Motor-evoked potentials as biomarkers for sexual arousal?

J Sex Med

October 2024

Department of Psychiatry and Psychotherapy, University of Regensburg, Universitaetsstrasse 84, Regensburg 93053, Germany.

Article Synopsis
  • This study explores how lying and sexual arousal affect motor cortex excitability in heterosexual men and women using motor-evoked potentials (MEPs) while they view arousing images.* -
  • Sixteen heterosexual couples participated in a series of trials where they viewed suggestive images and had to answer questions either truthfully or dishonestly regarding their preferences.* -
  • Results indicated that while both men and women showed higher excitability towards their preferred sexual stimuli, lying increased cortical excitability; however, MEPs are not reliable indicators for diagnosing sexual preference or related conditions.*

Background: Low frequency repetitive transcranial magnetic stimulation (rTMS) is commonly used to inhibit pathological hyperactivity of the auditory cortex in tinnitus. Novel and supposedly superior and faster inhibitory protocols such as continuous theta burst stimulation (cTBS) were examined as well, but so far there is not sufficient evidence for a treatment application in chronic tinnitus. rTMS effects in general are dependent on the brain state immediate before stimulation.

Background: Tinnitus represents a relatively common condition in the global population accompanied by various comorbidities and severe burden in many cases. Nevertheless, there is currently no general treatment or cure, presumable due to the heterogeneity of tinnitus with its wide variety of etiologies and tinnitus phenotypes. Hence, most treatment studies merely demonstrated improvement in a subgroup of tinnitus patients.

Myasthenic crisis (MC) is a life-threatening condition for patients with myasthenia gravis (MG). Muscle-specific kinase-antibodies (MuSK-ABs) are detected in ~ 6% of MG, but data on outcome of MuSK-MCs are still lacking. We made a subgroup analysis of patients who presented with MC with either acetylcholine-receptor-antibody positive MG (AchR-MG) or MuSK-MG between 2006 and 2015 in a retrospective German multicenter study.

Lumbar puncture (LP) is commonly used in the diagnostic workup of neurological patients, often to exclude inflammatory diseases of the central nervous system. In clinical practice, an increase of white blood cell count (WBC) in the cerebrospinal fluid (CSF) after a LP is often assumed as reactive to the first puncture. Scientific evidence of this hypothesis, however, is lacking.

The neural basis of sex differences in sexual behavior: A quantitative meta-analysis.

Front Neuroendocrinol

October 2016

Research Centre Jülich, Institute of Neuroscience and Medicine (INM-1), Wilhelm-Johnen-Strasse, 52428 Jülich, Germany; Heinrich Heine University, Institute of Clinical Neuroscience and Medical Psychology, Universitaetsstrasse 1, 40225 Düsseldorf, Germany.

Sexuality as to its etymology presupposes the duality of sexes. Using quantitative neuroimaging meta-analyses, we demonstrate robust sex differences in the neural processing of sexual stimuli in thalamus, hypothalamus, and basal ganglia. In a narrative review, we show how these relate to the well-established sex differences on the behavioral level.

A neural circuit encoding sexual preference in humans.

Neurosci Biobehav Rev

Heinrich Heine University, Institute of Clinical Neuroscience and Medical Psychology, Universitaetsstrasse 1, 40225 Düsseldorf, Germany; Research Centre Jülich, Institute of Neuroscience and Medicine (INM-1), Wilhelm-Johnen-Strasse, 52428, Jülich, Germany.

Sexual preference determines mate choice for reproduction and hence guarantees conservation of species in mammals. Despite this fundamental role in human behavior, current knowledge on its target-specific neurofunctional substrate is based on lesion studies and therefore limited. We used meta-analytic remodeling of neuroimaging data from 364 human subjects with diverse sexual interests during sexual stimulation to quantify neural regions associated with sexual preference manipulations.

TMS-related potentials and artifacts in combined TMS-EEG measurements: Comparison of three different TMS devices.

Neurophysiol Clin

May 2015

University of Regensburg, Department of Psychiatry and Psychotherapy, Universitaetsstrasse 84, 93053 Regensburg, Germany. Electronic address:

Objectives: Simultaneous use of transcranial magnetic stimulation (TMS) and electroencephalography (EEG) allows the measurement of TMS-induced cortical activity. A challenge in the interpretation of the cortical responses to TMS pulses is the differentiation between stimulation artifacts and cortical signals. Thus, we investigated TMS-evoked potentials and artifacts with respect to different TMS devices.

October 2014

Department of Psychiatry and Psychotherapy, University of Regensburg, Universitaetsstraße 84, 93053 Regensburg, Germany ; Interdisciplinary Tinnitus Center, University of Regensburg, Universitaetsstraße 84, 93053 Regensburg, Germany.

Repetitive transcranial magnetic stimulation (rTMS) of the temporal cortex has been used to treat patients with subjective tinnitus. While rTMS is known to induce morphological changes in healthy subjects, no study has investigated yet whether rTMS treatment induces grey matter (GM) changes in tinnitus patients as well, whether these changes are correlated with treatment success, and whether GM at baseline is a useful predictor for treatment outcome. Therefore, we examined magnetic resonance images of 77 tinnitus patients who were treated with rTMS of the left temporal cortex (10 days, 2000 stimuli/day, 1 Hz).

Reduced variability of auditory alpha activity in chronic tinnitus.

Neural Plast

October 2014

Department of Psychiatry and Psychotherapy, University of Regensburg, Universitaetsstrasse 84, 93053 Regensburg, Germany.

Subjective tinnitus is characterized by the conscious perception of a phantom sound which is usually more prominent under silence. Resting state recordings without any auditory stimulation demonstrated a decrease of cortical alpha activity in temporal areas of subjects with an ongoing tinnitus perception. This is often interpreted as an indicator for enhanced excitability of the auditory cortex in tinnitus.

Antipsychotic treatment with quetiapine increases the cortical silent period.

Schizophr Res

June 2014

Department of Psychiatry and Psychotherapy, University of Regensburg, Universitaetsstraße 84, D-93053 Regensburg, Germany. Electronic address:

Rationale: Measurement of motor cortex excitability with single and paired pulse transcranial magnetic stimulation has become an established method for in vivo characterization of the effects of central-acting drugs. The comparison of drug-free and medicated patients with schizophrenia suggests an association of neuroleptics intake and prolongation of the cortical silent period (CSP). However all available data come from cross-sectional non-randomized studies.

Background: Several years ago, repetitive transcranial magnetic stimulation (rTMS) of the auditory cortex has been introduced as a treatment approach for chronic tinnitus. Even if this treatment is beneficial for a subgroup of patients, the overall effects are limited. This limitation may be due to the fact that the auditory cortex is only one of several brain areas involved in tinnitus.

The immune pathogenesis of multiple sclerosis.

J Neuroimmune Pharmacol

September 2013

Department of Neurology, University of Regensburg, Universitaetsstrasse 84, 93053 Regensburg, Germany.

Multiple sclerosis is an autoimmune disease of the central nervous system (CNS) that leads to changes of nerve conduction due to damage of CNS- resident cells, primarily oligodendrocytes and neurons. CD4+ T cells are of primary importance in the immune cascades leading to tissue damage, but also CD8+ T cells, NK cells and B cells and antibodies contribute to tissue damage. In addition, the innate immune response and mainly microglial cells participate in the events leading to lesions.

Low-frequency repetitive transcranial magnetic stimulation (rTMS) of the auditory cortex has been shown to significantly reduce tinnitus severity in some patients. There is growing evidence that a neural network of both auditory and non-auditory cortical areas is involved in the pathophysiology of chronic subjective tinnitus. Targeting several core regions of this network by rTMS might constitute a promising strategy to enhance treatment effects.

Sexually dimorphic effects of oxytocin receptor gene (OXTR ) variants on Harm Avoidance.

Biol Sex Differ

July 2012

Department of Psychiatry and Psychotherapy, University of Regensburg, Universitaetsstrasse 84, 93053 Regensburg, Germany.

Background: Recent research has suggested that oxytocin receptor gene (OXTR) variants may account for individual differences in social behavior, the effects of stress and parenting styles. Little is known, however, on a putative role of the gene in heritable temperamental traits.

Methods: We addressed effects of two common OXTR variants, rs237900 and rs237902, on personality dimensions in 99 healthy subjects using the Temperament and Character Inventory.

Objective: It is a matter of debate whether patients with primary insomnia require a polysomnographic examination in order to exclude specific sleep disorders such as sleep apnea syndrome (SAS) or periodic limb movements (PLM). Using a prospective design, we investigated the prevalence of organic sleep disorders by means of polysomnography (PSG) in a series of patients who were previously diagnosed with primary insomnia. This diagnosis was based on a clinical exam and an ambulatory monitoring device or previous PSG.

Tinnitus is the perception of sound in the absence of an environmental sound source. Abnormal activity in central auditory pathways is considered as the neuronal correlate of tinnitus. However, there is increasing evidence from neuroimaging studies for an additional involvement of the frontal cortex in the pathophysiology of tinnitus, especially concerning its attentional and emotional aspects.
