Introduction: The evidence basis for therapy selection in women who have failed primary stress urinary incontinence (SUI) surgery is limited. The ICI-RS group discussed the available data at its meeting in June 2023, particularly the anatomical characteristics as assessed using magnetic resonance imaging (MRI) and ultrasound (US) modalities, functional characteristics associated with storage and voiding urodynamic assessment, as well as the patient characteristics that might influence outcomes. This paper summarizes the evidence base that supported these discussions and offers the basis for research proposals for future groups.

45,X/46,XY mosaicism: Clinical manifestations and long term follow-up.

Am J Med Genet A

March 2024

Division of Clinical and Metabolic Genetics, Department of Pediatrics, The Hospital for Sick Children, University of Toronto, Toronto, Ontario, Canada.

45,X/46,XY chromosomal mosaicism presents a range of clinical manifestations, including phenotypes from Turner syndrome through genital abnormalities to apparently unaffected phenotypic males; however, the full clinical spectrum has not yet been fully delineated since prior studies on the clinical phenotype and associated risk of gonadal tumors included small cohorts and limited follow-up. To better describe the clinical manifestations and long-term outcome of patients with 45,X/46,XY mosaicism. We conducted a retrospective chart review of patients with 45,X/46,XY from three health centers (Hospital for Sick Children and Mount Sinai Hospital in Canada, and University of Pittsburgh Medical Center in United States).

Cervical Cancer Screening: A Review.

JAMA

August 2023

Division of Cancer Epidemiology and Genetics, National Cancer Institute, Bethesda, Maryland.

Importance: Each year in the US, approximately 100 000 people are treated for cervical precancer, 14 000 people are diagnosed with cervical cancer, and 4000 die of cervical cancer.

Observations: Essentially all cervical cancers worldwide are caused by persistent infections with one of 13 carcinogenic human papillomavirus (HPV) genotypes: 16, 18, 31, 33, 35, 39, 45, 51, 52, 56, 58, 59, and 68. HPV vaccination at ages 9 through 12 years will likely prevent more than 90% of cervical precancers and cancers.

Article Synopsis
  • The study looked at two ways to treat a condition called stage I twin-to-twin transfusion syndrome in pregnant women: one way involved a special laser treatment, and the other was just waiting and monitoring.
  • Researchers checked several medical databases to gather information and compared the results from five different studies.
  • They found that there wasn’t a big difference in the survival of the babies between the two treatments and that using the laser treatment might not be necessary if the mother didn't have any serious problems.

Article Synopsis
  • There is growing interest in using AI for mammographic screening, but its performance needs thorough evaluation before it can be used independently for diagnosis.
  • A systematic review analyzed data from 16 studies involving over a million mammogram examinations to compare AI's performance with that of radiologists in interpreting digital mammography and digital breast tomosynthesis (DBT).
  • Results showed that AI had better standalone performance (higher AUC) in most reader studies on digital mammography and in DBT, though its sensitivity was higher and specificity lower compared to radiologists, indicating promise but also some drawbacks in using AI for screening.

Myelodysplastic/ Myeloproliferative neoplasms (MDS/MPN) demonstrate overlapping pathologic and molecular features of myelodysplastic (MDS) and myeloproliferative (MPN) neoplasms. Diagnosis is difficult based on morphology alone, requiring exclusion of various non-neoplastic causes for CBC abnormalities and morphologic findings and other myeloid neoplasms. Identifying a clonal abnormality by cytogenetics or molecular studies has vastly improved our ability to diagnose MDS/MPN and has been incorporated in the different classification schemas.
