Klippel-Feil Syndrome with Sprengel Deformity.

J Radiol Case Rep

May 2019

Department of Diagnostic Imaging, University Hospital St. Ekaterina, Medical University of Sofia, Sofia, Bulgaria.

Coexistence of Klippel-Feil syndrome with Sprengel deformity and omovertebral bone is a rare complex bone abnormality with unknown incidence and etiology. Herein, we report a case of a 6-year-old girl with coexistence of these congenital abnormalities evaluated by three-dimensional computed tomography. We also make a brief review and discuss in details the role of this imaging modality in the evaluation of such complex cases.

Total hip arthroplasty (THA) is one of the most successful and cost-effective interventions in medicine nowadays, providing reliable pain relief and functional improvement to those with osteoarthritis or inflammatory arthritis of the hip (P Kinov, B Antonov,'Possibilities for surgical treatment of acetabular osteolysis subsequent to revision arthroplasty of hip joint', Orthop Trauma. 52, 2015). Revision hip arthroplasty or severe reconstructive procedures in cases with significant anatomical defects require skilled surgical staff and accurate preoperative planning, including bone insufficiency, deficiency, discontinuity (anatomical assessment) as well as augmentation, cage or other requirements.

The claustrum is a subcortical nucleus, found in the telencephalon of all placental mammals. Earlier Golgi studies have mostly focused on a qualitative description of the types of neurons. The aim of the present study was to describe the types of neurons found in the dorsal claustrum of the cat using the Golgi impregnation method and to perform a quantitative analysis of the following morphometric parameters: number of terminals (ends), total dendritic length, dendritic complexity, spine density (in spiny projection neurons), varicosity density (in aspiny interneurons).

Background: Recent reports in rat models have shown that fibroblasts in the epiligament, an enveloping tissue of the ligament, are not static cells and play an important role during the early ligament healing of isolated grade III injury of the collateral ligaments of the knee. Fibroblasts produce collagen types I, III and V and infiltrate within the ligament body via the endoligament. In addition, similarities have been reported between the structure of the epiligament of the medial collateral ligament and anterior cruciate ligament of the knee in rat and in human.

The medial collateral ligament of the knee joint is one of the most commonly injured ligaments of the knee. Recent data have shown that the thin layer of connective tissue covering the ligament, known as the epiligament, is essential for its nutrition and normal function, as well as its healing after injury. The aim of the present study was to investigate and compare the changes in the epiligament of the medial collateral ligament which occurred during operative and non-operative treatment throughout the first month after injury.

Introduction A reoperation for a cubital tunnel syndrome is not uncommon. Patients often complain of sensorimotor symptoms in the ulnar nerve distribution after their primary surgery. The documented etiologies for such a phenomenon include a "new" kinking of the distal ulnar nerve and a "new" compression of the ulnar nerve by the fascial septum in between or tendinous bands over the muscles of the forearm.

Objective: Spinal arteriovenous malformations (AVM) manifest in the pediatric population very differently from the ones in adulthood. Despite that fact, the treatment strategy is quite the same, which provokes a question - whether some of the therapies have an advantage and if so, in which age group.

Methods: For this reason, the authors searched the world literature between 1989 and 2018 for spinal AVM in children and further categorized the studies into two age groups: < 12 and ≥12.

Coracobrachialis Longus Muscle: Humeroepitrochlearis.

Cureus

May 2018

Department of Anatomy, Histology and Embryology, Medical University of Sofia, Bulgaria, University Hospital Queen Giovanna, Sofia, BGR.

During a routine anatomical dissection of the right brachium of a 68-year-old male cadaver, an extremely rare variation of the coracobrachialis longus muscle was discovered. It started from the medial surface of the middle part of the humerus with a well-formed muscle portion and then continued into the well-presented distal tendinous portion, which was attached to the medial epicondyle of the humerus. We also briefly review the reported variations of the coracobrachialis and their potential clinical significance.

Brachial Artery Coiling: Report of a Rare Case.

Nowadays, the transradial approach is increasingly used for performing percutaneous coronary interventions and is preferred over the transfemoral approach. In the latest European Society of Cardiology guidelines (2017) for management of acute myocardial infarction in patients presenting with ST-segment elevation, the transradial approach is recommended over the transfemoral one if performed by an experienced radial operator (MATRIX study). Transradial procedure failures may be related to puncture failure, artery spasm, or to anatomical variations that require specific catheter handling or changing with a contralateral or transfemoral approach.

Purpose: Many randomised controlled trials and meta-analysis studies have presented the efficacy of tranexamic acid (TXA) without an increase of complications. However, questions still remain about the type of administration, optimal dose and secondary outcomes of TXA in total hip arthroplasty and total knee arthroplasty. The aim of this review is to summarise the existing information in literature concerning the pharmacological characteristics of TXA, forms, doses, types of application and contraindications for its use.

Dermatofibrosarcoma protuberans (DFSP) has been described as a rare, locally invasive, malignant fibroblastic tumor with a high rate of recurrence that usually affects middle-aged patients. Herein, we describe a rare case of a pedunculated dermatofibrosarcoma protuberans of the right thumb in an 80-year-old woman treated with excision through the base of the pedicle. We also make a brief literature review concerning this tumor.

Purpose: The extensor indicis proprius (EIP) is a muscle of the forearm that originates from the posterior surface of the ulna and the adjacent interosseous membrane and attaches to the index finger. Many anatomical variations of this muscle have been reported in the literature. The extensor digitorum brevis manus (EDBM) is a variant muscle found on the back of the wrist and hand.

An Active Giant Cell Tumor of the Patella: A Case Report.

Cureus

September 2017

Department of Orthopaedics and Traumatology, Medical University of Sofia, Bulgaria, University Hospital of Orthopaedics - Sofia.

Primary neoplasms of the patella account for less than 1% of all primary bone tumors of the lower extremity, the most frequent of them being the giant cell tumor of bone, the chondroblastoma, and the aneurysmal bone cyst. Herein, we report the case of a 29-year-old woman with an active giant cell tumor of the patella (GCTP) with its clinical and radiological features and a brief review of the literature.

During a routine anatomical dissection in the right brachium of a 75-year-old male cadaver, a novel variation of the coracobrachialis muscle (CB) was discovered. It consisted of three parts: proximal, starting from the upper border of the scapula; medial, presenting the well-accessed CB; and distal, formed by a proximal tendinous part that arose from the coracoid process and then transitioned into a well-defined muscle body and a distal tendinous part attached to the medial epicondyle. We also present a new systematic classification of CB variations, dividing them into two simple groups.

Osteochondromas, occurring usually in the metaphyses of long bones, are among the most frequent benign musculoskeletal neoplasms and both their sporadic and hereditary variants have been studied extensively. Extraskeletal osteochondromas, however, are much less common. They have been shown to arise near joints or synovial spaces of feet, hands, or bursae.

Aim: To examine the normal morphology of the epiligament tissue of the knee medial collateral ligament (MCL) in humans.

Methods: Several samples of the mid-substance of the MCL of the knee joint from 7 fresh human cadavers (3 females and 4 males) were taken. Examination of the epiligament tissue was conducted by light microscopy and photomicrography on semi-thin sections of formalin fixed paraffin-embedded blocks that were routinely stained with haematoxylin and eosin, Mallory stain and Van Gieson's stain.
