Article Synopsis
  • The study investigates ultrasound features of adnexal masses that are challenging to classify as benign or malignant, using various ultrasound scoring systems.
  • A total of 133 adnexal masses were analyzed, revealing that 66.2% were benign and 33.8% malignant, with ultrasound assessments correctly identifying malignant lesions in all cases.
  • Some borderline tumors and specific malignant types were not consistently detected across ultrasound scores, leading to misclassification of several benign lesions due to certain features like solid areas or papillae.

Background: Several ultrasound (US) features help ultrasound experts in the classification of benign vs. malignant adnexal masses. US scores serve in this differentiation, but they all have misdiagnoses.

Background: Polypharmacy can complicate the course and management of chronic diseases, and has been little explored in patients with inflammatory bowel disease (IBD) to date.

Aim: The aim of this study was to determine the prevalence of polypharmacy in a series of IBD patients, describing associated factors and its correlation with poor disease outcomes.

Materials And Methods: Retrospective study of a single-center series.

Tyrosine Kinase Inhibition: a New Perspective in the Fight against HIV.

Curr HIV/AIDS Rep

October 2019

AIDS Immunopathology Unit, National Center of Microbiology, Instituto de Salud Carlos III, Ctra. Majadahonda-Pozuelo km2, 28220, Madrid, Spain.

Purpose Of Review: HIV-1 infection is incurable due to the existence of latent reservoirs that persist in the face of cART. In this review, we describe the existence of multiple HIV-1 reservoirs, the mechanisms that support their persistence, and the potential use of tyrosine kinase inhibitors (TKIs) to block several pathogenic processes secondary to HIV-1 infection.

Recent Findings: Dasatinib interferes in vitro with HIV-1 persistence by two independent mechanisms.

Article Synopsis
  • Two phase III trials were combined to assess the effectiveness of standard endocrine therapy (ET) versus ET plus bevacizumab (Bev) for hormone receptor-positive metastatic breast cancer patients.
  • The addition of Bev improved progression-free survival (PFS) from 14.3 to 19 months, with higher objective response rates and clinical benefit rates, but did not affect overall survival (OS).
  • However, patients receiving ET + Bev experienced significantly more severe side effects, including hypertension and proteinuria.

Immune checkpoint inhibitors have revolutionized cancer treatment due to their undeniable efficacy, but a range of new adverse events (AE) has emerged. In particular, cardiac toxicity is a potentially fatal AE, and introduces new challenges regarding its underlying molecular mechanisms of occurrence, optimal treatment and follow up, and prevention. We present a clinical case of a patient with advanced kidney cancer treated with nivolumab as a third line treatment.

Sarcomatoid carcinoma is a subtype of non-small cell lung cancer (NSCLC) characterized by mesenchymal - epithelial transition component and awful prognosis. In this report, based on a case of stage IV lung sarcomatoid carcinoma with an extraordinary evolution and survival over 4 years, we address unresolved questions about the treatment of this cancer. We also make a literature review about the key factors that characterize this histology and that should be considered when treating those patients.

Objectives: Malnutrition and sarcopenia frequently coexist in elderly patients. The aim of this study was to assess the effect of nutritional status in the diagnosis of sarcopenia in nursing home residents.

Methods: A cross-sectional study was performed with data collected from 339 elderly patients living in five nursing homes.

Diabetes-specific formulas are an effective alternative for providing nutrients and maintaining glycemic control. This study assesses the effect of treatment with an oral enteral nutrition with a hypercaloric diabetes-specific formula (HDSF) for one year, on health-care resources use, health-care costs, glucose control and nutritional status, in 93 type-2 diabetes mellitus (T2DM) malnourished patients. Changes in health-care resources use and health-care costs were collected the year before and during the year of intervention.

HIV-1 Reservoir Association with Immune Activation.

EBioMedicine

September 2015

Laboratory of Immunovirology, Department of Infectious Diseases, Institute Ramón y Cajal of Health Research (IRyCIS), Universitary Hospital Ramón y Cajal, Madrid, Spain.

In this issue of EBioMedicine, Ruggiero and colleagues describe immune activation biomarkers associated with the size of the HIV reservoir in a carefully designed cross-sectional study. The cohort consists of a homogeneous sample of HIV-1-infected patients with long-term plasma HIV-1 RNA suppression under antiretroviral treatment (ART). It is crucial to explore the potential utility of biomarkers that are easier (less labor intensive, less expensive) to measure than integrated HIV DNA load, in order to quickly and accurately quantify cellular reservoirs of HIV.

Background: Follow-up studies revealed that subjects with borderline personality disorder (BPD) present high rates of clinical remission, although psychosocial functioning often remains impaired. The aim of this study is to evaluate the efficacy of a cognitive rehabilitation intervention versus a psychoeducational program on psychosocial functioning in subjects with BPD.

Methods: A multicenter, randomized, and positive-controlled clinical trial was conducted.

Background: Numerous studies have analyzed the effects of raltegravir intensification on HIV-1 viral replication in infected individuals receiving suppressive combined antiretroviral treatment (cART). Nevertheless, there are only two studies on the effect of raltegravir in HTLV-1 infection, and none in HTLV-2.

Objective: To study the effect of raltegravir on HTLV-2 infection in HIV-1-co-infected individuals.

Patients with advanced non-small-cell lung cancer (NSCLC) carrying epidermal growth factor receptor (EGFR) mutations can now have specific treatment based on the result of biomarker analysis and patients with rearrangements of the anaplastic lymphoma kinase (ALK) gene will probably soon be able to. This will give them better quality of life and progression-free survival than conventional chemotherapy. This consensus statement was conceived as a joint initiative of the Spanish Society of Medical Oncology (SEOM) and the Spanish Society of Pathology (SEAP), and makes diagnostic and treatment recommendations for advanced NSCLC patients based on the scientific evidence on biomarker use.

The gastric antral vascular ectasia (GAVE) is a well recognizable endoscopic entity characterized by the presence of multiple linear angioectatic vessels predominantly located in the antrum, with a typical appearance of "watermelon stomach". This condition typically affects elderly females presenting as iron-deficiency anaemia due to chronic gastric bleeding. Standard treatment is endoscopic ablation of the gastric mucosa.

Background: Extramammary Paget disease (EMPD) is a rare condition that most commonly affects the anogenital region in the elderly. It may be associated with an underlying invasive carcinoma. Surgical excision is nowadays considered a standard treatment for extramammary Paget's disease, although this approach might not be suitable for all patients.

Sunitinib is an orally available small-molecule multikinase inhibitor. This agent potently inhibits the vascular endothelial growth factor receptor, platelet-derived growth factor receptor, and c-Kit in addition to other kinases in biochemical and cell-based assays. In several relevant preclinical cancer models, sunitinib exerts significant antiangiogenesis and antitumor effects.

In the present study we analyzed the anatomical basis for the use of the pectoralis major muscle based on alternative pedicles in order to evaluate its clinical applications in cases of potential lesion of the internal thoracic artery. The patterns of blood supply depending on the internal thoracic artery, previously dissected for coronary by-pass surgery, were studied in both sides of five embalmed cadavers and five anterior thoracic walls taken from autopsies. The secondary vascular pedicles depending on the internal thoracic artery for the pectoralis major muscle were dissected and injected with physiological saline stained solution in the embalmed cadavers.
