Sarcoidosis is an inflammatory disease known to be associated with multiple autoimmune disorders. There is a restricted number of descriptions of the association between sarcoidosis and autoimmune thrombotic thrombocytopenic purpura (aTTP). We present the case of a 63-year-old woman admitted to the hospital to investigate a possible sarcoidosis who had hemolytic anemia and thrombocytopenia, with low ADAMTS13 activity and anti-ADAMTS13 antibodies, leading to a diagnosis of aTTP.

Article Synopsis
  • Moderately effective therapies (METs) have been the standard treatment for pediatric-onset multiple sclerosis (POMS), but there's still no clear consensus on treatment strategies as highly effective therapies (HETs) emerge.
  • The study aimed to evaluate the effectiveness of HET compared to MET in reducing disease activity in treatment-naive children with relapsing-remitting POMS, using a retrospective cohort design over a median follow-up of 5.8 years.
  • Results from 530 included patients indicated that both HET and MET reduced the risk of relapse within the first 2 years, with HET showing a significant 54% decrease in first relapses compared to MET.

Application of PET imaging delta radiomics for predicting progression-free survival in rare high-grade glioma.

Sci Rep

February 2024

Imagerie Adaptative Diagnostique et Interventionnelle, Institut National de la Santé et de la Recherche Médicale U1254, Université de Lorraine, 54000, Nancy, France.

This study assesses the feasibility of using a sample-efficient model to investigate radiomics changes over time for predicting progression-free survival in rare diseases. Eighteen high-grade glioma patients underwent two L-3,4-dihydroxy-6-[F]-fluoro-phenylalanine positron emission tomography (PET) dynamic scans: the first during treatment and the second at temozolomide chemotherapy discontinuation. Radiomics features from static/dynamic parametric images, alongside conventional features, were extracted.

Sudden cardiac death (SCD) and ventricular fibrillation are rare but severe complications of many cardiovascular diseases and represent a major health issue worldwide. Although the primary causes are often acute or chronic coronary diseases, genetic conditions, such as inherited channelopathies or non-ischemic cardiomyopathies are leading causes of SCD among the young. However, relevant experimental models to study the underlying mechanisms of arrhythmias and develop new therapies are still needed.

Background And Aims: Inflammation is a risk factor for major adverse cardiovascular events (MACE). Elevated levels of both high-sensitivity C-reactive protein (hsCRP) and interleukin-6 (IL6) have been associated with MACE. However, few studies have compared IL6 to hsCRP for cardiovascular risk assessment.

Use of the Win Ratio Analysis in Critical Care Trials.

Am J Respir Crit Care Med

April 2024

Université de Lorraine, Centre d'Investigations Cliniques Plurithématique, Institut National de la Santé et de la Recherche Médicale U1116, Nancy, France.

Article Synopsis
  • Composite outcomes in critical care trials often treat all events equally, which can misrepresent their clinical importance; for example, death is more significant than organ failure.
  • The win ratio (WR) method offers an alternative way to evaluate these outcomes by prioritizing events based on their clinical relevance, making it useful for trials with smaller sample sizes common in critical care.
  • While WR has potential benefits for more accurately assessing treatment effects, it also faces challenges like ignoring tied outcomes and difficulties in interpreting clinical significance, which are discussed alongside its application to recent trials like HYPO-ECMO.

Multiomics Analysis Provides Novel Pathways Related to Progression of Heart Failure.

J Am Coll Cardiol

November 2023

Department of Cardiology, University Medical Center Groningen, University of Groningen, Groningen, the Netherlands.

Article Synopsis
  • Major advances in heart failure treatment haven't significantly reduced mortality, indicating that current therapies might be missing key biological pathways.
  • A study integrated genetic and molecular data from over 2,500 heart failure patients to identify critical pathways linked to mortality, validating the results with a separate group of nearly 1,800 patients.
  • Four major pathways associated with higher death rates were found: PI3K/Akt, MAPK, Ras signaling, and resistance to epidermal growth factor inhibitor, which are linked to reduced activation of the protective ERBB2 receptor influenced by neuregulin.
