[The effects of implanted valve sizes on ventricular hypertrophy in aortic stenosis].

Objective: We aimed to study the effects of the valve sizes according to body surface area on aortic gradient and ventricular hypertrophy in the cases of aortic valve replacement due to isolated aortic stenosis.

Congenital coronary-to-pulmonary artery fistulas are rare anomalies and they generally have an asymptomatic course. We presented three symptomatic patients (2 men, 1 woman; age range 46 to 53 years) who underwent surgical repair via the epicardial approach for coronary-to-pulmonary artery fistulas. Thromboelastography is an alternative method to conventional coagulation tests for the general evaluation of hemostatic system. Cardiac surgery with cardiopulmonary bypass is accomplished by complex alterations of hemostasis, including acquired dysfunction of platelets, consumption coagulopathy and increased fibrinolysis. Despite major advances in blood conservation methods and perioperative care of the patients, transfusion rates in cardiac surgery remain high.

A sixty-six year-old male patient with the complaint of palpitation presented at the out-patient clinic of the thorax diseases department. The patient underwent open heart surgery (cystectomy and capitonage) for the cystic hydatidosis which was located on the front wall of the right atrium and extended to the atrioventricular cleavage. An increasing number of patients are found to be unsuitable for revascularization by conventional approaches to the epicardial vessels. Transmyocardial laser revascularization (TMR) is a treatment choice for patients who have refractory angina pectoris when bypass surgery or percutaneous transluminal angioplasty is not indicated. Transmyocardial laser revascularization leads to significant improvements in angina.

Graft occlusions are the main problems that may arise during long-term follow-up period after coronary artery bypass surgery. Knowledge of pathologies developed in saphenous grafts and attempts to reduce their frequency are important for reduction of the incidence of saphenous graft vein occlusions. For today the patency rate for 10 years saphenous vein grafts are about 60%.
