer-aided method that automatically measures and scores HER2 gene status from invasive tissue regions of breast cancer using whole slide images (WSI) through silver hybridization (SISH) staining. Image processing and deep learning techniques are employed to isolate untruncated and non-overlapping single nuclei from cancer regions.

Article Synopsis
  • Fluorescence in situ hybridization (FISH) is the standard method for checking HER2 status in breast cancer, but it has downsides like needing special training and facing signal degradation; an alternative method, silver-enhanced in situ hybridization (SISH), offers a more durable solution for analysis via bright-field microscopy.* -
  • This study introduces a novel deep learning approach to classify Normal, Amplified, and Non-Amplified regions in HER2-SISH whole slide images, aiming to improve accuracy over traditional methods.* -
  • Using a private dataset, the researchers achieved a striking 99.9% patch-level accuracy and an average accuracy of 98% through k-fold cross-validation, suggesting that this deep

Background: To enhance the quality of care available for children with central nervous system (CNS) tumors across the world, a systematic evaluation of capacity is needed to identify gaps and prioritize interventions. To that end, we created the pediatric neuro-oncology (PNO) resource assessment aid (PANORAMA) tool.

Methods: The development of PANORAMA encompassed 3 phases: operationalization, consensus building, and piloting.

Recent advancements in medical imaging have greatly enhanced the application of computational techniques in digital pathology, particularly for the classification of breast cancer using in situ hybridization (ISH) imaging. HER2 amplification, a key prognostic marker in 20-25% of breast cancers, can be assessed through alterations in gene copy number or protein expression. However, challenges persist due to the heterogeneity of nuclear regions and complexities in cancer biomarker detection.

Breast tissue markers are essential in localising tumours post-neoadjuvant chemotherapy prior to breast-conserving surgery. However, due to the advancement in neoadjuvant therapies, greater efficacy in reducing tumour size increases the possibility of marker migration, potentially compromising surgical outcomes. We report a case of a 34-year-old woman with left breast invasive carcinoma, where the tissue marker, placed under ultrasound guidance before chemotherapy, migrated and was undetectable after eight chemotherapy cycles.

Diagnosis of Chronic Granulomatous Disease: Strengths and Challenges in the Genomic Era.

J Clin Med

Department of Paediatric Immunology and Infectious Diseases, Bristol Royal Hospital for Children, University Hospitals Bristol and Weston NHS Foundation Trust, Upper Maudlin Street, Bristol BS2 8BJ, UK.

Chronic granulomatous disease (CGD) is a group of rare primary inborn errors of immunity characterised by a defect in the phagocyte respiratory burst, which leads to severe and life-threatening infective and inflammatory complications. Despite recent advances in our understanding of the genetic and molecular pathophysiology of X-linked and autosomal recessive CGD, and growth in the availability of functional and genetic testing, there remain significant barriers to early and accurate diagnosis. In the current review, we provide an up-to-date summary of CGD pathophysiology, underpinning current methods of diagnostic testing for CGD and closely related disorders.

Technical tips, diagnostic yield and safety of endobronchial ultrasound-guided transbronchial mediastinal cryobiopsy.

Med J Malaysia

July 2024

Sarawak General Hospital, Department of Medicine, Division of Respiratory Medicine, Jalan Hospital, Kuching, Sarawak, Malaysia.

Endobronchial ultrasound-guided transbronchial needle aspiration (EBUS-TBNA) is commonly used to diagnose and stage lung cancer. In clinical practice, cytology specimens from EBUS-TBNA may be low in cellularity, especially with necrotic lesions. Endobronchial ultrasound-guided transbronchial mediastinal cryobiopsy (EBUS-TBMC) has recently become the preferred method for obtaining histology biopsy.

The ureteral access sheath (UAS) has been a boon and a bane in flexible ureteroscopy (FURS), with its merits and demerits well established. Its design and dimensions were instrumental in reshaping the way flexible scopes were used and were key adjuncts to establishing retrograde intrarenal surgery (RIRS) as a standard of care in the endourological management of renal stones. With the ever-changing landscape of RIRS over the decades shaped by technological advancements in lasers and flexible scopes, the UAS has also continuously evolved.

Purpose: Children with anorectal malformation (ARM) and Hirschsprung's disease (HD) often experience bowel symptoms into adulthood, despite definitive surgery. This study evaluates the quality of life (QOL) and bowel functional outcome of children treated for ARM and HD in comparison to healthy controls.

Methods: Between December 2020 and February 2023, we recruited patients with ARM and HD aged 3-17 years at four tertiary referral centres, who had primary corrective surgery done >12 months prior.

Background: The commonest indication for hospitalization in COVID-19 patients is hypoxemia or severe respiratory symptoms. However, COVID-19 disease may result in extrapulmonary complications including kidney-related pathology. The reported incidence of renal involvement related to COVID infection varies based on geographical location.

Motivators and barriers to the acceptability of a human milk bank among Malaysians.

PLoS One

Faculty of Medicine, Department of Social and Preventive Medicine, University Malaya, Federal Territory of Kuala Lumpur, Kuala Lumpur, Malaysia.

The World Health Organisation (WHO) recommends that all babies be exclusively breastfed, stating that donor milk is the next best alternative in the absence of the mother's own milk. Milk sharing takes many forms, namely wet nursing, co-feeding, cross-feeding, and a human milk bank (HMB). However, the establishment of a human milk bank is still not widely accepted and is a debatable topic because of religious concerns in Malaysia.

Article Synopsis
  • The study aimed to compare oral rehydration therapy (ORT) and intravenous rehydration therapy (IVT) in treating hyperemesis gravidarum (HG) in hospitalized pregnant women.
  • 124 women were randomly assigned to receive either ORT or IVT, with primary outcomes focusing on satisfaction, weight change, and improvement in ketonuria within 12 hours.
  • Results showed that IVT resulted in higher participant satisfaction, more weight gain, and fewer reported vomiting episodes than ORT, leading to a significant number of participants switching from ORT to IVT during the study.

Introduction: The use of dexamethasone (DXM) has been associated with decreased mortality in the patients with hypoxemia during the coronavirus disease-2019 (COVID-19) pandemic, while the outcomes with methylprednisolone (MTP) have been mixed. This real-life study aimed to evaluate the outcomes of patients with severe respiratory failure due to COVID-19 who were treated with high doses of MTP.

Materials And Methods: This retrospective cohort study enrolled hospitalised patients between May 2021 and August 2021, aged 18 years and above, with severe respiratory failure defined by a ratio of oxygen saturation to fraction of inspired oxygen (SF ratio) of less than 235.

Transdermal patches are a non-invasive method of drug administration. It is an adhesive patch designed to deliver a specific dose of medication through the skin and into the bloodstream throughout the body. Transdermal drug delivery has several advantages over other routes of administration, for instance, it is less invasive, patient-friendly, and has the ability to bypass first-pass metabolism and the destructive acidic environment of the stomach that occurs upon the oral ingestion of drugs.
