An increasing number of studies have highlighted the existence of a sex-specific immune response, wherein men experience a worse prognosis in cases of acute inflammatory diseases. Initially, this sex-dependent inflammatory response was attributed to the influence of sex hormones. However, a growing body of evidence has shifted the focus toward the influence of chromosomes rather than sex hormones in shaping these inflammatory sex disparities.

Article Synopsis
  • Macrocephaly is when head size is significantly larger than average for a child's age and sex, and the case presented involves a 16-month-old boy who had congenital progressive macrocephaly and required a shunt for hydrocephalus.
  • After initially experiencing complications following the shunt placement, including seizures and anomalies in brain wave patterns, further examination revealed abnormal brain blood vessels indicative of a dural arteriovenous fistula (DAVF).
  • The DAVF, which is a rare but significant cause of macrocephaly, was treated successfully with interventional procedures, highlighting the importance of carefully monitoring head size in diagnosing related conditions.

Pediatric anesthesia in Europe: Variations within uniformity.

Paediatr Anaesth

September 2024

Department of Anesthesiology, Pharmacology, Intensive Care and Emergency Medicine, University Hospitals of Geneva, Geneva, Switzerland.

Article Synopsis
  • The organization of healthcare and requirements for postgraduate medical training in pediatric anesthesia are inconsistent across European countries, influenced by country-specific regulations.
  • The European Board of Anaesthesiology has established advisory training standards, but these are not mandatory, leading to significant variations in pediatric anesthesia training practices, such as supervision requirements and clinical experience in different countries.
  • Most European countries lack official pediatric anesthesia fellowship programs, and while there is growing interest in creating a unified European pediatric anesthesia curriculum, it remains unclear how these training differences impact clinical outcomes in pediatric care.

Choledocholithiasis is one of the most common indications for endoscopic retrograde cholangiopancreatography (ERCP) in daily practice. Although the majority of stones are small and can be easily removed in a single endoscopy session, approximately 10-15% of patients have complex biliary stones, requiring additional procedures for an optimum clinical outcome. A plethora of endoscopic methods is available for the removal of difficult biliary stones, including papillary large balloon dilation, mechanical lithotripsy, and electrohydraulic and laser lithotripsy.

Article Synopsis
  • Many SGA (small for gestational age) patients have particular syndromes that can complicate understanding their response to rhGH (recombinant human growth hormone) treatment; this study examines these patients and their growth outcomes.
  • The study analyzed 272 SGA patients in Belgium, identifying 42 with syndromic conditions, which made them generally younger, shorter, and thinner when they began rhGH treatment compared to non-syndromic patients.
  • While both groups had similar initial responses to rhGH, syndromic patients showed different growth patterns, gaining more height before puberty but less during puberty, and still had lower average adult heights compared to non-syndromic patients.

Collaboration between explainable artificial intelligence and pulmonologists improves the accuracy of pulmonary function test interpretation.

Eur Respir J

May 2023

Laboratory of Respiratory Diseases and Thoracic Surgery, Department of Chronic Diseases Metabolism and Ageing, KU Leuven, Leuven, Belgium

Background: Few studies have investigated the collaborative potential between artificial intelligence (AI) and pulmonologists for diagnosing pulmonary disease. We hypothesised that the collaboration between a pulmonologist and AI with explanations (explainable AI (XAI)) is superior in diagnostic interpretation of pulmonary function tests (PFTs) than the pulmonologist without support.

Methods: The study was conducted in two phases, a monocentre study (phase 1) and a multicentre intervention study (phase 2).

Automated breast ultrasound is a three-dimensional ultrasonographic technique allowing the evaluation of women with dense glandular breast tissue. In this group of patients, mammography has a low sensitivity because dense breasts can obscure breast cancer on mammogram. On the other hand, women with dense breast tissue, types C and D on the BI-RADS scale, are at an increased risk of developing breast cancer compared to women with fatty breast tissue.
