The disadvantaged populations eGFR (estimated glomerular filtration rate) epidemiology (DEGREE) study was designed to gain insight into the burden of chronic kidney disease (CKD) of undetermined cause (CKDu) using standard protocols to estimate the general-population prevalence of low eGFR internationally. Therefore, we estimated the age-standardized prevalence of eGFR under 60 ml/min per 1.73m in adults aged 18-60, excluding participants with commonly known causes of CKD; an ACR (albumin/creatinine ratio) over 300 mg/g or equivalent, or self-reported or measured (HT) hypertension or (DM) diabetes mellitus, stratified by sex and location.

Article Synopsis
  • The DEGREE study explored the impact of chronic kidney disease of undetermined cause (CKDu) globally by analyzing the prevalence of low estimated glomerular filtration rate (eGFR) in adults aged 18-60, focusing on participants without known CKD causes.
  • The study included data from 60,964 participants across 43 areas in 14 countries collected from 2007 to 2023, revealing the highest CKDu prevalence in rural areas of Uddanam, India, and Northwest Nicaragua (14%).
  • The findings indicate that CKDu is notably prevalent in specific regions, especially within Central America and South Asia, prompting the need for global monitoring while acknowledging the potential existence of unidentified disease clusters elsewhere.

Introduction: Health, safety, and well-being training programs provide essential education on anticipating, identifying, and mitigating exposures like infectious diseases. Gaps in infectious diseases awareness and education became especially apparent during the COVID-19 pandemic and subsequently were exacerbated by mis- and disinformation.

Methods: Vaccine-preventable infectious diseases training (influenza, hepatitis A and B, and tetanus infections, including COVID-19) was developed, delivered, and evaluated among 1,043 farmworkers, bodega workers, and production management in the Rio Grande Valley using mobile-learning technologies.

Adult-onset asthma, notably prevalent among healthcare professionals, especially nurses, is often attributed to occupational factors such as exposure to cleaning agents. Studies consistently underscore the substantial role of such exposure in work-related asthma among hospital staff. We aimed to (a) identify and characterize current practices in cleaning and aerosolized medication administration; (b) assess changes in practices since a similar 2003 study of Texas healthcare workers; and (c) identify factors contributing to diverse exposures within healthcare job categories.

Article Synopsis
  • HPV is a prevalent sexually transmitted infection linked to various cancers, and vaccination rates are low despite its effectiveness in prevention.
  • One significant factor influencing vaccination uptake is the recommendation from healthcare providers, particularly dental hygienists, who can leverage their position to advocate for HPV vaccination.
  • A study conducted at a dental hygiene conference showed that while a continuing education course improved hygienists' knowledge of HPV, it did not significantly enhance their confidence in counseling or recommending the vaccine, highlighting a need for more effective training tools.

Introduction: Cardiometabolic risk factors and epigenetic patterns, increased in physically inactive individuals, are associated with an accelerated brain aging process.

Objective: To determine whether cardiometabolic risk factors and epigenetic patterns mediate the association of physical inactivity with unfavorable brain morphology.

Methods: We included dementia and stroke free participants from the Framingham Heart Study Third Generation and Offspring cohorts who had accelerometery and brain MRI data (n = 2,507, 53.

Background: Recent evidence suggest that extracorporeal cardiopulmonary resuscitation (ECPR) may improve survival rates for nontraumatic out-of-hospital cardiac arrest (OHCA). Eligibility criteria for ECPR are often based on patient age, clinical variables, and facility capabilities. Expanding access to ECPR across the U.

Article Synopsis
  • Mycobacterium tuberculosis (Mtb) causes around 1.5 million deaths annually, with a significant portion of infections remaining latent rather than developing into active tuberculosis (TB).
  • Research indicates that M1-polarized macrophages (M1-MΦs) can effectively inhibit Mtb in vitro, suggesting their key role in regulating TB immunity, while M2-polarized macrophages (M2-MΦs) allow Mtb proliferation.
  • The findings reveal that M1-MΦs show increased expression of specific regulatory genes that help degrade Mtb and enhance immune response, while M2-MΦs have reduced expression of these genes, highlighting the distinct contributions of different macrophage types to TB control and potential therapeutic

SARS-CoV-2 infection prominently affects the respiratory system, and patients hospitalized with COVID-19 are at an increased risk of developing respiratory conditions. We examined the risk of new respiratory conditions of COVID-19 among hospitalized patients in the national Veterans Health Administration between 15 February 2020 and 16 June 2021. The study cohort included all COVID-19-tested, hospitalized individuals who survived the index admission and did not have any previously diagnosed chronic respiratory conditions (asthma, bronchitis, chronic lung disease, chronic obstructive pulmonary disease (COPD), emphysema, or venous thromboembolism) before SARS-CoV-2 testing.

There is increased interest over the last decade in the use of Shared Decision Making with individuals with serious mental illness to improve engagement in treatment and clinical outcomes. We conducted semi-structured qualitative interviews with 15 individuals with serious mental illness treated in an outpatient transitional care clinic serving people immediately after discharge from a psychiatric hospitalization. Parallel interviews were conducted with a variety of clinical providers (n = 9).

  • The project aims to reduce unnecessary use of antibiotics for a condition called asymptomatic bacteriuria (ASB), which is when people have bacteria in their urine but no symptoms.
  • It was inspired by a previous successful campaign and will be implemented in four different Veterans Health Administration facilities to see what works best.
  • The main methods include creating an easy-to-follow guideline for doctors and providing them feedback on how well they follow it, along with regular assessments to make sure the changes stay effective over time.

In this era of precision oncology, there has been an exponential growth in the armamentarium of genomically targeted therapies and immunotherapies. Evaluating early responses to precision therapy is essential for "go" versus "no go" decisions for these molecularly targeted drugs and agents that arm the immune system. Many different response assessment criteria exist for use in solid tumors and lymphomas.

Objective: Self-persuasion is an effective behavior change strategy, but has not been translated for low-income, less educated, uninsured populations attending safety-net clinics or to promote human papillomavirus (HPV) vaccination. We developed a tablet-based application (in English and Spanish) to elicit parental self-persuasion for adolescent HPV vaccination and evaluated its feasibility in a safety-net population.

Methods: Parents (N=45) of age-eligible adolescents used the self-persuasion application.
