Background: For the time being, tafamidis is the only approved treatment for wild-type transthyretin cardiac amyloidosis. However, benefits on all-cause death only emerge after ≈18 months. The current available staging systems are unable to specifically discriminate patients at high risk of death within 18 months from diagnosis, and the selection of patients who are expected to benefit from tafamidis is left to the clinical judgment of treating physicians, being often based primarily (and sometimes only) on age.

November 2024

Laboratory of Innovation in Oncology; Gynecological, Genitourinary and Skin Tumor Unit, HM CIOCC (Clara Campal Comprehensive Cancer Centre), Sanchinarro HM Universitary Hospital, HM Hospitales, Madrid, Spain, Madrid, Spain.

Article Synopsis
  • Precision medicine is changing how we treat cancer, but many patients still lack access to the necessary genetic testing and treatments.
  • A study in Spain examined how providing free RNA sequencing tests impacted the treatment of solid cancer patients who had already failed standard therapies, analyzing samples from 395 patients.
  • The results showed actionable genetic alterations in 14.4% of patients, leading to limited use of targeted therapies, highlighting a need for better access to comprehensive molecular testing and personalized treatments.

Article Synopsis
  • Specific training in colorectal surgery is crucial but currently faces challenges in evaluating its effectiveness and potential benefits.
  • An online survey conducted by the "Grupo Joven de la Asociación Española de Coloproctología" revealed that 84% of 128 participating surgeons reported moderate to high satisfaction with their training, highlighting the need for improvement in advanced surgical techniques and academic knowledge.
  • While trainees successfully performed common procedures, there was a noticeable lack of exposure to pelvic floor surgeries and diagnostic techniques, suggesting a need for further development in these areas to enhance overall training in minimal invasive methods.

Objectives: The management of the facial nerve (FN) is a major issue in parotid cancer, especially when there is no preoperative facial palsy and FN invasion is discovered intraoperatively. The aim of this study was to assess the impact of FN resection in patients with parotid cancer abutting the FN, without pretreatment facial palsy, using a propensity score matching.

Materials And Methods: Data from all patients treated between 2009 and 2020 for a primary parotid cancer abutting or invading the FN but without pretreatment facial palsy were extracted from the national multicentric REFCOR database.

Pseudoaneurysms of the popliteal artery represent a rare vascular pathology. Leaving aside traumatic antecedents, in the presence of sudden swelling of the popliteal region, it is useful to suspect this entity, especially in the presence of infectious processes or connective tissue disorders. We present two cases from our institution where management included surgical intervention and control of the underlying diseases.

Robot-assisted radical prostatectomy (RARP) is a complex surgery with a steep learning curve (LC). No clear evidence exists for how previous laparoscopic experience affects the RARP LC. We report the LC of three surgeons with vast experience in laparoscopy (more than 400 procedures), analyzing the results of functional and oncological outcomes under the "Trifecta" concept (defined as the achievement of continence, potency, and oncological control free of biochemical recurrence).

Article Synopsis
  • The rise of ultra-minimally invasive endoscopic spine surgery is changing how patients with heart conditions are assessed before elective spine surgeries, as it leads to shorter surgery times and less trauma.
  • Traditional extensive cardiac evaluations and medication stoppage are becoming less necessary due to the safer profile of these newer surgical techniques.
  • The article calls for updated guidelines to improve patient safety and surgical outcomes, urging future research on evolving preoperative protocols for patients with cardiac comorbidities.

Article Synopsis
  • Researchers looked at how bleeding in the brain (intracerebral hemorrhage or ICH) affects people who had a specific treatment for severe strokes called endovascular thrombectomy (EVT).
  • Out of the 351 patients studied, many experienced bleeding, especially those who had EVT, but serious types of bleeding were rare.
  • In the end, having some bleeding didn’t make the patients’ health outcomes worse, and there may be new treatments that could help those with bleeding issues in similar cases.

Diagnosis and clinical management of thrombotic thrombocytopenic purpura (TTP): a consensus statement from the TTP Catalan group.

Blood Transfus

March 2024

Medical Direction, Banc de Sang i Teixits, Càtedra UAB de Medicina Transfusional i Teràpia Cel·lular i Tissular, Barcelona, Catalonia, Spain.

Thrombotic thrombocytopenic purpura (TTP) is a low prevalence disease characterized by severe deficiency of the enzyme ADAMTS13, leading to the development of thrombotic microangiopathy (TMA) and often resulting in severe organ disfunction. TTP is an extremely serious condition and, therefore, timely and appropriate treatment is critical to prevent life-threatening complications.Over the past 25 years, significant advances in the understanding of the pathophysiology of immune TTP have led to the development of readily available techniques for measuring ADAMTS13 levels, as well as new drugs that are particularly effective in the acute phase and in preventing relapses.

Background: Few studies have evaluated the long-term impact on health-related quality of life (HRQoL) in patients who have been hospitalized for COVID-19 pneumonia. Specific follow-up should be carried out to detect and treat possible pulmonary abnormalities, and the worsening of HRQoL should be estimated to target necessary resources for care of these patients after acute phase. The objective was to know the impact on HRQoL of patients who have been admitted for COVID-19 pneumonia, and to evaluate the clinical-radiological and functional changes of patients who have overcome COVID-19 pneumonia at 3 and 10 months of follow-up.

Background: Metastatic breast cancer (mBC) causes nearly all BC-related deaths. Next-generation sequencing (NGS) technologies allow for the application of personalized medicine using targeted therapies that could improve patients' outcomes. However, NGS is not routinely used in the clinical practice and its cost induces access-inequity among patients.
