Context: The utility of thyroglobulin (Tg) in the follow-up of differentiated thyroid cancer (DTC) patients has been well-documented. Although third-generation immunoassays have improved accuracy, limitations persist (interfering anti-Tg antibodies and measurement variability). Evolving treatment strategies require a reevaluation of Tg thresholds for optimal patient management.

Background: Adding ibrutinib to standard immunochemotherapy might improve outcomes and challenge autologous stem-cell transplantation (ASCT) in younger (aged 65 years or younger) mantle cell lymphoma patients. This trial aimed to investigate whether the addition of ibrutinib results in a superior clinical outcome compared with the pre-trial immunochemotherapy standard with ASCT or an ibrutinib-containing treatment without ASCT. We also investigated whether standard treatment with ASCT is superior to a treatment adding ibrutinib but without ASCT.

Neoadjuvant chemotherapy plus radical surgery could be a safe alternative to chemo-radiation in cervical cancer patients who are not willing to receive radiotherapy. The response to neoadjuvant chemotherapy is the main factor influencing the need for adjunctive treatments and survival. In the present paper we aim to develop a machine learning model based on cervix magnetic resonance imaging (MRI) images to stratify the single-subject risk of cervical cancer.

A Prospective Multicenter Study Examining the Relationship Between Thyroid Cancer Treatment Outcomes and the Presence of Autoimmune Thyroiditis.

Thyroid

November 2023

Endocrine Oncology Unit, Department of Endocrine and Metabolic Diseases, IRCCS Istituto Auxologico Italiano, Milan, Italy.

There is some controversy on the potential relationship between autoimmune processes and clinicopathologic features as well as prognosis of differentiated thyroid cancer (DTC), and the evidence is limited by its largely retrospective nature. We examined the relationship between the presence of autoimmune thyroiditis (AT) and 1-year thyroid cancer treatment outcomes in a large multicenter study using prospectively collected data. We included data from consecutive DTC patients enrolled in the Italian Thyroid Cancer Observatory (ITCO) database (NCT04031339).

Article Synopsis
  • Recent research highlights the importance of improving risk stratification for patients with differentiated thyroid cancer (DTC) beyond the existing 2015 American Thyroid Association (ATA) guidelines.
  • A study utilizing the Italian Thyroid Cancer Observatory database analyzed 4,773 DTC cases to develop a decision tree model that predicts persistent/recurrent disease by considering a range of predictive factors.
  • The new model demonstrated better performance than the ATA guidelines by increasing sensitivity and predictive accuracy, suggesting that incorporating additional variables like age and tumor size can enhance patient risk assessment.

Encephalopathy related to Status Epilepticus during slow Sleep (ESES) is a childhood epilepsy syndrome characterized by the appearance of cognitive, behavioral, and motor disturbances in conjunction with a striking activation of EEG epileptic abnormalities during non-REM sleep. After more than 50 years since the first description, the pathophysiological mechanisms underlying the appearance of encephalopathy in association with a sleep-related enhancement of epileptic discharges are incompletely elucidated. Recent experimental data support the hypothesis that the development of the ESES encephalopathic picture depends on a spike-induced impairment of the synaptic homeostasis processes occurring during normal sleep and that is particularly pronounced during the developmental age.

The standard treatment for young patients with untreated PTCLs is based on anthracycline containing-regimens followed by high-dose-chemotherapy and stem-cell-transplantation (HDT + SCT), but only 40% of them can be cured. Romidepsin, a histone-deacetylase inhibitor, showed promising activity in relapsed PTCLs; in first line, Romidepsin was added with CHOP. We designed a study combining romidepsin and CHOEP as induction before HDT + auto-SCT in untreated PTCLs (PTCL-NOS, AITL/THF, ALK-ALCL), aged 18-65 years.

April 2022

U.O.C. di Neuropsichiatria Infantile, Dipartimento ad Attività Integrata Materno Infantile - AOUI di Verona, Verona, Italy, Centro Ricerca per le Epilessie in età Pediatrica (CREP), Azienda Ospedaliera Universitaria di Verona, Verona, Italy, Child Neuropsychiatry, Department of Surgical Sciences, Dentistry, Gynecology and Pediatrics, University of Verona, Verona, Italy.

Article Synopsis
  • Febrile status epilepticus (SE) can develop from febrile seizures in about 5% of cases, but identifying it can be difficult, particularly when there are no clear motor symptoms.
  • In a study of 18 children who experienced non-convulsive febrile status epilepticus (NCFSE) after a febrile seizure, common characteristics included altered awareness, abnormal posturing, and certain EEG patterns observed shortly after the seizure.
  • Most children showed normal development and EEG results after treatment, although some had learning disabilities, and a few experienced relapses of NCFSE without progressing to afebrile seizures.

Article Synopsis
  • The study investigates the outcomes of two surgical strategies for treating acute Type A aortic dissection (A-AAD): limited repair (ascending aorta/hemiarch) versus extended repair (including aortic arch).
  • The analysis includes 213 patients from 2006 to 2020, comparing early and late survival rates, and complications between the two groups over two eras.
  • Results show that arch replacement leads to better long-term survival and lower complications without increasing mortality, suggesting it should be considered for A-AAD treatment in skilled medical centers.
