Article Synopsis
  • Patients and physicians often disagree on the severity of diseases, known as discordant severity grading (DSG), which affects their relationship and can cause frustration.
  • A study aimed to identify factors contributing to DSG through a qualitative phase to develop a model followed by a quantitative phase that validated this model using data from dermatology patients and their physicians in Singapore.
  • The research involved 1,053 patients with psoriasis or eczema and analyzed differences in severity ratings, finding that positive discordance (patients rating their severity significantly higher than physicians) and negative discordance (patients rating it significantly lower) were influenced by various cognitive, behavioral, and disease-specific factors.

Importance: Selecting the optimal antiplatelet agent in patients who have received percutaneous coronary intervention is especially important in those with diabetes due to the heightened risk of ischemic events in this population. Studies on the efficacy and safety of clopidogrel vs aspirin for long-term maintenance after percutaneous coronary intervention in patients with diabetes are lacking.

Objective: To investigate cardiovascular outcomes with clopidogrel vs aspirin in patients with and without diabetes.

Background: Clonal hematopoiesis of indeterminate potential (CHIP) increases the risk of cerebrovascular events, while its association with cerebral white matter hyperintensity (WMH) is undemonstrated. We evaluated the effect of CHIP and its major driving mutations on cerebral WMH severity.

Methods: From an institutional cohort of a routine health check-up program with a DNA repository database, subjects who were ≥50 years of age, with one or more cardiovascular risk factors but no central nervous system disorder, and performed brain MRI were included.

Background: The prevalence of nonalcoholic fatty liver disease (NAFLD) is increasing in parallel with the epidemic of obesity and metabolic syndrome. Lifestyle modification is a crucial strategy for the treatment of NAFLD, which can lead to a reduction in liver fat with concomitant weight loss. The use of eHealth technologies is an effective approach to improve health outcomes in patients as they do not have any time and space limitations.

Rationale: Alteplase is the only approved thrombolytic agent for acute stroke. An alternative plasminogen activator, tenecteplase, has been previously shown to increase early biological effectiveness (reperfusion) resulting in early clinical recovery in acute stroke patients with target mismatch on perfusion imaging; however, phase III data are lacking.

Aim And Hypothesis: In this study, we assess the efficacy and safety of tenecteplase compared to alteplase in acute stroke patients with target mismatch on perfusion imaging.

BackgroundPharmacy training programs infrequently include formal training in the areas of diversity, equity, and inclusion (DEI). Hence, the purpose of this report is to offer perspectives gained from the delivery of a DEI curriculum within a pharmacy residency program aimed at expanding experiential learning focused on DEI and health equity. Pharmacy residents at an academic medical center were invited to participate in a longitudinal DEI/equity seminar series that was thoughtfully and strategically developed by a team of residents and program leadership based on a six-step process.

Article Synopsis
  • - This study aimed to find risk factors for needing permanent clean intermittent catheterization (CIC) and urinary incontinence in patients with lipomyelomeningocele (LMMC), as well as to assess how LMMC impacts bladder function over time using a urodynamic score.
  • - Researchers analyzed health records of 120 patients who had neurosurgical repairs for LMMC between January 2012 and December 2016, tracking bladder function at various points before and after surgery.
  • - Key findings suggest that factors such as age at surgery and the type of LMMC are significant risks for needing permanent CIC; also, urinary retention and high urodynamic scores three months post-surgery are linked to ongoing incontinence issues.

The (After)life of a Trial: Biocommunicability of an At-Risk Pregnancy.

Med Anthropol

November 2022

Department of Obstetrics and Gynecology, NorthShore University Healthcare System, Evanston, Illinois, USA.

The publication of A Randomized Trial of Induction Versus Expectant Management (ARRIVE), conducted in the United States in 2018, heralded a paradigm shift within the obstetrical management of term pregnancy among people who have not previously given birth. ARRIVE finds its home among other canonical - and controversial - randomized controlled trials (RCTs) within obstetrics. We argue that RCTs have their own (after)life, both creating new subjects for biomedical intervention and recalibrating who reproductive health practitioners consider to be at risk of adverse health outcomes.

Purpose Of Review: In this review we highlight the most recent studies furthering the clinical development of selinexor, a novel exportin-1 inhibitor, for the treatment of multiple myeloma and non-Hodgkin lymphomas.

Recent Findings: Three pivotal trials, the SADAL trial for diffuse large B-cell lymphoma, and the BOSTON and selinexor treatment of refractory myeloma trials for multiple myeloma, have recently led to the regulatory approval of selinexor monotherapy or combination regimens. They are complemented by several earlier phase clinical trials with iterative combinations, adding selinexor to novel therapies and cytotoxic chemotherapy regimens at various stages in the disease courses.

Objective/background: When isolated rapid eye movement (REM) sleep behavior disorder (iRBD) is combined with obstructive sleep apnea (OSA), the pattern of temporal association between REM without atonia (RWA) and apnea-hypopnea events may be related to improvements in RBD symptoms after positive airway pressure (PAP) treatment. We evaluated the frequency of improvement in RBD symptoms after PAP and the relationship of the degree of co-occurrence between RWA and apnea-hypopnea events to RBD symptom improvement after PAP treatment.

Patients/methods: In an institutional cohort with sleep disorders, 31 patients with video-polysomnography confirmed iRBD and concomitant OSA with an apnea-hypopnea index (AHI) of ≥15/h who received PAP treatment were included.
