Article Synopsis
  • ESBL-producing Enterobacterales (ESBL-PE) are common in long-term care facilities (LTCFs), prompting a study across six sites in Europe to assess how residents acquire these bacteria and the associated risk factors.
  • Over 32 weeks, researchers screened 299 residents and found that 16.4% were colonized at the start, with a new acquisition rate of 0.79 per 1000 resident-days, influenced by factors like age, vascular disease, and antibiotic use.
  • Key findings highlight the importance of infection control measures, such as ensuring hand sanitizers and adequate nurse staffing, as well as using genomic surveillance to inform strategies for managing ESBL-PE in LTCFs.

Background: Genetically immunodeficient mice lacking Il2rg and Rag2 genes have been widely utilized in the field of biomedical research. However, immunodeficient rats, which offer the advantage of larger size, have not been as extensively used to date. Recently, Severe Combined Immunodeficiency (SCID) rats were generated using CRISPR/Cas9 system, targeting Il2rg and Rag2 in National BioResource Project in Japan.

Pentavalent rotavirus vaccine (RV5) coverage and changes in rotavirus detection in the prevaccine and postvaccine era in Chengdu were investigated. The results showed that the coverage of RV5 had been increasing but still relatively low. Nevertheless, the dramatical decline in the rotavirus detection was observed after the introduction of RV5.

Making a difference: 5 years of Cardiac Surgery Intersociety Alliance (CSIA).

Asian Cardiovasc Thorac Ann

June 2024

Brigham and Women's Hosp. and Mass General Hospital, Harvard University, Boston, MA, USA.

Informed by the almost unimaginable unmet need for cardiac surgery in the developing regions of the world, leading surgeons, cardiologists, editors in chief of the major cardiothoracic journals as well as representatives of medical industry and government convened in December 2017 to address this unacceptable disparity in access to care. The ensuing "Cape Town Declaration" constituted a clarion call to cardiac surgical societies to jointly advocate the strengthening of sustainable, local cardiac surgical capacity in the developing world. The Cardiac Surgery Intersociety Alliance (CSIA) was thus created, comprising The Society of Thoracic Surgeons (STS), the American Association for Thoracic Surgery (AATS), the Asian Society for Cardiovascular and Thoracic Surgery (ASCVTS), the European Association for Cardio-Thoracic Surgery (EACTS) and the World Heart Federation (WHF).

Article Synopsis
  • This study investigates recent trends in surgical treatment for metastatic spinal tumors from 2012 to 2020 using a national database, highlighting an increase in surgical procedures, particularly fusion surgeries.
  • Analyzed data from over 10,000 patients showed that surgical treatments increased 1.68 times, while in-hospital mortality and length of stay improved significantly.
  • The findings suggest that advancements in cancer treatment and surgical techniques over the past decade may be contributing to these positive trends in patient outcomes.

Recently, in the past decade, high-frequency oscillations (HFOs), very high-frequency oscillations (VHFOs), and ultra-fast oscillations (UFOs) were reported in epileptic patients with drug-resistant epilepsy. However, to this day, the physiological origin of these events has yet to be understood. Our study establishes a mathematical framework based on bifurcation theory for investigating the occurrence of VHFOs and UFOs in depth EEG signals of patients with focal epilepsy, focusing on the potential role of reduced connection strength between neurons in an epileptic focus.

  • The review outlines neuroprotection trials for multiple system atrophy (MSA), a serious neurodegenerative disease, and discusses important factors for conducting successful clinical trials, such as diagnosis and study design.
  • Over 30 compounds have been evaluated in MSA trials, but only two have been successful in meeting their primary endpoint, indicating a need for better understanding of the disease.
  • Current trials focus on targeting α-synuclein, a key feature of MSA, and emphasize the necessity of improving disease models, clinical measurement tools, and biomarker research to enhance future trial outcomes.

Long-term outcome of kidney function in patients with ANCA-associated vasculitis.

Nephrol Dial Transplant

August 2024

Department of Clinical Sciences Lund, Division of Nephrology, Lund University, Lund, Sweden.

Background: Kidney involvement is common in anti-neutrophil cytoplasm antibody-associated vasculitis (AAV) and the prognosis is determined by the severity of kidney damage. This study focused on long-term kidney outcomes, defining possible risk factors and comparing the performance of three different histological classifications to predict outcomes for patients with AAV.

Methods: The dataset included 848 patients with newly diagnosed AAV who participated in seven randomized controlled trials (RCTs) (1995-2012).

Biallelic inactivation of the NF1 tumour suppressor gene in juvenile myelomonocytic leukaemia: Genetic evidence of driver function and implications for diagnostic workup.

Br J Haematol

February 2024

Division of Pediatric Hematology and Oncology, Department of Pediatrics and Adolescent Medicine, Medical Center, Faculty of Medicine, University of Freiburg, Freiburg, Germany.

Article Synopsis
  • Juvenile myelomonocytic leukaemia (JMML) is a type of cancer that occurs in children and is linked to problems with certain genes in the RAS pathway.
  • Kids with a condition called neurofibromatosis type 1 (NF-1) are more likely to get JMML because of a faulty NF1 gene they inherit.
  • The study looked at 25 JMML patients, finding different types of NF1 gene problems and suggesting that testing for NF1 gene issues is important even if patients don’t show signs of NF-1.

Objectives: To evaluate the safety and efficacy of remibrutinib in patients with moderate-to-severe Sjögren's syndrome (SjS) in a phase 2 randomised, double-blind trial (NCT04035668; LOUiSSE (LOU064 in Sjögren's Syndrome) study).

Methods: Eligible patients fulfilling 2016 American College of Rheumatology/European League Against Rheumatism (EULAR) criteria for SjS, positive for anti-Ro/Sjögren's syndrome-related antigen A antibodies, with moderate-to-severe disease activity (EULAR Sjögren's Syndrome Disease Activity Index (ESSDAI) (based on weighted score) ≥ 5, EULAR Sjögren's Syndrome Patient Reported Index (ESSPRI) ≥ 5) received remibrutinib (100 mg) either one or two times a day, or placebo for the 24-week study treatment period. The primary endpoint was change from baseline in ESSDAI at week 24.

Article Synopsis
  • The study investigates how thymic dysplasia affects T cells in patients with 22q11.2 deletion syndrome, focusing on their characteristics and response to IL-7 treatment.
  • Findings show a strong Th1 response in these patients, characterized by specific T cell expansions and increased production of various cytokines like IFN-γ and IL-10.
  • IL-7 treatment leads to a reduction in naive T cells and an increase in exhaustion markers, indicating that the Th1 bias remains through adolescence and promotes early maturation of T cells.

Introduction: The proportion of older adults within society is sharply increasing and a better understanding of how we age starts to be critical. However, given the paucity of longitudinal studies with both neuroimaging and epigenetic data, it remains largely unknown whether the speed of the epigenetic clock changes over the life course and whether any such changes are proportional to changes in brain aging and cognitive skills. To fill these knowledge gaps, we conducted a longitudinal study of a prenatal birth cohort, studied epigenetic aging across adolescence and young adulthood, and evaluated its relationship with brain aging and cognitive outcomes.
