Clustering COVID-19 ARDS patients through the first days of ICU admission. An analysis of the CIBERESUCICOVID Cohort.

Crit Care

March 2024

Centro de Investigación Biomédica en Red en Enfermedades Respiratorias (CIBERES), Instituto de Salud Carlos III, Madrid, Spain.

Background: Acute respiratory distress syndrome (ARDS) can be classified into sub-phenotypes according to different inflammatory/clinical status. Prognostic enrichment was achieved by grouping patients into hypoinflammatory or hyperinflammatory sub-phenotypes, even though the time of analysis may change the classification according to treatment response or disease evolution. We aimed to evaluate when patients can be clustered in more than 1 group, and how they may change the clustering of patients using data of baseline or day 3, and the prognosis of patients according to their evolution by changing or not the cluster.

Metabolic biomarkers, particularly glycated hemoglobin and fasting plasma glucose, are pivotal in the diagnosis and control of diabetes mellitus. Despite their importance, they exhibit limitations in assessing short-term glucose variations. In this study, we propose labile hemoglobin as an additional biomarker, providing insightful perspectives into these fluctuations.

Effects of Sesquiterpene Lactones on Primary Cilia Formation (Ciliogenesis).

Toxins (Basel)

October 2023

Research Unit, Biomedical Research and Innovation Institute of Cádiz (INiBICA), Department of Urology, University Hospital of Jerez de la Frontera, 11407 Jerez, Spain.

Sesquiterpene lactones (SLs), plant-derived metabolites with broad spectra of biological effects, including anti-tumor and anti-inflammatory, hold promise for drug development. Primary cilia, organelles extending from cell surfaces, are crucial for sensing and transducing extracellular signals essential for cell differentiation and proliferation. Their life cycle is linked to the cell cycle, as cilia assemble in non-dividing cells of G/G phases and disassemble before entering mitosis.

Community-acquired pneumonia represents the third-highest cause of mortality in industrialized countries and the first due to infection. Although guidelines for the approach to this infection model are widely implemented in international health schemes, information continually emerges that generates controversy or requires updating its management. This paper reviews the most important issues in the approach to this process, such as an aetiologic update using new molecular platforms or imaging techniques, including the diagnostic stewardship in different clinical settings.

Background: Diagnosing COVID-19-associated pulmonary aspergillosis (CAPA) can be challenging since radiological and clinical criteria in the critically ill patient are nonspecific. Microbiological diagnostic support is therefore crucial. The aim of this study was to document the incidence of aspergillosis using bronchoalveolar lavage (BAL) as the diagnostic method and to determine the performance of the current mycological diagnostic tests most widely used for the diagnosis of CAPA, together with evaluation of the Asp lateral flow device (LFD).

Procalcitonin and C-reactive protein to rule out early bacterial coinfection in COVID-19 critically ill patients.

Intensive Care Med

August 2023

CIBER de Enfermedades Respiratorias (CIBERES), Instituto de Salud Carlos III, Madrid, Spain.

Article Synopsis
  • The study investigates the effectiveness of procalcitonin (PCT) and C-reactive protein (CRP) as biomarkers for identifying bacterial coinfection in COVID-19 patients admitted to ICUs in Spain.
  • Of 4,076 patients studied, only 3% had bacterial coinfection, and while PCT and CRP showed high negative predictive values, their overall predictive capability was found to be low.
  • The findings indicate that measuring PCT and CRP at hospital admission is not a reliable method for diagnosing bacterial coinfection in COVID-19 pneumonia patients.
