Article Synopsis
  • Drains are often used to manage abdominal fluid collections after surgical procedures, but their necessity, especially after cholecystectomy, is debated.
  • This study analyzed data from over 6,000 laparoscopic cholecystectomies to assess the outcomes and indications for using sub-hepatic drains in patients, particularly focusing on those undergoing emergency surgeries.
  • Results indicated that while drains were frequently used, complications were minimal, and many post-operative issues resolved spontaneously, suggesting a selective approach to drainage might be beneficial.

Introduction: There is evidence that sex differences exist in stroke presentation, risk factors, severity, treatment, and outcomes. To further understand this, we explored how sex differences influence acute stroke management, secondary prevention prescribing, and mortality outcomes in a well-characterised cohort of first-ever stroke patients in Scotland.

Methods: This is a retrospective, population-based, data-linkage study of stroke admissions to acute care hospitals in Scotland between January 1, 2011, and December 31, 2018.

Article Synopsis
  • The study examines how previous abdominal surgeries impact the difficulty and complications associated with laparoscopic cholecystectomy (LC), specifically looking at different surgical histories and scar locations.
  • Data from 5916 patients over 30 years revealed that 31.2% had prior abdominal surgery, leading to increased rates of adhesiolysis, longer operation times, and higher risks of bowel injuries compared to those without such history.
  • The findings suggest that while laparoscopic procedures can be more challenging in patients with previous surgeries, modified access techniques can mitigate risks, showing no bowel complications in patients using specific access methods.

Background: Anti-inflammatory therapy with long-term colchicine prevented vascular recurrence in coronary disease. Unlike coronary disease, which is typically caused by atherosclerosis, ischaemic stroke is caused by diverse mechanisms including atherosclerosis and small vessel disease or is frequently due to an unknown cause. We aimed to investigate the hypothesis that long-term colchicine would reduce recurrent events after ischaemic stroke.

Background And Aims: What is the relationship between blood tests for iron deficiency, including anaemia, and the response to intravenous iron in patients with heart failure?

Methods: In the IRONMAN trial, 1137 patients with heart failure, ejection fraction ≤ 45%, and either serum ferritin < 100 µg/L or transferrin saturation (TSAT) < 20% were randomized to intravenous ferric derisomaltose (FDI) or usual care. Relationships were investigated between baseline anaemia severity, ferritin and TSAT, to changes in haemoglobin from baseline to 4 months, Minnesota Living with Heart Failure (MLwHF) score and 6-minute walk distance achieved at 4 months, and clinical events, including heart failure hospitalization (recurrent) or cardiovascular death.

Results: The rise in haemoglobin after administering FDI, adjusted for usual care, was greater for lower baseline TSAT (Pinteraction < .

Allopurinol and blood pressure variability following ischemic stroke and transient ischemic attack: a secondary analysis of XILO-FIST.

J Hum Hypertens

April 2024

School of Cardiovascular and Metabolic Health, College of Medical, Veterinary & Life Sciences, University of Glasgow, Queen Elizabeth University Hospital, Glasgow, G51 4TF, UK.

Blood Pressure Variability (BPV) is associated with cardiovascular risk and serum uric acid level. We investigated whether BPV was lowered by allopurinol and whether it was related to neuroimaging markers of cerebral small vessel disease (CSVD) and cognition. We used data from a randomised, double-blind, placebo-controlled trial of two years allopurinol treatment after recent ischemic stroke or transient ischemic attack.

Molecular landscape and functional characterization of centrosome amplification in ovarian cancer.

Nat Commun

October 2023

Cancer Research UK Cambridge Institute, University of Cambridge, Li Ka Shing Centre, Robinson Way, Cambridge, CB2 0RE, UK.

Article Synopsis
  • High-grade serous ovarian carcinoma (HGSOC) has a poor prognosis and is linked to significant chromosome instability (CIN), making it a challenging cancer to treat.
  • This study examined 287 HGSOC tissue samples and 73 cell line models, revealing that centrosome amplification (CA) through centriole overduplication is a common and variable characteristic of HGSOC.
  • High levels of CA in ovarian cancer cell lines correlate with increased resistance to treatments, especially paclitaxel, suggesting that CA may drive tumor evolution and serve as a valuable biomarker for treatment response.

During the second wave of COVID-19 cases within Scotland, local evidence suggested that a large number of interhospital transfers occurred due to both physical capacity and staff shortages. Although there are inherent risks with transferring critically ill patients between hospitals, there are signals in the literature that mortality is not affected in COVID-19 patients when transferred between intensive care units. With a lack of evidence in the Scottish population, and as the greatest source of capacity transfers in our critical care network at that time, we sought to determine whether these transfers impacted on survival to hospital discharge.
