Neural responses to facial emotions and subsequent clinical outcomes in difficult-to-treat depression.

Psychol Med

May 2024

Centre of Affective Disorders, Institute of Psychiatry, Psychology & Neuroscience, Centre for Affective Disorders, King's College London, London, UK.

Article Synopsis
  • - The study focused on the amygdala and dorsal anterior cingulate cortex's role in predicting treatment responses in difficult-to-treat major depressive disorder (MDD) patients, who had not responded to at least two antidepressants.
  • - Using fMRI, researchers found that lower activation in the amygdala when responding to sad and happy faces predicted better clinical outcomes in these MDD patients, while the anterior cingulate cortex showed no significant effects.
  • - The results suggest that a decreased amygdala response to emotional expressions may be a helpful indicator of symptom improvement in MDD, with heightened anxiety levels diminishing this adaptive response.

Objectives: Lifetime DSM-5 diagnoses generated by the lay-administered Composite International Diagnostic Interview for DSM-5 (CIDI) in the World Mental Health Qatar (WMHQ) study were compared to diagnoses based on blinded clinician-administered reappraisal interviews.

Methods: Telephone follow-up interviews used the non-patient edition of the Structured Clinician Interview for DSM-5 (SCID) oversampling respondents who screened positive for five diagnoses in the CIDI: major depressive episode, mania/hypomania, panic disorder, generalized anxiety disorder, and obsessive-compulsive disorder. Concordance was also examined for a diagnoses of post-traumatic stress disorder based on a short-form versus full version of the PTSD Checklist for DSM-5 (PCL-5).

LGI1-antibody encephalitis: how to approach this highly treatable dementia mimic in memory and mental health services.

Br J Psychiatry

June 2024

University Department of Psychiatry, University of Oxford, UK; and Department of Psychological Medicine, John Radcliffe Hospital, Oxford University Hospital NHS Foundation Trust, Oxford, UK.

Leucine-rich glioma-inactivated 1-antibody-encephalitis is a treatable and potentially reversible cause of cognitive and psychiatric presentations, and may mimic cognitive decline, rapidly progressive dementia and complex psychosis in older patients. This aetiology is of immediate relevance given the alternative treatment pathway required, compared with other conditions presenting with cognitive deficits.

Systematic culture of central catheters and infections related to catheters in a neonatal intensive care unit: an observational study.

Sci Rep

April 2024

Department of Neonatal Paediatrics, APHP, Service de Néonatologie, Sorbonne Université, Hôpital Armand Trousseau, 26 Av. du Dr Arnold Netter, 75012, Paris, France.

Article Synopsis
  • Systematic cultures of central line tips in NICUs may not be as clinically useful due to high bacterial contamination rates during removal, leading to debate on their necessity.
  • A retrospective study analyzed infections and bacterial colonization in 375 newborns with central catheters, finding a 43% colonization rate and 31 confirmed catheter-related infections among suspected sepsis cases.
  • The study suggests that only culturing catheters when infections are suspected could be a more efficient approach than routine screening, as antibiotic effectiveness did not significantly differ between expert recommendations and actual prescriptions.

Introduction: Expert consensus operationalized treatment response and remission in obsessive-compulsive disorder (OCD) as a Yale-Brown Obsessive-Compulsive Scale (Y-BOCS) reduction ≥35% and score ≤12 with ≤2 on Clinical Global Impressions Improvement (CGI-I) and Severity (CGI-S) scales, respectively. However, there has been scant empirical evidence supporting these definitions.

Methods: We conducted a systematic review and an individual participant data meta-analysis of randomized-controlled trials (RCTs) in adults with OCD to determine optimal Y-BOCS thresholds for response and remission.

Background: Anxiety disorders are highly prevalent and socio-economically costly. Novel pharmacological treatments for these disorders are needed because many patients do not respond to current agents or experience unwanted side effects. However, a barrier to treatment development is the variable and large placebo response rate seen in trials of novel anxiolytics.

Daily routines, including in-person school and extracurricular activities, are important for maintaining healthy physical activity and sleep habits in children. The COVID-19 pandemic significantly disrupted daily routines as in-person school and activities closed to prevent spread of SARS-CoV-2. We aimed to examine and assess differences in objectively measured physical activity levels and sleep patterns from wearable sensors in children with obesity before, during, and after a period of school and extracurricular activity closures associated with the COVID-19 pandemic.

Background: Determining when to recommend a change in treatment regimen due to insufficient improvement is a common challenge in therapeutics.

Methods: In a sample of 7215 patients with major depressive disorder treated with transcranial magnetic stimulation (TMS) and with PHQ-9 scores before, during and after the course, 3 groups were identified based on number of acute course sessions: exactly 36 sessions (N = 3591), more than 36 sessions (N = 975), and less than 36 sessions (N = 2649). Two techniques were used to determine thresholds for percentage change in PHQ-9 scores at assessments after 10, 20, and 30 sessions that optimized prediction of endpoint response status: the Youden index and fixing the false positive rate at 10%.

Article Synopsis
  • * The study analyzed data from over 22,000 pregnant participants and found that those living in food insecure areas tended to have lower birth weights and higher chances of having small-for-gestational-age babies.
  • * Individual food insecurity did not show a significant association with birth outcomes, suggesting that neighborhood food access may be a more critical factor during pregnancy.

Advocacy and Policy: A Focus on Migrant Youth.

Child Adolesc Psychiatr Clin N Am

April 2024

Boston Children's Hospital, 1 Brookline Place, Suite 552, Boston, MA 02445, USA.

Mental health challenges experienced by migrant children are shaped by multiple factors within the health care system and society at large. It is essential for health care providers to recognize the profound impact of these influences on child well-being. By actively engaging in advocacy and policy initiatives, health care providers can address structural barriers, social inequalities, and stigma that perpetuate mental health disparities.

Lymph node (LN) fine needle aspiration (LN FNA) represents a powerful technique for minimally invasive sampling of human LNs in vivo and has been used effectively to directly study aspects of the human germinal center response. However, systematic deep phenotyping of the cellular populations and cell-free proteins recovered by LN FNA has not been performed. Thus, we studied human cervical LN FNAs as a proof-of-concept and used single-cell RNA-sequencing and proteomic analysis to benchmark this compartment, define the purity of LN FNA material, and facilitate future studies in this immunologically pivotal environment.

Article Synopsis
  • This narrative review examines goal setting and goal attainment scaling as methods to implement shared decision making (SDM) in treating major depressive disorder (MDD).
  • The review highlights that, despite the recommendation of SDM for mental health treatment, its application has been inconsistent, and goal setting is still a developing concept in MDD care with limited studies evaluating its effectiveness.
  • The authors argue that goal setting can enhance patient engagement, motivation, and recovery while supporting self-determination and personal growth, indicating that goal attainment scaling can serve as an effective personalized outcome measure in clinical trials for MDD.

In 2022, the Australian Federal Minister for Health and Aged Care commissioned the Medicare Benefits Schedule (MBS) Review Advisory Committee (MRAC) to conduct a post-implementation review of MBS telehealth services, including settings of video and telephone consultations. The MRAC has made a series of administrative recommendations for telehealth practice that appear at cross-purposes to the evidence-base on medical consultations and that would limit patient access to medical specialist assessment in Australia. These recommendations particularly underestimate the role of telehealth in rural and remote Australia and did not take into account high patient satisfaction with telehealth assessment and treatment during the ongoing coronavirus disease 2019 (COVID-19) pandemic.

Article Synopsis
  • The prevalence of substance use disorders (SUDs) is increasing among older adults, particularly in rural areas where they face challenges accessing care; these individuals often have other chronic health conditions that lead to higher emergency department (ED) visits.
  • Nurse practitioners (NPs) are becoming essential in providing primary care to this population, yet factors like lack of support and autonomy in their work environment may limit their effectiveness.
  • A study found that greater support for NPs’ independent practice in rural settings is linked to significantly fewer ED visits among older adults with SUDs, highlighting the need for practice administrators to enhance organizational support for NPs.

Background: Approximately 1800 opioid treatment programs (OTPs) in the US dispense methadone to upwards of 400,000 patients with opioid use disorder (OUD) annually, operating under longstanding highly restrictive guidelines. OTPs were granted novel flexibilities beginning March 15, 2020, allowing for reduced visit frequency and extended take-home doses to minimize COVID exposure with great variation across states and sites. We sought to use electronic health records to compare retention in treatment, opioid use, and adverse events among patients newly entering methadone maintenance in the post-reform period in comparison with year-ago, unexposed, controls.

Electronic health records (EHRs) have been widely adopted in Australian public sector healthcare and will remain an ongoing, essential data system. However, recent substantial data breaches from hacked business data systems in Australian enterprises, as well as international healthcare providers, mean that EHR data breaches are increasingly likely in Australia. Risks include medical identity theft and extortion attempts based on threats to release sensitive patient information.

Many researchers have reported on the high prevalence of anxiety and depression during pregnancy as well as the influence on delivery outcomes during the past decade. Preterm birth and premature labor, bleeding, higher frequency of cesarean section (CS), low birth weight, preeclampsia, stillbirth, miscarriage, NICU hospitalization, and a low Apgar score are the most commonly referenced outcomes assessed. Clarifying the relationship between exposure and result may help us to understand the risk factors and guide us to future clinical and research practices.
