Early detection of focal cortical dysplasia (FCD) using brain MRI in young children presenting with drug-resistant epilepsy may facilitate prompt surgical treatment, resulting in better control of seizures and decreased associated cognitive difficulties. Characteristics of FCD described in the literature are predominantly based on MRI findings in a fully myelinated brain; therefore, changes occurring during early brain maturation are not well known. In this case report, we describe distinct MRI features of a FCD visualized best before completion of myelination of the cortex and subcortical white matter.

Background: Concerns persist regarding the cognitive safety of achieving very low levels of low-density lipoprotein (LDL) cholesterol. Although short-term studies are reassuring, the long-term cognitive effects of sustained exposure to very low LDL cholesterol levels through combined proprotein convertase subtilisin-kexin type 9 (PCSK9) inhibition and statin therapy remain unknown.

Methods: This prospective study enrolled a subset of adults with atherosclerotic cardiovascular disease who had completed a neurocognitive substudy (EBBINGHAUS) of a placebo-controlled randomized trial of evolocumab (FOURIER) and were eligible for a long-term open-label extension.

Since the late 1980s, the standard approach for treating D-transposition of the great arteries has been the arterial switch operation (ASO), replacing the Mustard/Senning procedure. Although ASO has shown impressive long-term survival rates, recent case series have revealed late complications such as neoaortic dilation and coronary artery stenosis. New findings emphasize the need for comprehensive evaluation of coronary risk and a deeper understanding of the mechanisms leading to coronary artery stenosis and myocardial ischemia over the long term.

EXPRESS: Unpacking mentalizing: the roles of age and executive functioning in self-other appraisal and perspective taking.

Q J Exp Psychol (Hove)

December 2024

Developmental Clinical Psychology Research Unit, Faculty of Psychology and Educational Sciences, University of Geneva, Switzerland.

Mentalizing involves a number of psychological processes designed to appraise self and others from different points of view. Factors affecting the flexibility in the ability to switch between self-other appraisals and perspectives remain yet unclear. In this study, we sought to (1) assess individual variability in processing and switching between self and other-oriented mental representations and perspectives in a sample of typically developing youths; (2) examine how age and executive functioning may affect this switching process.

Purpose: Transient arginine vasopressin deficiency (AVP-D), previously called diabetes insipidus, is a well-known complication of transsphenoidal pituitary surgery (TPS) with no definite predictive biomarker to date making it difficult to anticipate. While oxytocin (OXT) was previously suggested as a possible biomarker to predict syndrome of inappropriate diuresis (SIAD)-related hyponatraemia after TPS, its secretion in patients presenting with AVP-D remains poorly understood. We therefore hypothesized that OXT might present a different secretion in the case of AVP-D which would support its potential as an early biomarker of AVP-D.

Article Synopsis
  • Intestinal ischemia-reperfusion injury can lead to both large-scale (macrocirculatory) and small-scale (microcirculatory) blood flow failures, and the study aimed to test if combining a vasoconstrictor (norepinephrine) with a vasodilator (ilomedin) improves blood pressure and circulation after injury.
  • The research was conducted on 21 pigs, with three groups receiving different treatments before and after a 2-hour induced small bowel ischemia followed by reperfusion, measuring various circulatory parameters and biomarkers.
  • Results showed no significant differences in mean arterial pressure or cardiac index between the groups, but the combination therapy group had notably better microcirculation performance immediately after and two

  • This study assesses the outcomes of adrenalectomy for pheochromocytoma in obese versus nonobese patients, using data from 46 centers from 2012 to 2022.
  • It finds that while obesity does not significantly increase the rate of complications or comprehensive complication index (CCI), it does lead to a longer hospital stay for patients.
  • Furthermore, minimally invasive surgical techniques, like laparoscopic and robotic surgeries, are associated with less morbidity in obese patients, and a retroperitoneal approach may provide additional benefits.

Failure of neuromuscular blockade despite high doses of rocuronium and atracurium in a patient with colorectal carcinoma: A case report.

Eur J Anaesthesiol

January 2025

From the Division of Anaesthesiology, Department of Anaesthesiology, Pharmacology, Intensive Care and Emergency Medicine, University Hospitals of Geneva, Geneva, Switzerland (ADSR, MB, RS, GA, ES), Department of Anaesthesia, Northern Lisbon University Hospital Centre, Lisbon, Portugal (ADSR), University Clinic of Anesthesiology and Reanimation, Faculty of Medicine, University of Lisbon, Portugal (ADSR), Division of Clinical Pharmacology and Toxicology, Department of Anaesthesiology, Pharmacology and Intensive Care, University Hospitals of Geneva, Geneva, Switzerland (AN).

Resistance to the effects of rocuronium and other neuromuscular blocking agents (NMBA) has been previously reported, including delayed onset of relaxation, rapid recovery and incomplete paralysis under recommended doses. These conditions have been associated with denervation injury, burns, immobilisation, infections, metabolic disorders, and drug interactions. In this report, we describe the case of a young male without any known pre-disposing factor for NMBA resistance, who failed to attain muscle relaxation with non-depolarising NMBA despite high doses of rocuronium and atracurium during two surgeries for colorectal cancer, several months apart.

Adults and children with cholestatic liver disease are at risk for type C hepatic encephalopathy (HE) and may present lifelong neurocognitive impairment. While the underlying cellular and molecular mechanisms are still incompletely understood, ammonium and bile acids (BAs) seem to play a key role in this pathology, by crossing the blood-brain-barrier and modifying neuronal homeostasis and synaptic plasticity. This experimental study aimed to investigate the effects of ammonium and BAs on dendritic spines of rat hippocampal CA1 neurons.

Article Synopsis
  • Dementia, particularly with severe behavioral and psychological symptoms (BPSD), greatly affects quality of life and is a main cause of death in older adults; this study looked at factors predicting mortality in these patients.
  • Over 4 years, 32.5% of 508 participants with severe BPSD died, with non-survivors generally being older, more likely male, and showing greater symptom severity and lower cognitive and functional capabilities at the start.
  • Key mortality predictors identified were male sex, older age at diagnosis, higher BPSD severity scores, lower cognitive function, worse daily living capabilities, and lower quality of life, while the use of antidepressants was linked to a reduced mortality risk.

Article Synopsis
  • - The study investigates the retinal phenotype in two siblings with new genetic variants linked to hereditary spastic paraplegia type 56 (HSP 56), which resemble type 2 macular telangiectasis (MacTel).
  • - Five family members underwent extensive ophthalmic evaluations and genetic testing, revealing that the affected siblings exhibited specific retinal anomalies, including loss of retinal transparency and abnormal pigment distribution.
  • - The findings suggest a potential connection between the observed retinal issues and the genetic variants, indicating a shared pathway in the development of both MacTel and the hereditary condition.
