Background: Results from the phase 3 KEYNOTE-177 study established pembrolizumab as a new first-line standard of care for microsatellite instability-high or mismatch repair-deficient (MSI-H/dMMR) metastatic colorectal cancer (mCRC). Previous results from KEYNOTE-177 showed a statistically significant and clinically meaningful improvement in progression-free survival (PFS) with pembrolizumab versus chemotherapy ± bevacizumab/cetuximab in MSI-H/dMMR mCRC. Results after >5 years of follow-up are reported.

Article Synopsis
  • The study investigates two surgical approaches (no-touch vs. superior mesenteric artery) in patients undergoing pancreatoduodenectomy for pancreatic ductal adenocarcinoma, focusing on circulating tumor cells during surgery.
  • *Patients were randomly assigned to each group, and samples were taken to measure tumor cell activity at different surgical stages.
  • *Results showed no significant differences in tumor cell mobilization or metastasis-free survival between the two approaches, but patients with tumor cell clusters mobilized during surgery had a higher rate of distant metastases within the first year.

Article Synopsis
  • The case involves a newborn diagnosed with tracheal agenesis, a rare condition usually found postmortem, emphasizing the importance of teamwork in early diagnosis through collaborative efforts among medical professionals.
  • The infant faced severe respiratory issues at birth, where typical intubation methods failed, leading to an urgent fiberoptic procedure that confirmed the diagnosis and enabled temporary ventilation.
  • Despite the initiation of complex treatment, including extracorporeal membrane oxygenation, the baby suffered a prolonged cardiorespiratory arrest and passed away shortly after birth, highlighting the critical role of multidisciplinary management in managing such medical emergencies.

Arthroscopic anatomic lateral ligament reconstruction of the ankle joint has proven to be a safe option in the treatment of chronic ankle instability (CAI), with good functional results as well as allowing simultaneous management of associated lesions. We described an arthroscopic technique for anatomic reconstruction of the anterior talofibular ligament and calcaneofibular ligament using only 2 arthroscopic portals. This surgical technique to treat CAI is technically less demanding than other described techniques that use 3 or 4 arthroscopic portals.

Acquired von Willebrand syndrome (AvWS) is a rare bleeding disorder caused by dysfunction of the von Willebrand factor (vWF), leading to bleeding manifestations. It usually occurs due to an underlying disorder in patients with no family or personal history of bleeding diathesis. The exact mechanism causing this syndrome is not fully understood, but it involves a complex interplay of factors.

Background And Aims: Despite enteral nutrition (EN) is the preferred route of nutrition in patients with critical illness, EN is not always able to provide optimal nutrient provision and parenteral nutrition (PN) is needed. This is strongly associated with gastrointestinal (GI) complications, a feature of gastrointestinal dysfunction and disease severity. The aim of the present study was to investigate factors associated with the need of PN after start of EN, together with the use and complications associated with EN.

In this observational and multicenter study, that included all patients who underwent a heart transplantation (HT) in Spain from 1984 to 2018, we analyzed the incidence, management, and prognosis of colorectal cancer (CRC) after HT. Of 6,244 patients with a HT and a median follow-up of 8.8 years since the procedure, 116 CRC cases (11.

Aims: Limited data exist on low-density lipoprotein-cholesterol (LDL-C) level variability or long-term persistence with the monoclonal antibody evolocumab in routine clinical practice. HEYMANS (NCT02770131) is the first multi-country, multicenter, observational study of European patients initiating evolocumab as part of their routine clinical management, based on local reimbursement criteria (overall data recently published). The aim of this analysis is to describe clinical characteristics, baseline and changes in LDL-C levels, treatment patterns and persistence to evolocumab over 30 months in the Spanish cohort using data from the HEYMANS Registry.

Article Synopsis
  • CDK7 is essential for the growth of multiple myeloma (MM) cells, as it regulates critical programs like E2F and MYC, which are linked to cell survival and metabolism.
  • Targeting CDK7 with a specific inhibitor (YKL-5-124) not only disrupts these oncogenic pathways but also shows strong anti-tumor effects with minimal harm to normal cells, leading to reduced glycolysis and lactate production in MM cells.
  • The promising results from mouse models highlight CDK7 as a key player in MM progression and suggest that YKL-5-124 could be an effective treatment option worth pursuing in clinical settings.

Background: The integration of immune checkpoint inhibitors (ICI) for the treatment of melanoma has resulted in remarkable and durable responses. Given the potential role of immunosenescence, age may contribute to differential ICI efficacy and toxicity. While older patients have been studied in detail, outcomes from ICI in young patients (≤40 years) are not well characterised.

Introduction: The high burden of respiratory syncytial virus (RSV) infection in young children disproportionately occurs in low- and middle-income countries (LMICs). The PROUD (Preventing RespiratOry syncytial virUs in unDerdeveloped countries) Taskforce of 24 RSV worldwide experts assessed key needs for RSV prevention in LMICs, including vaccine and newer preventive measures.

Methods: A global, survey-based study was undertaken in 2021.

  • Common Variable Immunodeficiency (CVID) is a diverse group of primary immunodeficiency disorders that presents a wide range of clinical challenges, including significant non-infectious health issues among patients.
  • A nationwide study in Spain over three years examined 250 CVID patients, revealing a diagnostic delay of about 10 years, with infectious complications initially predominant, though non-infectious lung diseases and immune disorders were more common overall.
  • The study highlighted a high prevalence of associated conditions, such as benign lymphoproliferation and cancers, indicating a need for further research to develop better treatment strategies and improve the quality of life for those affected by CVID.

Article Synopsis
  • The VALIGA study analyzed the use of the Oxford Classification for immunoglobulin A nephropathy (IgAN) in 1147 patients across 13 European countries, focusing on biopsy scoring and its impact on treatment decisions.
  • The findings revealed that local pathologists tended to report more lesions than central pathologists, and variability in scoring was noted, particularly for mesangial hypercellularity and endocapillary hypercellularity.
  • Ultimately, the study highlighted discrepancies in pathology assessments affecting treatment choices and clinical outcomes, suggesting that central reviews could offer more reliable evaluations for certain lesions.

The introduction of Rituximab has improved the outcome and survival rates of Burkitt lymphoma (BL). However, early relapse and refractoriness are current limitations of BL treatment and new biological factors affecting the outcome of these patients have not been explored. This study aimed to identify the presence of genomic changes that could predict the response to new therapies in BL.

Transforming growth factor-β (TGF-β) signaling is associated with tumor progression and vascularization in malignant glioma. In the present study, magnetic resonance imaging was used to evaluate changes in the size and vascularity of glioblastomas in 12 patients who were treated with lomustine and the novel inhibitor of TGF-β signaling, LY2157299 monohydrate. A response in tumor size was observed in 2 of the 12 patients; in 1 of these 2 patients, a reduction in vascular permeability and perfusion was also detected.
