The incidence of early-onset colorectal cancer (eoCRC) is rising, and its pathogenesis is not completely understood. We hypothesized that machine learning utilizing paired tissue microbiome and plasma metabolome features could uncover distinct host-microbiome associations between eoCRC and average-onset CRC (aoCRC). Individuals with stages I-IV CRC (n = 64) were categorized as eoCRC (age ≤ 50, n = 20) or aoCRC (age ≥ 60, n = 44).

Background: Natural killer (NK) cells are non-antigen specific innate immune cells that can be redirected to targets of interest using multiple strategies, although none are currently FDA-approved. We sought to evaluate NK cell infiltration into tumors to develop an improved understanding of which histologies may be most amenable to NK cell-based therapies currently in the developmental pipeline.

Methods: DNA (targeted/whole-exome) and RNA (whole-transcriptome) sequencing was performed from tumors from 45 cancer types (N = 90,916 for all cancers and N = 3365 for prostate cancer) submitted to Caris Life Sciences.

Deleterious effects of environmental exposures may contribute to the rising incidence of early-onset colorectal cancer (eoCRC). We assessed the metabolomic differences between patients with eoCRC, average-onset CRC (aoCRC), and non-CRC controls, to understand pathogenic mechanisms. Patients with stage I-IV CRC and non-CRC controls were categorized based on age ≤ 50 years (eoCRC or young non-CRC controls) or  ≥ 60 years (aoCRC or older non-CRC controls).

Article Synopsis
  • A study explored the significance of poor prostate-specific antigen (PSA) response to neoadjuvant androgen deprivation therapy (ADT) in men with high-risk prostate cancer receiving radiation therapy, indicating potential for worse long-term outcomes.!
  • Out of 930 men analyzed, 57% had a poor PSA response (PSA >0.5 ng/mL), linked to increased risks of metastasis, biochemical recurrence, prostate-specific mortality, and overall survival rates.!
  • The findings suggest that patients with elevated PSA levels post-ADT might face more severe disease progression, highlighting the need for closer monitoring and tailored treatment strategies in this group.!

Purpose: Pelvic radiation therapy may lead to decreased bone mineral density (BMD) and increased risk of fracture that could be of particular concern in patients with prostate cancer who also receive androgen deprivation therapy (ADT). We performed an exploratory analysis of a randomized, double-masked, placebo-controlled trial to determine whether exposure to prior pelvic external beam radiation therapy (XRT) affects BMD and risk of fracture in patients with prostate cancer treated with ADT.

Methods And Materials: Patients with nonmetastatic prostate cancer aged ≥70 years or <70 years with low BMD (T-score < -1) or osteoporotic fracture who had been receiving ADT for ≥12 months were randomly assigned to receive densoumab or placebo every 6 months for 3 years.

Article Synopsis
  • A recent review of abstracts from the American Society of Clinical Oncology (ASCO) meetings (2020-2022) focused on the examination of gender disparities in cancer research while also looking at racial/ethnicity disparities.
  • Out of 1219 abstracts presented, only 7 addressed gender disparities, with just 2 (29%) also investigating racial disparities, indicating a significant lack of integrated research.
  • The findings suggest that over 70% of gender disparities research has been conducted separately from racial disparities, highlighting the need for combined studies to enhance understanding and develop effective solutions in oncology.

We investigated whether inter-patient variation in the dynamic trajectory of hemoglobin (Hb), neutrophil to lymphocyte ratio (NLR), platelet to lymphocyte ratio (PLR), lymphocyte to monocyte ratio (LMR), and prostate-specific antigen (PSA) can prognosticate overall survival (OS) in de novo mHSPC. This is a secondary analysis of the LATITUDE trial in which high-risk de novo mHSPC patients were randomly assigned to receive either androgen deprivation therapy (ADT) plus abiraterone or ADT plus placebo. We used a five-fold cross-validated joint model approach to determine the association of temporal changes in the serological markers with OS.

Purpose: We conducted a systematic review to evaluate the outcome of patients with early-stage (stages I-III) mismatch repair deficient (dMMR) colorectal cancer (CRC) receiving neoadjuvant immunotherapy (NIT) with immune checkpoint inhibitor (ICI)-based regimens.

Methods: MEDLINE, Scopus, Embase, Web of Science, and Cochrane Central Register of Controlled Trials were searched for publications reporting the outcome of patients with early-stage dMMR CRC receiving NIT. The primary outcome measures were the complete response (CR) rate (clinical CR [cCR] or pathologic CR [pCR]) and the incidence of grade 3 or higher toxicities.

Improved maintenance treatments are needed for patients with relapsed/refractory aggressive lymphomas after autologous haematopoietic stem cell transplantation (ASCT). Several studies with lenalidomide have been found to have activity in the treatment of relapsed/refractory aggressive lymphomas. In the present phase I/II, single-arm, open-label study, 59 patients with high-risk relapsed non-Hodgkin lymphoma received pretransplant BEAM chemotherapy and ASCT followed by 12 months of maintenance lenalidomide once daily on Days 1-21 (28-day cycles) beginning at post-transplantation Day 100.

Background: We performed a secondary analysis of ACIS study to determine if synchronous versus metachronous metastatic presentation has any association with survival and treatment response to dual androgen receptor axis-targeted therapy (ARAT) in docetaxel naïve metastatic castrate resistant prostate cancer (mCRPC).

Methodology: In this phase III randomized controlled trial, docetaxel naïve mCRPC patients were randomized to either apalutamide or placebo combined with abiraterone and prednisone. Multivariable Cox regression models were applied to determine the adjusted association of M-stage with radiographic progression-free survival (rPFS) and overall survival (OS).

Article Synopsis
  • Lenalidomide maintenance following autologous stem cell transplant (ASCT) provides better progression-free and overall survival for multiple myeloma (MM) patients, but high-risk multiple myeloma (HRMM) patients don't see the same benefits.
  • A study examined 503 HRMM patients to compare outcomes of lenalidomide versus bortezomib-based maintenance therapies post-ASCT, finding that those on lenalidomide had better survival rates.
  • Results showed that while 67% of the patients were on lenalidomide, they had a 75% progression-free survival rate at 2 years compared to 63% for those on bortezomib, and overall survival was also higher

Article Synopsis
  • Fibula free flaps (FFF) have traditionally been used for mandibular reconstruction, but scapular system free flaps (SFF) are gaining traction due to their versatility and fewer complications at the donor site.
  • A study reviewed 176 patients who underwent either FFF or SFF reconstructions; operative times were similar, but SFF showed less donor site morbidity.
  • The results indicate that SFF could be a preferred choice for complex mandibular defects, thanks to its quicker harvesting process and reduced complications compared to FFF.

Context: Randomized controlled trials (RCTs) have shown that addition of docetaxel or androgen receptor axis-targeted therapy (ARAT) to androgen deprivation therapy (ADT) or addition of ARAT to ADT and docetaxel improves overall survival (OS) in metastatic hormone-sensitive prostate cancer (mHSPC). However, it is unknown whether docetaxel, when given as part of triplet therapy, has an independent OS benefit.

Objective: To compare the efficacy of ADT plus ARAT with the triplet of ADT, ARAT, and docetaxel through a network meta-analysis (NMA) of RCTs in mHSPC.

The Accreditation Council of Graduate Medical Education mandates that all internal medicine residents gain exposure to internal medicine subspecialties including hematology and oncology. While many residents meet this criterion through inpatient oncology rotations, the current structure of many inpatient oncology rotations leaves little opportunity for formal education. We therefore designed a novel oncology curriculum consisting of one-page oncology teaching sheets to increase the number, breadth, and quality of formal teaching sessions on our resident inpatient oncology services.
