Background: Differentiated thyroid carcinoma (DTC) is characterized by an excellent prognosis with a 10-year survival rate > 90%. However, when DTC develops in a metastatic form, it has been shown to significantly impact patient survival and quality of life. Although I-131 has been shown to be an effective therapy in patients with metastatic DTC, whether its efficacy after recombinant human TSH (rhTSH) is comparable to endogenous TSH stimulation by thyroid hormone deprivation (THW) is still debated.

Background: Since its outbreak in early 2020, the COVID-19 pandemic has diverted resources from non-urgent and elective procedures, leading to diagnosis and treatment delays, with an increased number of neoplasms at advanced stages worldwide. The aims of this study were to quantify the reduction in surgical activity for indeterminate thyroid nodules during the COVID-19 pandemic; and to evaluate whether delays in surgery led to an increased occurrence of aggressive tumours.

Methods: In this retrospective, international, cross-sectional study, centres were invited to participate in June 22, 2022; each centre joining the study was asked to provide data from medical records on all surgical thyroidectomies consecutively performed from Jan 1, 2019, to Dec 31, 2021.

Purpose: An accurate postoperative assessment is pivotal to inform postoperative I treatment in patients with differentiated thyroid cancer (DTC). We developed a predictive model for post-treatment whole-body scintigraphy (PT-WBS) results (as a proxy for persistent disease) by adopting a decision tree model.

Methods: Age, sex, histology, T stage, N stage, risk classes, remnant estimation, TSH, and Tg were identified as potential predictors and were put into regression algorithm (conditional inference tree, ctree) to develop a risk stratification model for predicting the presence of metastases in PT-WBS.

SPINAL ANESTHESIA IN DAY SURGERY - EARLY EXPERIENCES.

Acta Clin Croat

September 2022

Akromion, Special Hosiptal for Orthopaedic Surgery and Traumatology, Krapinske Toplice, Croatia.

Day surgery is a term that refers to performing an operation under anesthesia without needing to stay in the hospital for more than 24 hours. Day surgery has many advantages over surgery that involves a multiple-day hospital stay. Anesthesia for day surgery must have a rapid onset of action, a rapid cessation of action, and be free of, or have minimal side effects.

Bone involvement in primary hyperparathyroidism (PHPT) is characterized by reduced bone mineral density (BMD) using dual X-ray absorptiometry (DXA). A hallmark of PHPT is BMD loss at cortical sites while trabecular bone remains relatively preserved. PHPT is associated with increased fracture risk at both trabecular and cortical skeletal sites, which cannot be explained based on BMD values alone.

Analysis of Prognostic Factors Impacting Pediatric Acute Mastoiditis Outcomes.

J Int Adv Otol

January 2023

Bruce Rappaport Faculty of Medicine, Technion-Israel Institute of Technology, Haifa; Department of Otorhinolaryngology, Hillel Yaffe Medical Center, Hadera, Israel.

Article Synopsis
  • The study focused on pediatric patients with acute mastoiditis, analyzing how intravenous steroid therapy, patient demographics, and inflammatory markers affect outcomes.
  • It involved 73 patients treated over a 10-year period, revealing that those who needed surgery had significantly higher C-reactive protein levels at admission.
  • The findings suggest that steroid therapy can reduce hospital stays, males may experience more severe symptoms, and C-reactive protein levels are reliable indicators for surgical intervention.

Objective: Current staging and risk-stratification systems for predicting survival or recurrence of patients with differentiated thyroid carcinoma may be ineffective at predicting outcomes in individual patients. In recent years, nomograms have been proposed as an alternative to conventional systems for predicting personalized clinical outcomes. We conducted a systematic review to evaluate the predictive performance of available nomograms for thyroid cancer patients.
