Introduction: The COVID-19 pandemic has had a major impact on the Healthcare System, changing the patterns of Emergency Department access. In fact, accesses for trauma and less severe cases decreased significantly. This decline has generally been attributed to both the effects of the lockdown, imposed by the government, and the fear of being infected by SARS-CoV-2 in the hospital.

Since SARS-CoV-2 emerged in 2019, strict monitoring of post-COVID-19 patients in order to ensure the early detection of sequelae and/or chronic organ damage that could been associated with the infection has been essential. Potential involvement of the NO pathway in the development of post-COVID-19 lung fibrotic alterations is feasible, since the majority of respiratory cells can produce NO, and fractional exhaled NO (FeNO) represents a biomarker of airway inflammation. The aim of this study was to investigate the potential utility of multiple-flow FeNO parameters in a post-COVID-19 population and to compare it with other indicators of lung damage proposed in the literature.

Article Synopsis
  • The study aimed to evaluate the efficacy of SB5, a biosimilar of adalimumab, in treating noninfectious uveitis (NIU) due to concerns regarding immunogenicity and safety.
  • A retrospective analysis was conducted on patients treated with SB5, measuring factors like uveitis relapses, visual acuity, and medication dosage before and after starting treatment.
  • Results showed a dramatic reduction in uveitis relapses, improved visual acuity, significantly lowered glucocorticoid usage, and a high retention rate for SB5 over 20 months, indicating its effectiveness in managing NIU.

Recent expiry of patents for tumor necrosis factor (TNF)-α inhibitors has led to the employment of biosimilars in clinical practice. The aim of the study was to identify any change in the control of ocular inflammatory manifestations among patients with non-infectious uveitis switching from an originator to a corresponding anti-TNF-α biosimilar. Thirty-seven consecutive patients (62 eyes involved) with non-infectious uveitis undergoing the switch from anti-TNF-α originators to biosimilars were retrospectively enrolled; the frequency of ocular flares before and after the switch as well as best corrected visual acuity (BCVA), central macular thickness (CMT), daily systemic corticosteroid intake, and frequency of uveitic macular edema (UME) at the switch and at the following assessments were statistically analysed.

Introduction: This case report describes the management and the possible therapeutic solutions for the treatment of femoral bone loss associated with an open fracture IIIB Gustilo Andersonin a polytrauma that includes floating knee (GF) Fraser IIB from damage control orthopedics to final treatment.

Case Report: The patient was treated with an external fixator femorotibial bridge after extensive cleaning and debridement of open fractures. After 17-day post-trauma, we substitute the fixator with a less invasive stabilization system plate and screws with contralateral allograft bone strut.

Article Synopsis
  • Quantitative coronary angiography (QCA) is traditionally used to assess stent results but may miss suboptimal deployment features, prompting the analysis of optical coherence tomography (OCT) in this context.
  • In a study of 125 lesions in patients with major cardiac events, 105 exhibited optimal QCA results, yet OCT revealed suboptimal stent placement in 54% of these cases, highlighting significant discrepancies between the two methods.
  • The findings underscore the importance of using OCT for a more comprehensive evaluation of stent deployment, as optimal angiographic results can still correlate with suboptimal outcomes.

June 2018

Department of Cardiac Surgery, IRCCS Policlinico San Donato, Via Morandi, 30-20097 San Donato Milanese (Milan), Italy.

Aims: The age, creatinine, and ejection fraction (ACEF) score was introduced in 2009 and is presently included in the guidelines for myocardial revascularization of the European Society of Cardiology and Association for Cardio-Thoracic Surgery as a risk stratification tool for surgical and percutaneous myocardial revascularization. The present study introduces an updated version of the ACEF (ACEF II) inclusive of emergency surgery and pre-operative anaemia.

Methods And Results: The development series includes 7011 consecutive cardiac surgery patients operated at a single institution.

New therapeutic solutions for Behçet's syndrome.

Expert Opin Investig Drugs

July 2016

a Research Center of Systemic Autoinflammatory Diseases and Behçet's Disease Clinic, Department of Medical Sciences, Surgery and Neurosciences , University of Siena, Siena , Italy.

Introduction: Behçet's syndrome (BS) is a systemic inflammatory disorder characterized by a wide range of potential clinical manifestations with no gold-standard therapy. However, the recent classification of BS at a crossroads between autoimmune and autoinflammatory syndromes has paved the way to new further therapeutic opportunities in addition to anti-tumor necrosis factor agents.

Areas Covered: This review provides a digest of all current experience and evidence about pharmacological agents recently described as having a role in the treatment of BS, including interleukin (IL)-1 inhibitors, tocilizumab, rituximab, alemtuzumab, ustekinumab, interferon-alpha-2a, and apremilast.
