A metal-free dual photo- and organocatalytic decarbonylation of aliphatic aldehydes is described for the first time. On the one hand, a wide range of tertiary aldehydes are smoothly decarbonylated thanks to the combination of thioxanthone as photocatalyst and diphenyl disulfide as organocatalyst. On the other hand, by simply using 4-CzIPN as photocatalyst instead of thioxanthone, various secondary aldehydes readily undergo decarbonylation in an orthogonal manner, via the in situ formation of 1,4-dihydropyridines.

Permafrost thaw poses diverse risks to Arctic environments and livelihoods. Understanding the effects of permafrost thaw is vital for informed policymaking and adaptation efforts. Here, we present the consolidated findings of a risk analysis spanning four study regions: Longyearbyen (Svalbard, Norway), the Avannaata municipality (Greenland), the Beaufort Sea region and the Mackenzie River Delta (Canada) and the Bulunskiy District of the Sakha Republic (Russia).

Inter-Rater Disagreements in Applying the Montreal Classification for Crohn's Disease: The Five-Nations Survey Study.

Background: The Montreal classification has been widely used in Crohn's disease since 2005 to categorize patients by the age of onset (A), disease location (L), behavior (B), and upper gastrointestinal tract and perianal involvement. With evolving management paradigms in Crohn's disease, we aimed to assess the performance of gastroenterologists in applying the Montreal classification.

The insulative properties of soil organic carbon (SOC) and surface organic layers (moss, lichens, litter) regulate surface-atmosphere energy exchanges in the Arctic through a coupling with soil temperatures. However, a physical description of this process is lacking in many climate models, potentially biasing their high-latitude climate predictions. Using a coupled surface-atmosphere model, we identified a strong feedback loop between soil insulation, surface air temperature, and snowfall.

Evaluation of the Learning Curve Threshold in Robot-Assisted Lung Cancer Surgery: A Nationwide Population-Based Study.

Background: Recent publications suggest that the threshold for validation of the learning curve is 25 procedures. The aim of this study was to evaluate this threshold using another rarely used method, based on a composite quality indicator.

Methods: We included all patients from the French medico-administrative database receiving robot-assisted surgery for lung cancer, with a focus on hospitals performing at least 25 procedures over the period 2019-2022.

European consensus-based interdisciplinary guideline for melanoma. Part 2: Treatment - Update 2024.

Background And Hypothesis: Cardiovascular diseases are a leading cause of morbidity and mortality in patients with chronic kidney disease (CKD). Acute kidney injury (AKI) has been increasingly recognized as a potential exacerbating factor for cardiovascular events in these patients. The CKD-REIN study aims to explore the relationship between AKI and the risk of major adverse cardiovascular events (MACE) in a cohort of CKD patients.

Purpose: Acute postoperative endophthalmitis (APE) is a rare but severe complication of cataract surgery. During the most recent decades, the use of povidone-iodine (PVI) before surgery combined with intracameral cefuroxime has drastically reduced the incidence of endophthalmitis. The goal of this study is to describe the changes in endophthalmitis rates after cataract surgery over the past two decades in a large tertiary ophthalmology center.

[Techniques for imaging optic disc vasculature in glaucomatous optic neuropathy: A review of the literature].

The anatomy and vasculature of the optic nerve head are complex and subject to numerous variations. The main risk factor for glaucomatous optic neuropathy is elevated intraocular pressure, but many other factors have been identified. A vascular component seems to play an important role in the pathogenesis and/or progression of glaucomatous optic neuropathy, either under the influence of ocular hypertension or as an independent risk factor, particularly as in normal tension glaucoma (NTG).

[A case of post-traumatic paracentral acute middle maculopathy associated with glaucoma].

Paracentral acute middle maculopathy is a lesion of the inner nuclear layer and outer plexiform layer of the macula associated with local or systemic vascular diseases that can induce retinal hypoxia. The etiologic mechanism may be related to the increase in intraocular pressure which creates conditions of hypoperfusion in the retinal vasculature. We present a case that shows a possible association between paracentral acute middle maculopathy and glaucoma with the trauma of elevated intraocular pressure as a triggering event.

Circulating tumor cells in breast cancer: clinical validity and utility.

Circulating tumor cells (CTCs) have been extensively studied in breast cancer (BC), with large studies establishing CTCs as a robust prognostic biomarker in early and metastatic breast cancer (MBC). Several phase II and phase III trials have investigated the clinical utility of CTCs in BC. Here, we outline the current landscape for the use of CTCs in the clinic at different stages of BC, focusing first on early BC, then on MBC, with a particular focus on interventional clinical trials based on CTCs.

Which Degree for Which Occupation? Vertical and Horizontal Mismatch Among Immigrants, Their Children, and Grandchildren in France.

Prior research has found that immigrants are often overeducated: their educational attainment is higher than required or commonly observed in their occupation. Yet, less is known about the education-occupation mismatch among immigrants' children and grandchildren (the second and third generations). Using the French Trajectories and Origins 2 (TeO2, 2019-2020) survey, we test theoretically grounded hypotheses on the level of vertical (educational attainment) and horizontal (field of study) mismatch in the first, second, and third generations as well as on the mechanisms underlying the mismatch.

[Management of congenital thrombotic thrombocytopenic purpura in the era of recombinant ADAMTS13 protein: Recommendations from the Reference Center for Thrombotic Microangiopathies (CNR-MAT)].

Thrombotic Thrombocytopenic Purpura (TTP) is a rare disease characterized by a severe deficiency of ADAMTS13, the specific protease that cleaves von Willebrand factor. The congenital form of TTP (cTTP) results from pathogenic variants of the ADAMTS13 gene. cTTP has two peaks of incidence: one in childhood and the other in adulthood, mainly in an obstetric context.

An International Factorial Vignette-Based Survey of Intubation Decisions in Acute Hypoxemic Respiratory Failure.

Objectives: Intubation is a common procedure in acute hypoxemic respiratory failure (AHRF), with minimal evidence to guide decision-making. We conducted a survey of when to intubate patients with AHRF to measure the influence of clinical variables on intubation decision-making and quantify variability.

Design: Factorial vignette-based survey asking "Would you recommend intubation?" Respondents selected an ordinal recommendation from a 5-point scale ranging from "Definite no" to "Definite yes" for up to ten randomly allocated vignettes.

Background: Quality of life is decreased in bipolar disorders (BD) and contributes to poor prognosis. However, little is known about the causal pathways that may affect it. A substantial body of research has shown an association between career mobility and mental health in later life through health selection and causal processes, with favourable health outcomes associated with upward mobility. However, gender differences in these associations have been largely overlooked, despite evidence of gender inequalities in career opportunities. We use data on individuals aged 55-69 from the CONSTANCES cohort.
