Cervical necrotizing fasciitis (CNF) is a rare condition in the United States, with very few cases reported as a sequela of mandibular fracture. The authors describe the case of a 40-year-old man with poor oral health and no significant past medical history who developed CNF following delayed treatment of an open mandibular fracture, leading to life-threatening septic shock, thrombophlebitis of the internal jugular vein, and septic emboli to the lungs. Successful management of this patient was accomplished by serial surgical debridement, the administration of broad-spectrum antibiotics, and hemodynamic support.

Therapeutic management and prognostication for patients with B-acute lymphoblastic leukaemia (B-ALL) require appropriate disease subclassification. BCR::ABL1-like B-ALL is unique in that it is defined by a gene expression profile similar to BCR::ABL1+ B-ALL rather than a unifying recurrent translocation. Current molecular/cytogenetic techniques to identify this subtype are expensive, not widely accessible, have long turnaround times and/or require an adequate liquid biopsy.

Purpose: Neonates present unique challenges for pediatric surgical teams. To optimize outcomes, it is imperative to standardize perioperative care by using early extubation and multimodal analgesic techniques. The quadratus lumborum (QL) block provides longer duration and superior pain relief than other single-injection abdominal fascial plane techniques.

Background: Current American Academy of Pediatrics guidelines recommend discharge at physiologic maturity regardless of weight; however, our institution's neonatal ICU policy requires a minimum weight of 1800 g for discharge even when infant is physiologically mature. So, we wanted to determine if discharge at physiologic maturity (PM), based on national guidelines, would decrease hospital days (HD).

Methods: We reviewed 129 infants with birthweight 1300g- 1800 g.

Background: Cystic fibrosis related diabetes (CFRD) is associated with insulin-remediable pulmonary decline, so early detection is critical. Continuous glucose monitors (CGM) have shown promise in screening but are not recommended by clinical practice guidelines. Little is known about the reproducibility of CGM results for a given patient.

The Pandemic Allocation of Ventilators Model Penalizes Infants with Bronchopulmonary Dysplasia.

Children (Basel)

August 2023

Department of Neonatology, University Hospitals Rainbow Babies and Children's Hospital, Case Western Reserve University, Cleveland, OH 44106, USA.

During the COVID-19 pandemic, institutions developed ventilator allocation models. In one proposed model, neonates compete with adults for ventilators using a scoring system. Points are given for conditions that increase one- and five-year (y) mortality.

Purpose Of Review: Cystic fibrosis is a genetic disease that increases risk of death from respiratory failure because of impairment in mucociliary clearance. Complex daily care regimens including medications and airway clearance techniques (ACTs) aim to preserve lung function and alleviate symptoms for people with cystic fibrosis (pwCF). The success of highly effective modulator therapy (HEMT) permits evaluation of treatment simplification.

KAT6A::EP300 fusion in congenital myeloid sarcoma: Yet another novel molecular marker indicating spontaneous remission?: A case report.

Medicine (Baltimore)

July 2023

Department of Pediatrics, Division of Pediatric Hematology-Oncology, University Hospitals-Rainbow Babies and Children's Hospital, Cleveland Medical Center, Cleveland, OH.

Rationale: Acute myeloid leukemia (AML)/myeloid sarcoma (MS) is risk-stratified based on cytogenetics. Although most congenital AML/MS have a dismal prognosis, certain genetic variants such as t (8, 16) [KAT6A::cAMP response element-binding protein (CREB) - binding protein fusion] and more recently t (8, 22) [KAT6A::EP300 fusion] have shown spontaneous remissions. KAT6A located on chromosome 8p11 encodes KAT6A protein, a histone/lysine acetyltransferase enzyme.

Background: The COVID-19 pandemic has led to significant disruptions in medical care, resulting in an estimated 40% of US adults avoiding care. However, the return to baseline health care utilization following COVID-19 restrictions within the pediatric orthopedic population remains unexplored. We sought to analyze the visit volume and demographics of pediatric orthopedic patients at 3 timepoints: prepandemic (2019), pandemic (2020), and pandemic post-vaccine availability (2021), to determine the impact of COVID-19 restrictions on our single-center, multisite institution.

Article Synopsis
  • SARS-CoV-2 infection can lead to long-term health issues known as post-acute sequelae of SARS-CoV-2 infection (PASC) or long COVID, which can manifest as ongoing or new symptoms after the initial infection.
  • The RECOVER-Adult study aims to better understand PASC by investigating its prevalence, symptoms, risk factors, and underlying biological mechanisms through a large cohort of nearly 15,000 adults.
  • Participants will provide ongoing data through questionnaires, physical examinations, and biological samples over several months, helping researchers gather critical insights into the complexities of long COVID.

  • Tibial spine fractures (TSFs) are rare but can lead to serious issues in children, with no universally accepted treatment method established.
  • A systematic review analyzed 47 studies involving 1922 pediatric TSFs, concluding that both open and arthroscopic surgeries were commonly used, with a slight preference for arthroscopic techniques.
  • The findings indicated nonunion rates in certain fracture types, higher risk of range of motion loss in severe fractures, and secondary ACL injuries more prevalent in less severe cases.

Article Synopsis
  • This text talks about how doctors use ultrasounds to check for hydronephrosis, a condition where kidneys swell due to urine buildup. Different ways to grade this problem exist but they aren’t always super accurate.
  • Researchers are working on a computer program using machine learning (like artificial intelligence) to help classify the severity of hydronephrosis based on ultrasound images more reliably.
  • In their study, they tested this new computer model against expert doctors and found that the machine was pretty good at predicting the kidney condition correctly, with high accuracy rates!

Background: Outcomes for children with high-risk renal (HRR) and INI-1-deficient (INI-) tumors are unacceptably poor. Concerns about excessive toxicity-as many are infants and/or undergo nephrectomy-have resulted in decreased chemotherapy dosing and omission of the nephrotoxic drug ifosfamide in collaborative group studies. As cause of death for children with these cancers remains overwhelmingly more from progressive disease rather than treatment toxicity, we examined the tolerability of an intensive ifosfamide-containing regimen.
