Background And Objective: The role of prostate-specific membrane antigen (PSMA)-based positron emission tomography (PET)/computed tomography (CT) in addition to magnetic resonance imaging (MRI) for local staging of prostate cancer (PC) has been poorly addressed so far. Our aim was to assess the diagnostic accuracy of PSMA PET/CT and MRI, alone and combined, for detection of extraprostatic extension (EPE) and seminal vesicle invasion (SVI) in PC.

Methods: We conducted a multicenter retrospective study evaluating patients undergoing PSMA PET/CT and MRI before radical prostatectomy.

Article Synopsis
  • The study examines how daily biventricular pacing percentage (BiV%) affects heart failure (HF) status in patients with cardiac resynchronization therapy (CRT) using the HeartLogic algorithm.
  • Researchers monitored 306 patients over 26 months and found that low BiV% and prolonged drops in pacing were linked to a higher risk of heart failure alerts, indicating worsening conditions.
  • The findings suggest that even small decreases in BiV% can significantly indicate potential heart failure problems, emphasizing the importance of maintaining high pacing levels for patient outcomes.

March 2024

Dipartimento di Diagnostica per Immagini, Radioterapia Oncologica ed Ematologia, Fondazione Policlinico Universitario "A. Gemelli" IRCCS, Roma; Sezione di Ematologia, Dipartimento di Scienze Radiologiche ed Ematologiche, Università Cattolica del Sacro Cuore.

Autologous peripheral blood stem cell transplantation (aPBSCT) provides optimal outcomes in POEMS syndrome but the definition of the best treatment before aPBSCT remains to be defined because of the rarity of the disease and the heterogeneity of published case series. We collected clinical and laboratory data of patients with POEMS syndrome undergoing aPBSCT from 1998 to 2020 in ten Italian centers. The primary endpoint of the study was to evaluate the impact of prior therapies and mobilization regimen on outcome.

Background: Although the therapeutic role of extended pelvic lymph node dissection (ePLND) in patients with prostate cancer (PCa) is still under debate, this procedure is recommended for staging purposes in selected cases. Nomograms for predicting lymph node invasion (LNI) do not account for prostate-specific membrane antigen (PSMA) positron emission tomography (PET) imaging, which is characterized by a high negative predictive value for nodal metastases.

Objective: To externally validate models predicting LNI in patients with miN0M0 PCa at PSMA PET and to develop a novel tool in this setting.

Introduction: The prediction of ventricular tachyarrhythmias among patients with implantable cardioverter defibrillators is difficult with available clinical tools. We sought to assess whether in patients with heart failure (HF) and reduced ejection fraction with defibrillators, physiological sensor-based HF status, as summarized by the HeartLogic index, could predict appropriate device therapies.

Methods: Five hundred and sixty-eight consecutive HF patients with defibrillators (n = 158, 28%) or cardiac resynchronization therapy-defibrillators (n = 410, 72%) were included in this prospective observational multicenter analysis.

Article Synopsis
  • - The HeartLogic algorithm, developed by Boston Scientific, effectively predicts imminent heart failure (HF) by using various heart metrics from implanted devices, and this study aimed to see if it can also flag patients at high risk for death.
  • - In a study involving 568 patients with implantable cardioverter-defibrillators, 1,200 alerts were logged over about 26 months, indicating increased health risks; notably, the death rate was much higher during alert states compared to non-alert states.
  • - The findings suggest that patients in an alert state have a significantly increased risk of death, indicating that the HeartLogic index is a valuable tool for identifying those who may need more intensive monitoring or intervention.

Cardiology Division, Department of Biomedical, Metabolic and Neural Sciences, University of Modena and Reggio Emilia, Policlinico di Modena, Via del Pozzo, 71, 41125 Modena (MO), Italy.

Article Synopsis
  • The study investigates the link between heart failure (HF), sleep apnea (SA), and the incidence of atrial high-rate events (AHRE) in patients with implantable defibrillators (ICDs).
  • Researchers monitored 411 patients over 26 months, finding that severe SA was present in 58% of cases and a significant percentage experienced varying levels of AHRE.
  • The findings suggest that while the combination of IN-alert HF state and severe SA is rare, when it does occur, it leads to a notably high rate of AHRE events.

  • A study on 34 adult lymphoma patients undergoing their first chemotherapy (CHT) treatment revealed that most experienced significant telomere shortening during and after their treatment.
  • On average, patients lost about 673 base pairs (bp) of telomeres by the sixth cycle, with the most significant loss occurring by the third cycle.
  • The results indicated that baseline telomere length and gender are key factors influencing how telomeres respond to chemotherapy, with younger women showing less significant telomere loss compared to other groups.

Surgical Metastasectomy for Visceral and Bone Prostate Cancer Metastases: A Mini-Review.

Eur Urol Focus

March 2023

Unit of Urology/Division of Oncology, Gianfranco Soldera Prostate Cancer Laboratory, IRCCS San Raffaele Scientific Institute, Milan, Italy; Vita-Salute San Raffaele University, Milan, Italy. Electronic address:

Despite growing interest in metastasis-directed therapy (MDT) for prostate cancer (PCa), little is known regarding the feasibility and effectiveness of surgical metastasectomy for isolated lesions. We performed a narrative review of the available evidence supporting metastasectomy for M1b-c lesions in men diagnosed with oligometastatic or oligorecurrent PCa. The case series and case reports we identified indicate that surgical MDT is a safe and feasible treatment option for well-selected patients with a small number of PCa metastases diagnosed via molecular imaging.

Nutritional Support in Cancer patients: update of the Italian Intersociety Working Group practical recommendations.

J Cancer

May 2022

Medical Oncology Unit, Fondazione IRCCS Policlinico San Matteo and Department of Internal Medicine, University of Pavia, Pavia, Italy.

Malnutrition is a frequent problem in cancer patients, which leads to prolonged and repeated hospitalizations, increased treatment-related toxicity, reduced response to cancer treatment, impaired quality of life, a worse overall prognosis and the avoidable waste of health care resources. Despite being perceived as a limiting factor in oncologic treatments by both oncologists and patients, there is still a considerable gap between need and actual delivery of nutrition care, and attitudes still vary considerably among health care professionals. In the last 5 years, the Italian Intersociety Working Group for Nutritional Support in Cancer Patients (WG), has repeatedly revisited this issue and has concluded that some improvement in nutritional care in Italy has occurred, at least with regard to awareness and institutional activities.
