Objective: Antiretroviral therapy (ART) containing integrase inhibitors (INSTIs) and/or tenofovir alafenamide (TAF) has been associated with greater weight gain. Yet few studies have delineated between exposure to 'anchor' drugs [protease inhibitors (PI), nonnucleoside reverse transcriptase inhibitors (NNRTI) or INSTIs] and exposure to nucleoside reverse transcriptase inhibitors (NRTIs).

Design: In this cohort of antiretroviral drug-naive patients who initiated ART from 2008-2022, we analyzed BMI gain for eight contemporary 'anchor' drugs and three contemporary NRTIs during the first 3 years of ART.

Article Synopsis
  • This study analyzed long-term trends and risk factors for non-coronary complications in individuals with type 1 diabetes compared to matched controls using a large dataset from the Swedish National Diabetes Register.
  • Between 2001 and 2019, the incidence rates of complications like large artery disease and diabetic foot syndrome decreased significantly in those with type 1 diabetes, while certain risk factors like education and blood sugar levels had a substantial impact on these outcomes.
  • Interestingly, people with type 1 diabetes who maintained cardiometabolic risk factors at target levels showed lower risks for some complications, but had a significantly higher risk for diabetic foot disease if all risk factors were present.

J Pediatr Gastroenterol Nutr

September 2024

Division of Neonatal-Perinatal Medicine, Department of Pediatrics, UT Southwestern Medical Center, Dallas, Texas, USA.

Objectives: Prolonged antibiotic use after birth is associated with neonatal feeding intolerance and functional gastrointestinal disorders (FGIDs). A gastric dysrhythmia (tachygastria) with frequencies >4-9 cycles per minute, measured by electrogastrography (EGG), is associated with FGIDs. The relationship between prolonged antibiotic use and % time spent in tachygastria is unknown in preterm infants.

Limb Outcomes With Ticagrelor Plus Aspirin in Patients With Diabetes Mellitus and Atherosclerosis.

J Am Coll Cardiol

April 2024

French Alliance for Cardiovascular Trials, Département Hospitalo-Universitaire FIRE, Hôpital Bichat, Université de Paris, INSERM Unité 1148, Paris, France.

Article Synopsis
  • Ticagrelor, when added to aspirin, was found to reduce major adverse cardiovascular events (MACE) but also increased the risk of bleeding in patients with type 2 diabetes mellitus (T2DM) and coronary artery disease.
  • In a study involving 19,220 patients over a median of 3 years, those with peripheral artery disease (PAD) who received the placebo had significantly higher risks of both MACE and limb events compared to those on ticagrelor.
  • While ticagrelor demonstrated a reducing effect on limb events such as revascularization and acute limb ischemia, the overall bleeding risk also increased; further studies are necessary to explore the balance of benefits and risks in various patient groups.

Purpose Of Review: To review ongoing and planned clinical trials of weight loss among individuals with or at high risk of heart failure.

Recent Findings: Intentional weight loss via semaglutide among persons with heart failure and preserved ejection fraction and obesity significantly improves weight loss and health status as assessed by the KCCQ-CSS score and is associated with improvements in 6-min walk test. Ongoing and planned trials will explore the role of intentional weight loss with treatments such as semaglutide or tirzepatide for individuals with heart failure across the entire ejection fraction spectrum.

Effects of PCV10 and PCV13 on pneumococcal serotype 6C disease, carriage, and antimicrobial resistance.

Vaccine

April 2024

Medical Development and Scientific Clinical Affairs, Pfizer Vaccines, 500 Arcola Road, Collegeville, PA 19426, USA. Electronic address:

Background: The cross-protection of pneumococcal conjugate vaccines (PCV) against serotype 6C is not clearly documented, although 6C represents a substantial burden of pneumococcal disease in recent years. A systematic review by the World Health Organization that covered studies through 2016 concluded that available data were insufficient to determine if either PCV10 (which contains serotype 6B but not 6A) or PCV13 (containing serotype 6A and 6B) conferred protection against 6C.

Methods: We performed a systematic review of randomized controlled trials and observational studies published between January 2010 - August 2022 (Medline/Embase), covering the direct, indirect, and overall effect of PCV10 and PCV13 against 6C invasive pneumococcal disease (IPD), non-IPD, nasopharyngeal carriage (NPC), and antimicrobial resistance (AMR).

Background: Hereditary transthyretin cardiac amyloidosis (ATTRv-CA) has a long latency phase before clinical onset, creating a need to identify subclinical disease. We hypothesized circulating transthyretin (TTR) and retinol binding protein 4 (RBP4) levels would be associated with carrier status and correlated with possible evidence of subclinical ATTRv-CA.

Methods: TTR and RBP4 were measured in blood samples from V122I carriers and age-, sex- and race-matched non-carrier controls (1:2 matching) among Dallas Heart Study participants (phases 1 (DHS-1) and 2 (DHS-2)).
